Nissen fundoplication has a vagolytic effect on the lower esophageal sphincter.

BACKGROUND: Laparoscopic Nissen fundoplication is an increasingly utilized option for the treatment of gastroesophageal reflux disease (GERD). However, many questions remain as to the mechanism by which this operation prevents GERD in those without hiatal hernias or incompetent lower esophageal sphincters (LES). It is known that these patients experience reflux due to excess transient lower esophageal sphincter relaxations (TLESR), inappropriate and short-lived relaxation of the LES and crural diaphragm. The purpose of this study was to determine if Nissen fundoplication affects the neural pat

Facilitation-independent response of the diaphragm to cortical magnetic stimulation.

Neural diseases are often associated with respiratory muscle disorders. Assessment of the motor pathway from the central nervous system to the diaphragm is therefore highly clinically relevant from a diagnosis and follow-up point of view. Cortical magnetic stimulation (CxMS) combined with surface diaphragm electromyogram (EMGdi) has to date been limited in this application by the need of an underlying voluntary contraction to obtain a diaphragm response (facilitation). This study was performed to verify this point with high-powered stimulators and to describe the pattern of diaphragm response

Electrophysiology of the corticomotoneurone pathways in patients with movement disorders.

The corticomotoneurone pathways were examined in 21 patients with movement disorders, using the technique of percutaneous electrical stimulation of the motor cortex. Conduction in these pathways was assessed by measuring the latency to onset of electromyographic activity in the muscles of the upper limb after cortical stimulation. In all patients [five with primary (idiopathic) torsion dystonia and two with secondary (symptomatic) hemidystonia, seven with Huntington's disease, four with essential tremor, and three with Parkinson's disease] central motor conduction was normal. This and other ev

Course of the peripheral gustatory nerves.

The multiple variations of the course of the gustatory nerves still considered possible are discussed. Recent investigations lead to the conclusion that there is only one path for the gustatory fibers for each gustatory area: 1) from the anterior part of the tongue via the tympanic cord and facial nerve to the medulla oblongata; 2) for the posterior part of the tongue in the IX cranial nerve; and 3) from the soft palate via the greater superficial petrosal nerve to the facial nerve. The trigeminal nerve carries no gustatory fibers to the brain.

Focal hand dystonia in a patient with thoracic outlet syndrome.

A patient affected by thoracic outlet syndrome, with an involvement of the left lower primary trunk due to a rudimentary cervical rib, developed a severe hand dystonia on the same side. The dystonic posture was characterised by a flexion of the wrist with the fingers curled into the palm. Polygraphic recordings performed on the left flexor digitorum superficialis (FDS4) and extensor digitorum superficialis (EDC4) muscles, during a repetitive tapping task of the fourth digit, showed a loss of well formed bursts without a clear silent period along with long duration bursts of cocontraction in an

Synaptic actions on mitral and tufted cells elicited by olfactory nerve volleys in the rabbit.

1. A unitary study has been carried out of mitral and tufted cell responses to olfactory nerve volleys in the olfactory bulb of rabbits lightly anaesthetized with urethane-chloralose. 2. With volleys of different strengths, some mitral cells responded with a spike whose latency decreased considerably as the strength increased (elastic response); other cells responded at an invariant latency (inelastic response). The former may reflect diffuse olfactory nerve inputs to the dendritic tufts in the olfactory glomeruli, while tha latter may reflect input from discrete bundles of fibres. 3. The shor

Stability of complex sound representations in the auditory midbrain across the lifespan despite age-related brainstem delays.

The extent to which aging of the central auditory pathway impairs auditory perception in the elderly independent of peripheral cochlear decline is debated. To cause auditory deficits in normal hearing elderly, central aging needs to degrade neural sound representations at some point along the auditory pathway. However, inaccessible to psychophysical methods, the level of the auditory pathway at which aging starts to effectively degrade neural sound representations remains poorly differentiated. Here we tested how potential age-related changes in the auditory brainstem affect the stability of s

Clinical, electrophysiological, and immunopathological study of peripheral nerves in Hansen's disease.

Hansen's disease is a disease of peripheral nerves. Some patients develop peripheral neuropathy before the diagnosis of the disease, and others develop these complications after starting therapy. Electrophysiological (EP) studies were carried out in Hansen's disease patients. This work studied the neural deficits, electromyography (EMG) and motor nerve conduction (MNC) variables in different types of leprosy and the immunopathology of sural nerve tissue in patients with severe neural deficits. Forty leprosy patients had neurological examinations and EP study. Histopathological and immunopathol

Correlation of acetylator phenotype with peripheral, autonomic and central neuropathy in Northern Indian non-insulin-dependent diabetes mellitus patients.

OBJECTIVES: Genetic susceptibility to diabetic neuropathy has been suspected and tentatively explored; however, diabetic autonomic and centr … us with peripheral neuropathy in NIDDM subjects was observed in the present study. However, central neural conduction, primarily tested by ABERs, was significantly delayed in slow acetylators compared with fast acetylator NIDDM patients. Hence, there may be a predisposition to neuropathy in this group of patients, and such a predisposition may be better detected by …

Cryo-shocked neural stem cells as factor pool-functionalized nerve guidance conduits for enhanced peripheral nerve regeneration.

Peripheral nerve injuries (PNI) often induce severe motor and sensory impairments, subsequent muscle atrophy, and even permanent disability. Autologous nerve grafting, the clinical gold standard for PNI repair, is limited by donor site morbidity and graft shortage, while conventional nerve guidance conduits (NGCs) only provide physical guidance and fail to reconstruct the bioactive microenvironment essential for nerve regeneration. To address these limitations, we fabricated chitosan NGCs functionalized with cryo-shocked neural stem cells (Cryo-NSC NGCs) as a tissue-specific factor pool via se

Engineered BMSC-derived exosomes deliver miR-17-5p inhibitor to restore sensory conduction via the IL-11/GP130/STAT3/GAP-43 axis in dorsal column lesion.

Spinal cord dorsal column lesions (SDCL) disrupt sensory conduction, with no effective clinical treatments available. The miR-17-5p inhibitor and exosomes (EXOs) derived from bone marrow mesenchymal stem cells (BMSCs) have potential for promoting nerve regeneration. This study aims to engineer BMSC-derived exosomes to deliver the miR-17-5p inhibitor, synergistically enhancing axonal regeneration, restoring sensory processing in an SDCL rat model, and elucidating mechanisms. Bioinformatics analysis was used to identify critical pathways associated with the neuroregenerative effects of EXOs. The

Anaesthetic implications of LEOPARD syndrome.

LEOPARD syndrome is a neuroectodermal disorder presumed to result from an abnormality in neural crest cells. The acronym 'LEOPARD' is derived from the clinical features which include multiple lentigines, electrocardiographic abnormalities, ocular hypertelorism, pulmonary stenosis, abnormal genitalia, retarded growth, and deafness. Given the multisystem nature of the disease process, several issues may affect the perioperative care of these patients. Of primary importance are associated conditions of the cardiovascular system including congenital heart disease, conduction disturbances, and prog

Chronic motor neuropathies and lower motor neurone syndromes.

Immune-mediated motor neuropathies are most often disorders manifest clinically by slowly progressive, asymmetrical, distal weakness, starting in the hands more often than the legs. In some cases, electrophysiological findings show multifocal conduction block along the length of motor axons. Other patients have findings consistent with only axonal loss. Laboratory testing is unremarkable except for high-titre serum autoantibodies to GM1 and other neural antigens. Diagnosis of immune-mediated motor neuropathies provides an opportunity for effective immunomodulating therapy that can significantl

Label-free and fluorescence imaging of oligodendrocytes and myelin.

Axonal myelination finely tunes action potential conduction to control precise timing in neural circuits. Little is known about how dynamic myelinating oligodendrocytes are in the adult brain primarily due to limited approaches for their investigation at the cellular level over time in their native environment. This protocol describes optical imaging approaches that allow specific label-free detection of compact myelin which, when combined with genetically encoded fluorescence reporters and small molecule dyes, permits high-resolution longitudinal and fixed sample imaging of myelin and oligode

Reversible chest tube horner syndrome.

A 54-year-old woman who underwent chest tube placement after a lung biopsy was found on the first postoperative day to have ipsilateral ptosis and miosis, suggesting a Horner syndrome. A chest CT scan showed that the tip of the chest tube was apposed to the stellate ganglion. Repositioning of the chest tube later on the first postoperative day led to complete reversal of the Horner syndrome within 24 hours. We propose that the Horner syndrome arose as a result of pressure on the stellate ganglion, which interrupted neural conduction but did not sever the sympathetic pathway ("neurapraxia"). Wh…

Cubital tunnel syndrome of the ulnar nerve caused by an epineural ganglion cyst: a case report and review of the literature.

BACKGROUND: The ulnar nerve has a long and complex anatomical course, originating from the brachial neural plexus in the neck with nerve trunk formation at the posterior neck triangle, and on to the axilla. This intricate anatomical pathway renders the nerve susceptible to compression, direct injury, and traction throughout its course. Compression of the ulnar nerve is the second most common compression neuropathy of the median nerve adjacent to the wrist joint, after carpal tunnel syndrome. CASE PRESENTATION: A 45-year-old Sudanese housewife complained of progressive right forearm and hand m

Short-term plasticity of neuro-auditory processing induced by musical active listening training.

Although there is strong evidence for the positive effects of musical training on auditory perception, processing, and training-induced neuroplasticity, there is still little knowledge on the auditory and neurophysiological short-term plasticity through listening training. In a sample of 37 adolescents (20 musicians and 17 nonmusicians) that was compared to a control group matched for age, gender, and musical experience, we conducted a 2-week active listening training (AULOS: Active IndividUalized Listening OptimizationS). Using magnetoencephalography and psychoacoustic tests, the short-term p

Chronic fatigue syndrome: Abnormally fast muscle fiber conduction in the membranes of motor units at low static force load.

OBJECTIVE: Chronic fatigue syndrome (CFS) and fibromyalgia (FM) are disorders of unknown etiology and unclear pathophysiology, with overlapping symptoms of - especially muscular -fatigue and pain. Studies have shown increased muscle fiber conduction velocity (CV) in the non-painful muscles of FM patients. We investigated whether CFS patients also show CV abnormalities. METHODS: Females with CFS (n = 25), with FM (n = 22), and healthy controls (n = 21) underwent surface electromyography of the biceps brachii, loaded up to 20% of maximum strength, during short static contractions. The mean CV a

Visualization of electrophysiological activity at the carpal tunnel area using magnetoneurography.

OBJECTIVE: To establish a noninvasive method to measure the neuromagnetic fields of the median nerve at the carpal tunnel after electrical digital nerve stimulation and evaluate peripheral nerve function. METHODS: Using a vector-type biomagnetometer system with a superconducting quantum interference device, neuromagnetic fields at the carpal tunnel were recorded after electrical stimulation of the index or middle digital nerve in five healthy volunteers. A novel technique for removing stimulus-induced artifacts was applied, and current distributions were calculated using a spatial filter algo
