Symptomatic joint hypermobility.

Joint hypermobility is relatively common and has many influences such as age, gender, training, and ethnicity among many. Joint hypermobility may be asymptomatic or symptomatic. It may also be non-syndromic or syndromic. However, "asymptomatic" joint hypermobility may result in repetitive use injury, alter biomechanics of joints at other body sites, or become symptomatic later in life. Symptomatic joint hypermobility can result from soft-tissue rheumatism (e.g. bursitis, tendonitis, etc.) or muscular tension pain due to muscular imbalance. Generalized joint hypermobility (GJH) can be easily as

Managing dysphonia in paediatric patients with complex airway conditions.

OBJECTIVE: To suggest a phonosurgical management strategy that can be used for children who have previously undergone laryngotracheal reconstruction. METHODS: This cases series describes three children who presented with complex, multi-level airway stenosis and marked dysphonia. Phonosurgical intervention involved endoscopic and open approaches, and was combined with voice therapy. A phonosurgical reconstruction management algorithm is suggested for evaluating and treating these complex conditions. RESULTS: Pre-operative assessment is critical, and should involve voice analysis and glottal a

Buprenorphine maintenance therapy hinders acute pain management in trauma.

Buprenorphine is a mixed opiate receptor agonist-antagonist growing in popularity as an office-based treatment for opioid-dependent patients. It has high affinity, but only partial agonism at the micro-opioid receptor resulting in a ceiling analgesic effect. At higher doses, buprenorphine potentiates antagonism at the kappa-opioid receptor. These properties make buprenorphine an effective maintenance treatment for opioid-dependent patients. These same properties, however, can interfere with the management of acute pain in patients on maintenance buprenorphine therapy. We present a case of a yo

Surgical management of common peripheral vestibular diseases.

PURPOSE OF REVIEW: To review the latest developments in the surgical management of common peripheral vestibular disorders. RECENT FINDINGS: Although surgery is indicated mainly for patients with Meniere's disease, other less common disorders such as benign paroxysmal positional vertigo, semicircular canal dehiscence, perilymphatic fistulae, fistulae of semicircular canals, VIII nerve vascular compression, chronic vestibular neuronitis, and dizziness following sudden sensorineural hearing loss may require surgical intervention when conservative management has failed. SUMMARY: Surgery for vest

Economics of interstitial cystitis in clinical practice.

Urologists may be concerned that treatment of patients with interstitial cystitis (IC) is not economically feasible, and the time spent on these patients could be used for more profitable urologic therapy. However, care of IC patients can provide urologic practices with increased revenue opportunities through a wide range of procedures, including IC evaluation and management, treatment planning, and diagnostic, therapeutic, and even surgical techniques. Current Procedural Terminology (CPT) and evaluation and management (E&M) charge codes are provided for many types, levels, and complexities of

Ergonomics.

This review provides an evidence-based overview of ergonomics in otolaryngology-head and neck surgery, with guidance for optimizing positioning during in-office and operative procedures. Herein, we summarize recent findings highlighting the prevalence of musculoskeletal (MSK) pain among Otolaryngology, Head and Neck (OHN) surgeons and outline validated ergonomic risk assessment tools. Strategies for equipment and posture optimization are organized by visualization modality-open, endoscopic/exoscopic, microscopic, and robotic. We conclude with effective interventions to prevent and manage MSK d

Unveiling the Unlikely: Extraskeletal Ewing Sarcoma Masquerading as Gastrointestinal Bleeding.

While Ewing sarcoma is traditionally a malignant tumor of bone, it may uncommonly present extra-skeletally, leading to an array of puzzling presentations depending on the tissue involved. Here, we describe the case of a 66-year-old man who presented to the primary care office for evaluation of intermittent melena. He ultimately underwent capsule endoscopy and developed a secondary small bowel obstruction, unveiling his neoplasm. The tumor was then resected and managed with surveillance only, and the patient remains without evidence of disease after four years of follow-up.

Neonatal respiratory distress: a practical approach to its diagnosis and management.

Respiratory disorders are the most frequent cause of admission to the special care nursery both in term and preterm infants. Pediatricians and primary care providers may encounter newborn infants with respiratory distress in their office, emergency room, delivery room, or during physical assessment in the newborn nursery. The authors have proposed a practical approach to diagnose and manage such infants with suggestions for consulting a neonatologist at a regional center. Their objective is that practicing pediatricians should be able to assess and stabilize such infants, and transfer to or co

Eyelid margin disease (blepharitis and meibomian gland dysfunction): clinical review of evidence-based and emerging treatments.

Blepharitis is a common ophthalmic condition with multiple etiologies and no definitive, universal treatment. The treatment modalities for managing lid margin diseases vary depending on the disease's cause, location, and severity. For anterior blepharitis, management options include eyelid hygiene with warm compresses, eyelid scrubs, baby shampoo, and over-the-counter eyelid cleansers. Topical antibiotics and antibiotic-steroid combination drops/ointments for the eye and eyelid may accompany these. For posterior blepharitis/meibomian gland dysfunction (MGD), at-home warm compress or in-office

A Case of Myelodysplastic Syndrome-Induced Acquired Sideroblastic Anemia.

Hematological disorders are frequently encountered at the doctor's office and in the emergency room. Sideroblastic anemia, a rare hematological malady, is characterized by ring sideroblasts in the red blood cells due to accumulation of poorly transported and underutilized iron. Unlike primary sideroblastic anemia which can be conclusively diagnosed through genetic testing, the more common secondary form of the disease requires intricate cascading of several preliminary and confirmatory laboratory testing to determine the accurate etiology and provide the appropriate management regimen. The sco

Pediatric allergic contact dermatitis. Part 2: Patch testing series, procedure, and unique scenarios.

Patch testing is the criterion standard for diagnosing allergic contact dermatitis. Causative allergens differ between children and adults, necessitating the development of pediatric-specific patch test series. The Pediatric Baseline Series was developed in 2018 through expert consensus and includes relevant pediatric allergens that dermatologists can use in practice. Obstacles in patch testing, such as the need for multiple office visits, length of patch application, and avoidance of sweat and water on the testing area, are particularly challenging for the pediatric population, and several st

Prevalence of Low Back Pain and Its Related Risk Factors and Disability Following Lumbar Discectomy: A Single-Center Study.

Background Lumbar disc herniation is considered the most common cause of sciatica, which is a contributing factor to disability. Surgical management of lumbar disc herniation in the form of discectomy is superior to conservative management in terms of better recovery and pain relief. However, recurrence, residual back pain, and disability are common expected complications following surgery. Therefore, this study aims to determine the prevalence of low back pain and its related risk factors and disability following discectomy at King Fahad Hospital in the Al-Ahsa region. Methodology A cross-sec

Office Procedures in Dysphagia.

Dysphagia is a common complaint encountered by otolaryngologists in the United States. Usually, patients are referred to an otolaryngologist or other physician specializing in dysphagia (deglutologist) once empiric therapies have been tried and failed. Further investigation often required the utilization of office esophagoscopy, swallowing fluoroscopy, and now high-resolution pharyngoesophageal manometry with impedance. Over the past decades, significant advances have been made in improving care of patients with dysphagia in the clinic. In this article, diagnostic and therapeutic interventions

Public health physician perspectives on developing and deploying clinical practice guidelines during the 2009 H1N1 pandemic.

During the H1N1 outbreak of 2009, local public health units engaged in consultations with various levels of government to develop clinical practice guidelines. These guidelines provide specific clinical considerations around prevention, management, and treatment associated with the particular pathogen involved and are used by frontline healthcare professionals across many healthcare settings. In this article, we report on the lessons learned by Medical Officers of Health from across Canada on the guideline development and deployment processes and provide suggestions to improve guidelines devel

[The actions of the Public Prosecution Service in the field of health in prisons].

In the violent, insalubrious and overpopulated prison environment in which the concerns of penitentiary managers are predominantly focused on security, several actors of the legal world work incessantly to ensure that prisoners enjoy living conditions and access to health care in accordance with their constitutional rights. However, their discourses only appear infrequently in academic research and studies on health in prisons. We decided to ask for the opinion of a District Attorney of the State Public Prosecution Office, Dr. Érika Puppim to share with readers her view and the difficulties in

Tips for managing children with attention deficit hyperactivity disorder in the dental setting.

It has been estimated that between 3% and 7% of children suffer from attention deficit hyperactivity disorder (ADHD). It is probable, therefore, that these children will present in any dental office. ADHD is characterized by inattention, overactivity and impulsivity. Such symptoms can be quite severe and often present challenges in the dental setting. ADHD symptoms are often managed by medication; and it is important for the clinician to be familiar with these medications to determine when to schedule dental appointments. In dealing with ADHD children, adjunctive behavioral strategies are ofte

Epidemiology and economic impact of rhinosinusitis.

Rhinosinusitis is highly prevalent in the United States, affecting an estimated 16% of the adult population annually. The prevalence of the illness is increased in women and, relative to other US regions, in individuals living in the South. The direct costs associated with chronic rhinosinusitis are higher than the background costs in managed care patients because of increased office visits and medication prescriptions, and the indirect costs due to restricted activity days are enormous. Rhinosinusitis poses an immense economic burden in the United States. Data concerning the epidemiology and

Professional considerations for providing mifepristone-induced abortion.

Primary care clinicians who provide comprehensive reproductive heath care can now offer patients mifepristone (Mifeprex) as an abortifacient option. Clinicians, however, must first determine if the state in which they practice has regulations specifying who can perform abortions and dispense drugs, and they must consider clinical office zoning ordinances, staffing, public relations issues, and reimbursement. This article discusses the pharmacology of mifepristone and misoprostol, professional considerations, and how to prevent and manage adverse effects and complications of medical abortion.
