Transfer of a single fascicle from the ulnar nerve to the biceps muscle after avulsions of upper roots of the brachial plexus.

Thirty-six patients with avulsions of upper roots of the brachial plexus underwent transfer of a single fascicle from the ulnar nerve to the proximal motor branch of the biceps muscle to restore elbow flexion. The mean period of follow-up was 22 months. The average reinnervation time for the biceps muscle was 3.3 months. Thirty-four patients achieved biceps strength of Medical Research Council grade 3 or better. The operative results in the patients with C5, C6 avulsions were better than those with C5, C6, C7 avulsions. At the last follow-up examination, grip strength, pinch strength, moving t

The Effect of Ulnar Nerve Motor Branch Transfer in Cubital Tunnel Syndrome Surgery: A Comparison With In Situ Decompression Alone.

BACKGROUND: Advanced cubital tunnel syndrome causes progressive sensory and motor dysfunction, leading to hand weakness, intrinsic muscle atrophy, and functional impairment. Although surgical decompression relieves mechanical compression, functional recovery in McGowan Stage 3 patients is often limited due to irreversible axonal degeneration and delayed reinnervation of distal muscles. Distal end-to-side anterior interosseous nerve transfer has therefore been proposed as an adjunct to decompression to enhance reinnervation. The aim of this study was to evaluate the effects of this distal nerve…

Combined nerve and tendon transfer (CNaTT) for grasp and release function in patients with tetraplegia: a matched prospective pilot study.

INTRODUCTION: Traditional single-stage tendon transfer procedures in patients with tetraplegia are designed to restore active pinch and grasp function. However, digital extension is usually achieved by wrist tenodesis. More recently, nerve transfer has allowed potential restoration of digital extension. This matched observational pilot study compares clinical outcomes after traditional tendon transfers vs. a study group using two-stage using combined nerve and tendon transfer. METHODS: Eighteen patients in the traditional tendon transfer control group and 18 patients in the combined nerve and…

Selective transfer of nerve to supinator to restore digital extension in central cord syndrome: An anatomical study and a case report.

BACKGROUND: Nerve transfers are increasingly used to restore upper extremity function in patients with spinal cord injury. However, the role of nerve transfers for central cord syndrome is still being established. The purpose of this study is to report the anatomical feasibility and clinical use of nerve transfer of supinator motor branches (NS) to restore finger extension in a central cord syndrome patient. MATERIALS AND METHODS: The posterior interosseous nerve (PIN), its superficial division, and branches were dissected in 14 fresh cadavers, with a mean age of 65 (58-79). Measurements incl…

Transfer of the deep temporal nerve for eyelid reconstruction in Mobius syndrome - an anatomic feasibility study and proposed surgical approach.

Facial paralysis may result in significant functional, esthetic, and psychological morbidity. Mobius syndrome is a form of bilateral congenital facial paralysis that is particularly difficult to treat owing to the lack of readily available donor nerves, particularly in the upper face. In this study, we evaluate the feasibility of using the deep temporal nerves as donors for the innervation of free muscle grafts in the periorbital region. Preserved and fresh cadaver facial halves are dissected, and the course of the deep temporal nerves delineated. We find the middle branch of the deep temporal

Capsaicin-sensitive sensory nerves exert complex regulatory functions in the serum-transfer mouse model of autoimmune arthritis.

OBJECTIVE: The K/BxN serum-transfer arthritis is a widely-used translational mouse model of rheumatoid arthritis, in which the immunological components have thoroughly been investigated. In contrast, little is known about the role of sensory neural factors and the complexity of neuro-immune interactions. Therefore, we analyzed the involvement of capsaicin-sensitive peptidergic sensory nerves in autoantibody-induced arthritis with integrative methodology. METHODS: Arthritogenic K/BxN or control serum was injected to non-pretreated mice or resiniferatoxin (RTX)-pretreated animals where capsaici

Outcome of contralateral C7 transfer to two recipient nerves in 22 patients with the total brachial plexus avulsion injury.

The treatment of total brachial plexus avulsion injury is difficult with unfavorable prognosis. This report presents our experience on the contralateral C7 (CC7) nerve root transfer to neurotize two recipient nerves in the patients with total BPAI. Twenty-two patients underwent CC7 transfer to two target nerves in the injured upper limb. The patients' ages ranged from 13 to 48 years. The entire CC7 was transferred to pedicled ulnar nerve in the first stage. The interval between trauma and surgery ranged from 1 to 13 months. The ulnar nerve was transferred to recipients (median nerve and biceps

Surgical treatment of brachial plexus posterior cord lesion: a combination of nerve and tendon transfers, about nine patients.

Lesions of the posterior cord of the brachial plexus are rare. The symptoms are usually described as palsy of the deltoid and triceps brachii muscles and of the extensor muscles of the wrist, thumb and fingers. If there is no recovery, our strategy is to operate on these patients 6 months after the injury using a combination of nerve and tendon transfers. We present a series of nine patients, two with a partial palsy and seven with a complete palsy of the posterior cord. We performed five nerve transfers to the axillary nerve, four using intercostal nerves and one using the ulnar nerve. Six pa…

Distal nerve transfers are effective in treating patients with upper trunk obstetrical brachial plexus injuries: an early experience.

BACKGROUND: Current surgical management of obstetrical brachial plexus injury is primary reconstruction with sural nerve grafts. Recently, the nerve-to-nerve transfer technique has been used to treat brachial plexus injury in adults, affording the benefit of distal coaptations that minimize regenerative distance. The purpose of this study was to test the hypothesis that nerve transfers are effective in reconstructing isolated upper trunk obstetrical brachial plexus injuries. METHODS: Ten patients aged 10 to 18 months were treated with three nerve transfers: spinal accessory nerve to the supra…

Outcome of nerve transfers for traumatic complete brachial plexus avulsion: results of 28 patients by DASH and NRS questionnaires.

The objective of this study was to evaluate outcomes of patients who suffered complete brachial plexus avulsion before and after nerve transfers by assessing upper extremity function and pain using the DASH and NRS questionnaires. Patients who underwent nerve transfers improved their DASH and NRS scores compared with before surgery. Although individually there was no correlation with improved scores, the triple combination of rehabilitation exercises, electrical stimulation therapy and neurotrophic drugs postoperatively correlated positively with improved functional outcomes. This study sugges…

Transfer of supinator motor branches to the posterior interosseous nerve to reconstruct thumb and finger extension in tetraplegia: case report.

We treated a patient with tetraplegia who had paralysis of thumb and finger extension by transferring supinator motor branches to the posterior interosseous nerve. Surgery was performed bilaterally, 7 months after a spinal cord injury. Six months after surgery, with the wrist in neutral, extension of the thumb and finger were almost full, bilaterally. In tetraplegic patients with strong wrist extensors, supinator motor branch transfer is a promising new alternative for the reconstruction of thumb and finger extension.

Interosseous nerve transfers for tibialis anterior muscle paralysis (foot drop): a human cadaver-based feasibility study.

This study explored the anatomical feasibility of using an interosseous nerve transfer (routed between the tibia and fibula) to restore motor function to the tibialis anterior (TA) muscle, following injury to the common peroneal nerve (resulting in a foot drop). The specific nerve branches evaluated as possible donor nerves included the nerves to the medial gastrocnemius, the lateral gastrocnemius, and the soleus muscles. All nerve transfers were accomplished using a direct interosseous route and a direct repair (one medial gastrocnemius transfer did require interpositional grafting). The dist…

[Paralysis of the brachial plexus caused by supraclavicular injuries in the adult. Long-term comparative results of nerve grafts and transfers].

PURPOSE OF THE STUDY: Recovery of active elbow flexion constitutes the first priority in microsurgical repair after closed injuries of the supraclavicular brachial plexus in adults. However, there are many controversial issues between the proponents of nerve grafting from available roots, and the proponents of nerve transfer. MATERIAL AND METHODS: The results concerning elbow flexor muscle recovery following microsurgical nerve repair of supraclavicular brachial plexus lesions were analysed in 62 patients. The average age at operation was 23 years old and the average delay between trauma and …
