Current Clinical Evidence on Nose-to-Brain Drug Delivery.

Intranasal delivery is increasingly recognised as a promising strategy for direct drug transport to the brain via the nose-to-brain pathway, bypassing the blood-brain barrier and improving therapeutic efficacy. This approach has shown potential in the treatment of neurological disorders, including Alzheimer's disease, Parkinson's disease, epilepsy, multiple sclerosis, and acute psychiatric conditions, as well as in emergencies such as anxiety attacks and migraine episodes. Recent clinical studies investigating intranasal formulations of rivastigmine, insulin, and olanzapine, among other drugs,…

Novel, standardized sample collection from the brain-nose interface.

BACKGROUND: Diagnostics for neurodegenerative diseases lack non-invasive approaches suitable for early-stage biochemical screening and routine examination of neuropathology. Biomarkers of neurodegenerative diseases pass through the brain-nose interface (BNI) and accumulate in nasal secretion. Sample collection from the brain-nose interface presents a compelling prospect as basis for a non-invasive molecular diagnosis of neuropathologies. Here, we evaluated a novel medical device (nosecollect) that is tailored for the standardized collection of nasal secretion samples from BNI, focusing on its …

Nonsurgical Correction of Deviated Nose Using Hyaluronic Acid Fillers.

BACKGROUND: Nonsurgical correction of a deviated nose using hyaluronic acid (HA) fillers offers a minimally invasive alternative to traditional rhinoplasty. This study presents a series of cases demonstrating the efficacy of HA fillers in achieving improved nasal symmetry and alignment. METHODS: Five female patients, aged 23 to 44, with varying degrees of nasal deviation, were treated with HA fillers. The filler was strategically injected into the deep subcutaneous or supraperiosteal planes to correct the nasal contour. RESULTS: All patients showed significant improvement in nasal alignment …

Empty nose syndrome: new insights from a CFD approach.

OBJECTIVES: Empty Nose Syndrome (ENS) is a debilitating condition which usually arises after aggressive turbinate reduction. However, objective tests to help in the diagnosis of this condition are lacking. Accurate diagnosis of ENS patients is critical for effective diagnosis and treatment. The article's objectives are to utilize computational fluid dynamics (CFD) to analyze nasal airflow resistance and symmetry in suspected ENS patients, classify them into distinct groups based on CFD data, and demonstrate the potential of CFD analysis in refining ENS diagnosis and guiding individualized trea…

Nose-Ocomial Pulmonary Edema: A Rare Complication of Oxymetazoline.

A 72-year-old female with epiphora presented for outpatient punctoplasty with probing and lacrimal stent placement. Oxymetazoline was administered intranasally and the case was completed in standard fashion. Postoperatively, the patient desaturated with a workup revealing elevated cardiac enzymes, pulmonary congestion, and sinus bradycardia. However, the final cardiac testing was noncontributory, suggesting flash pulmonary edema secondary to intranasal oxymetazoline. This case highlights a rare presentation of pulmonary compromise secondary to oxymetazoline, emphasizing the importance of intra

Intranasal trigeminal function in patients with empty nose syndrome.

OBJECTIVES/HYPOTHESIS: Trigeminal nerve mediates the perception of nasal airflow. This study examines whether impaired intranasal trigeminal function is a part of the paradoxical nasal obstruction sensation in patients with empty nose syndrome (ENS). STUDY DESIGN: Prospective case-control study in a tertiary hospital. METHODS: Three groups were examined: 1) ENS patients with previous bilateral near total inferior turbinectomy, 2) patients who underwent near total inferior turbinate removal (ITR) without ENS symptoms, and 3) control participants. All participants examined with active anterior…

Ulcerative lupus vulgaris over nose, leading to cosmetic deformity.

Lupus vulgaris (LV), is a chronic and progressive form of secondary cutaneous tuberculosis. In India, it is commonly seen over buttocks, thighs, and legs whereas involvement of nose is quite rare. Ulcerative variant particularly over nose causes destruction of cartilage, leading to irreversible deformities and contracture. High-index of suspicion is required for early diagnosis and prevention of cosmetic deformity. A case of LV over nose in a young male with ulceration is reported who responded well to anti-tubercular therapy, but left with scarring of nose, which could have been prevented if …

Orbital emphysema: nose blowing leading to a blown orbit.

A 32-year-old woman with a painful swelling around the right eye few hours after blowing her nose, presented to the accident and emergency department. There was no associated history of facial trauma. Examination revealed a grossly swollen right eye and palpable subcutaneous emphysema. The patient's visual acuity and eye movements were normal. A CT scan of the orbit confirmed orbital emphysema secondary to a fracture of the floor of the orbit into the maxillary sinus, as a result of increased intranasal pressure during nose blowing. The patient was admitted and managed conservatively with anti…

Posttraumatic Nasal Deformities: Correcting the Crooked and Saddle Nose.

The nose is frequently traumatized in facial injuries and this often results from motor vehicle accidents, sports-related injuries, and altercations. Subsequently, posttraumatic nasal deformity is one of the most common reasons that patients seek consultation in the doctor's office. Depending on the type of nasal deformities, this can result in functional impairment and aesthetic problems. Two challenging problems to be addressed in the posttraumatic nose include the crooked nose deformity and the saddle nose deformity. The numerous publications on these two topics attest to the exacting surgi…

Evidence-based medicine: Unilateral cleft lip and nose repair.

LEARNING OBJECTIVES: After reading this article, the participant should be able to: 1. Describe the anatomical malformations found in unilateral cleft lip deformity. 2. Discuss current methods of measuring the deformity and subsequent outcomes. 3. Discuss preoperative assessments, workup, and the use of early interventions before definitive cheiloplasty (e.g., preoperative orthopedics, lip adhesion). 4. Discuss the different techniques used for cheiloplasty and nasal repair. 5. Discuss the use of postoperative splints, taping, or molding. 6. Discuss the outcomes and evidence of cleft lip repai

Extended osteocartilaginous spreader graft for reconstruction of deviated nose.

OBJECTIVE: Correction of a deviated nose is a challenging problem for which different approaches may be considered. This study was designed to introduce a new technique for correction of a deviated nose with a high success rate. STUDY DESIGN: The study was a prospective follow-up of 59 patients with deviated nose who had undergone corrective surgery. SETTING: An extended osteocartilaginous spreader graft was harvested from the quadrangular septum and the perpendicular plate of the ethmoid. After a medial osteotomy that was performed on the concave side, the nasal bone was lateralized and an …

[NOSE (Natural Orifice Specimen Extraction) in laparoscopic colorectal surgery].

INTRODUCTION: The aim of this study was to asses our initial first experience with NOSE techniques in laparoscopic colorectal surgery with both transanal and transvaginal extraction. MATERIAL AND METHODS: In this prospective study, the authors analyzed data from patients in whom NOSE laparoscopic sigmoid, rectosigmoid and rectal resections were performed in the Department of Surgery, University Hospital Ostrava, from May 2011 to October 2011. A group of 7 patients was analyzed based on demographic characteristics (sex, age and BMI). Tumor localization, type of extraction (transanal/transvagina…
