An unexpected life-threatening persistent oxygen desaturation in a child after extubation.

Ureteroscopic nephrolithotripsy is now being preferred over percutaneous nephrolithotomy for removal of kidney stones in children. Here, in this report, we have discussed persistent oxygen desaturation immediately after extubation in a two-year-old child who underwent Ho-YAG (holmium-yttrium-aluminium garnet) laser ureteroscopic nephrolithotripsy. The child developed bilateral pleural effusion after nephrolithotripsy and required continuous oxygen supplementation to maintain oxygen saturation above 95%, followed by ultrasound-guided thoracentesis.

[A case of colonic perforation: a complication of percutaneous nephrostomy managed conservatively].

Percutaneous nephropyelostomy is commonly performed prior to endoscopic procedures, such as percutaneous nephrolithotomy or endopyelotomy. We report a case of colonic perforation, complicating percutaneous nephrostomy, which was managed conservatively. A 10-year-old girl was admitted to our hospital for the diagnosis of ureteropelvic junction obstruction. The first percutaneous nephrostomy with sonographic guidance was performed for evaluating renal function. About three weeks later, the second percutaneous nephrostomy with fluoroscopic guidance was performed for endopyelotomy. The tract was d

The current standing of standard PCNL in the era of miniaturization.

OBJECTIVE: To evaluate how standard operating procedures (SOPs), modification in puncture techniques, advancements in preoperative imaging to better understand calyceal anatomy have revitalized standard percutaneous nephrolithotomy (PCNL) for large kidney stones (>2.5 cm). METHODS: At our center, renal stones are managed with extracorporeal shock wave lithotripsy (ESWL), percutaneous nephrolithotomy (PCNL), and retrograde intrarenal surgery (RIRS) according to stone characteristics and patient profile. Renal stones larger than 2.5 cm are managed with standard PCNL. In this retrospective study

Suction mini-PCNL versus standard PCNL for 2-4 cm renal stones: a prospective cohort analysis of average effects, heterogeneity, and policy value.

We compared 18Fr suction mini-percutaneous nephrolithotomy (SMP) with standard percutaneous nephrolithotomy (sPCNL) for 2-4 cm renal stones in a prospective, single-center, non-randomized cohort and separated three questions: population-average effects, treatment-effect heterogeneity, and the value of acting on that heterogeneity. Among 900 patients, 450 underwent each procedure. Targeted maximum likelihood estimation with cross-validated Super Learner estimated outcome-specific average effects. A Net Clinical Utility (NCU) composite was analyzed with doubly robust estimation, an honest causal

Renal struvite stones--pathogenesis, microbiology, and management strategies.

Infection stones-which account for 10-15% of all urinary calculi-are thought to form in the presence of urease-producing bacteria. These calculi can cause significant morbidity and mortality if left untreated or treated inadequately; optimal treatment involves complete stone eradication in conjunction with antibiotic therapy. The three key principles of treating struvite stones are: removal of all stone fragments, the use of antibiotics to treat the infection, and prevention of recurrence. Several methods to remove stone fragments have been described in the literature, including the use of ure

[Urolithiasis and laparoscopy. Treatment of renal stones in special anatomical and functionnal conditions].

Laparoscopic approach is used to treat urinary lithiasis. Pyelolithotomy and ureterolithtomy being excluded from this review, indications for laparoscopy consist in treatment of calyceal diverticula, nephrectomy and/or ureterectomy for non-functioning kidneys, or laparoscopy-assisted percutaneous nephrolithotomy in ectopic kidney, horseshoe kidney or calyceal diverticula. Laparoscopy gives the benefits of a mini-invasive treatment with reduced blood loss, pain, hospital stay and recovery. In these indications, laparoscopy is still a skillfull demanding procedure.

Squamous cell carcinoma of skin due to prolonged indwelling percutaneous nephrostomy catheter.

Percutaneous nephrostomy (PCN) catheter placement is a commonly performed procedure in the urological practice for various indications like percutaneous nephrolithotomy, pyonephrosis, infected hydronephrosis and after failed attempt of ureteric stenting. The nephrostomy catheter is usually associated with low complication and morbidity rate, but prolonged indwelling nephrostomy tube may be hazardous in some cases. We hereby report a case of squamous cell carcinoma of skin around the PCN, which was attributed to chronic inflammation and persistent irritation from a long-term neglected indwellin

[Treatment of upper tract stones and neurogenic bladder].

Upper tract stones are a frequent complication in neurourology and require specific treatment according to modalities adapted to the clinical setting. All modern treatment modalities for stones can be proposed: extracorporeal lithotripsy (ESWL), percutaneous nephrolithotomy and open surgery depending on the local conditions. The treatment objectives in the neurologically disabled population must be to combine a maximum of fragmentation and elimination in the same treatment session. ESWL therefore has a more limited place in this population. On the other hand, a renal stone measuring 1.5 cm mus

Experience of percutaneous access under ultrasound guidance in renal transplant patients with allograft lithiasis.

OBJECTIVE: Urolithiasis of the transplanted kidney has an incidence of 0.2 to 1.7%, it increases the risk of infection in immunosuppressed patients and it can lead to ureteral obstruction that is often associated with deterioration of renal function. Urolithiasis of the transplanted kidney has different characteristics compared to the native kidney, due to the absence of innervation, which does not lead to colic pain. Percutaneous approach is an optimal choice in transplant patients. MATERIAL AND METHODS: Here we report our experience in two cadaveric transplant patients with urolithiasis. Th

Epidemiology, diagnosis, and surgical management of urolithiasis in older adults: overview from EAU endourology.

PURPOSE OF REVIEW: Population ageing is changing everyday urological practice. The number of older adults is increasing, and urology already treats a patient population that is, on average, older than the general population. Consequently, older adults with urolithiasis represent a core part of contemporary endourological practice. Given this, a focused review of the available evidence is valuable to inform clinical practice. RECENT FINDINGS: A peak in stone disease can occur in older adults who may also be less likely to present with the classical features of renal colic. As such, delayed or

Comprehensive Management of Complex and Refractory Urolithiasis: Minimally Invasive Techniques, Perioperative Optimisation, and Recurrence Prevention.

BACKGROUND: Complex and refractory urolithiasis represents a growing clinical challenge, characterised by a large stone burden, recurrent disease, anatomical abnormalities, infection, and metabolic risk factors, all of which complicate management and increase morbidity. Contemporary management has shifted toward risk-stratified, minimally invasive strategies supported by advances in endourology, perioperative optimisation, and individualised patient assessment. To synthesise current evidence on the diagnostic evaluation, risk stratification, minimally invasive management, perioperative optimis

Clinical Outcomes Comparing Mini Versus Standard PCNL Without Postoperative Nephrostomy Tube: A Multi-institutional Randomized Controlled Trial From the EDGE Consortium.

OBJECTIVE: To investigate whether a smaller tract size for percutaneous nephrolithotomy (PCNL) may result in fewer complications including blood loss, while retaining similar stone-free rates and operative time, utilizing a modern approach without postoperative nephrostomy tube. METHODS: We performed a randomized controlled trial at five participating EDGE institutions in North America. Patients were randomized to receive 17.5Fr miniature tract vs 30Fr standard sized tract for PCNL without postoperative nephrostomy tubes. RESULTS: Demographic variables were similar. Stone fragmentation in percutaneous surgery: is there a paradigm shift?

PURPOSE OF REVIEW: Percutaneous nephrolithotomy (PCNL) remains the gold standard for the treatment of large and complex renal calculi. Traditionally, stone fragmentation during PCNL has relied on mechanical lithotripsy combined with manual or passive fragment extraction. This review examines whether recent advances in stone fragmentation and evacuation technologies represent a true paradigm shift in percutaneous stone surgery or rather a progressive refinement of established principles. RECENT FINDINGS: Recent literature highlights a progressive transition from conventional mechanical lithotr

Intrarenal antimicrobial irrigation as an adjunct to systemic prophylaxis in upper tract endourology: a PRISMA-ScR scoping review of a limited and heterogeneous evidence base.

PURPOSE: This review maps evidence for adding antimicrobial or antiseptic agents to intrarenal irrigation during upper tract endourology and the physiological rationale linking intrarenal pressure to septic complications. METHODS: Following PRISMA-ScR guidelines, we searched PubMed and Scopus (see Methods for dates) for clinical outcomes of antimicrobial or antiseptic irrigation during retrograde intrarenal surgery (RIRS) or percutaneous nephrolithotomy (PCNL) in adults. Given anticipated heterogeneity, findings were synthesized narratively without meta-analysis. RESULTS: Four heterogeneous

Bilateral duplex kidney and ureter with multiple stones: a case report.

BACKGROUND: Bilateral duplicated kidney and ureter is a rare condition in urology, and it is even rarer for patients to have multiple stones simultaneously. We delineate the diagnostic and therapeutic trajectory of a patient presenting with bilateral duplex kidneys and ureters, characterized by the presence of multiple stones. Notably, the left kidney is a complete duplication, whereas the right kidney exhibits an incomplete duplication. CASE PRESENTATION: A 47-year-old male patient was diagnosed with bilateral duplex kidney and ureter combined with multiple stones. Ureteral flexible lithotri

Kidney Stones in Several Spinal Abnormalities: A Challenging Treatment.

Patients with severe skeletal deformities are a challenging group to treat. A female, white, 35-year-old presented with right kidney stones located in renal pelvis, lower calyx, and upper ureter. She was affected by severe spinal deformity with restrictive respiratory obstruction, caused by kyphoscoliosis. Percutaneous nephrolithotomy in supine position was performed, achieving complete removal of kidney stones. The treatment of renal stones in this patient was complex, so special attention to respiratory function was mandatory; this was a challenging but feasible situation.
