AVN of the talus is a challenging disease process with respect to patho-physiology and treatment. We believe that our algorithm is a legitimate approach to aid the orthopedic surgeon in initiating a promising treatment. It is divided into different levels and allows to change between some. This is not the only way to proceed but it seems promising, especially if the long-term results with the vascularized bone grafts show revascularization of the talus. As always in medicine, the treatment needs to be individualized. Arthrodesis always should be the last option and is a challenging procedure.

A 52-year-old man had loss of vision and black discoloration of the lids of the right eye after a retrobulbar injection of 3 mL lidocaine hydrochloride 2% (Xylocaine). Examination of the right eye revealed no light perception with extensive necrosis of the lids. Anterior segment examination revealed conjunctival pallor, corneal edema, and necrosis of the sclera. This is a previously unreported complication of retrobulbar anesthesia comprising ophthalmic artery occlusion with scleral melt, ocular ischemia, and eyelid necrosis.

Endometriosis is a common gynaecological disorder causing pain, infertility, and emotional distress. Evidence presented here suggests that abnormal production of tumour necrosis factor-alpha (TNF-alpha) is required for the establishment and maintenance of endometriosis and also is responsible for the associated infertility through its effect on sperm motility and function and oocyte development. Infliximab, which blocks TNF-alpha function, could be used in the treatment of endometriosis to reverse the above effects.

We describe a 46-year-old woman who presented at intervals of seven years with osteonecrosis of the outer end of both clavicles. The clinical, radiological features and the appearances of the bone scans are described. Although the condition may be confused with osteolysis there is a dear histological distinction between the two conditions. If the symptoms fail to respond to conservative treatment, excision of the outer end of the clavicle is recommended.

Autoimmune hepatitis type 1 in a 8-year old girl is described. The diagnosis was established using International Autoimmune hepatitis group scoring system. This prospective study evaluated the correlation between plain radiographs, magnetic resonance imaging (MRI), and diagnostic arthroscopy in the staging of avascular necrosis of the femoral head. Twenty-two cases of nontraumatic avascular necrosis of the scaphoid are reported. In ten cases a metabolic cause could be revealed; for the other 12 cases no etiology could be established. Twelve were treated with a proximal row carpectomy, ten had a very satisfactory result, one was not satisfied and had to be converted into a wrist arthrodesis and one is in doubt.
