Metastatic Squamous Cell Lung Carcinoma to the Optic Nerve Sheath Mimicking Retrobulbar Optic Neuritis.

Lung carcinoma is the second most common malignancy in both men and women, and may metastasize to the orbit relatively early in the disease course. Metastasis to the optic nerve or its sheath is an exceedingly rare occurrence, and diagnosis may be complicated by nonspecific clinical and radiographic features. The authors present a case of squamous cell lung cancer metastatic to the optic nerve sheath, initially diagnosed as optic neuritis based on its equivocal clinical and radiographic features. This is the first histopathologically confirmed case of squamous cell lung cancer metastatic to th…

Laser speckle flowgraphy for differentiating between nonarteritic ischemic optic neuropathy and anterior optic neuritis.

PURPOSE: The aim of this study was to investigate the usefulness of laser speckle flowgraphy (LSFG) for the differentiation of acute nonarteritic ischemic optic neuropathy (NAION) from anterior optic neuritis (ON). METHODS: To investigate blood flow in the optic disc under normal conditions, NAION, and anterior ON, we compared the tissue blood flow of the right eye with that of the left eye in the control group, and that of the affected eye with that of the unaffected eye in the NAION and anterior ON groups. RESULTS: In the normal control group, the tissue blood flow did not significantly di…

Optic nerve diffusion tensor imaging after acute optic neuritis predicts axonal and visual outcomes.

BACKGROUND: Early markers of axonal and clinical outcomes are required for early phase testing of putative neuroprotective therapies for multiple sclerosis (MS). OBJECTIVES: To assess whether early measurement of diffusion tensor imaging (DTI) parameters (axial and radial diffusivity) within the optic nerve during and after acute demyelinating optic neuritis (ON) could predict axonal (retinal nerve fibre layer thinning and multi-focal visual evoked potential amplitude reduction) or clinical (visual acuity and visual field loss) outcomes at 6 or 12 months. METHODS: Thirty-seven patients prese…

Optic nerve magnetisation transfer ratio after acute optic neuritis predicts axonal and visual outcomes.

Magnetisation transfer ratio (MTR) can reveal the degree of proton exchange between free water and macromolecules and was suggested to be pathological informative. We aimed to investigate changes in optic nerve MTR over 12 months following acute optic neuritis (ON) and to determine whether MTR measurements can predict clinical and paraclinical outcomes at 6 and 12 months. Thirty-seven patients with acute ON were studied within 2 weeks of presentation and at 1, 3, 6 and 12 months. Assessments included optic nerve MTR, retinal nerve fibre layer (RNFL) thickness, multifocal visual evoked potentia…

Acute retrobulbar optic neuritis and macular detachment associated with morning glory optic disc anomaly.

A 31-year-old woman with morning glory optic disc anomaly (MGDA) developed acute retrobulbar optic neuritis and a bullous macular detachment. MRI demonstrated truncation of the perineural space of the affected optic nerve as well as focal optic nerve enhancement. Optical coherence tomography (OCT) showed retinoschisis associated with the macular detachment. The MRI and OCT findings support the vitreous as the source of the subretinal fluid. This is the first reported case of optic neuritis in MGDA.

Imaging of the optic disc and retinal nerve fiber layer in acute optic neuritis.

PURPOSE: To demonstrate whether optical coherence tomography (OCT-3) and scanning laser ophthalmoscopy (HRT-2) can be used to measure changes of the optic disc and peripapillary retinal nerve fiber layer (RNFL) in eyes with acute retrobulbar optic neuritis that have no clinically apparent optic disc swelling. To correlate these findings with presentation magnetic resonance imaging (MRI) of the affected optic nerve. METHODS: Eight consecutive patients with acute retrobulbar optic neuritis, who had no prior optic neuritis in either eye, were prospectively investigated at presentation and at bet…

Long time echo STIR sequence magnetic resonance imaging of optic nerves in optic neuritis.

Magnetic resonance images of optic nerves were obtained in 20 patients with acute optic neuritis (ON), and assessed by means of clinical, visual field and visual evoked potential evaluations; the imaging was repeated 1 year later. The results of the conventional Short Tau Inversion Recovery (STIR) sequence obtained using short time echo (STE-STIR: 22 msec) were compared with those of the long time echo sequence (LTE-STIR: 80 msec). The conventional STE-STIR sequence revealed lesions in 57.2% cases of acute ON and in 42.9% of the optic nerves affected by previous ON; the LTE-STIR sequence was d…

Contrast-enhanced MR imaging of optic nerve lesions in patients with acute optic neuritis.

The purpose of this study was to determine if administration of gadopentetate dimeglumine aids in the MR detection of optic nerve lesions in patients with acute optic neuritis and to establish an efficient MR imaging protocol to effectively demonstrate such lesions. Patients with acutely decreased visual acuity were referred for MR imaging of the brain and orbits. Predominantly, T1-weighted images were obtained in axial and coronal planes with and without contrast administration. Enhancing lesions were observed in the optic nerve (7/14 patients) and optic chiasm (2/14), and associated white ma…

Influence of antioxidant enzymes in reduction of optic disc edema in experimental optic neuritis.

Acute experimental allergic optic neuritis was induced in 18 adult strain-13 guinea pigs. The animals were divided into three groups. The first group of six animals received intraperitoneal injections of saline, the second group received superoxide dismutase (SOD), and the third group received catalase, given daily, starting 1 week after encephalogenic myelin sensitization. Morphometric analysis of longitudinal histopathologic sections of the optic nerve head revealed that the animals treated with the antioxidant enzyme catalase had a statistically significant reduction of optic disc edema whe…

Development of a Prognostic Model for Predicting Multiple Sclerosis After Optic Neuritis: A Secondary Analysis of Data From the Optic Neuritis Treatment Trial.

BACKGROUND: Optic neuritis can be the initial manifestation of multiple sclerosis (MS). The purpose of this study was to develop a prognostic model for predicting the risk of MS development among patients with optic neuritis. METHODS: The data from 388 patients with optic neuritis were retrieved from the Optic Neuritis Treatment Trial (ONTT). Cox proportional hazards regression analysis was used to develop a prognostic model. The performance of the model was assessed by using Harrell's C-index and calibration curves. The rates of MS development were estimated using the Kaplan-Meier method. R…

Multicenter clinical trial for evaluating methylprednisolone pulse treatment of idiopathic optic neuritis in Japan. Optic Neuritis Treatment Trial Multicenter Cooperative Research Group (ONMRG).

BACKGROUND: A randomized, controlled clinical trial was conducted in 1991 to compare an intravenous megadose of methylprednisolone with a control drug (mecobalamin) for treating acute idiopathic optic neuritis. CASES: Sixty-six cases from 22 clinical centers throughout Japan were examined to evaluate the treatment on visual function parameters, such as visual acuity, visual field, color vision, contrast sensitivity, and critical flicker frequency. OBSERVATIONS: The methylprednisolone pulse treatment group showed faster recovery of visual function, particularly the visual acuity at 1 week (P<…

Consensus statement for the diagnosis and management of optic neuritis.

This consensus statement provides a practical, step-by-step framework designed by 19 national specialists to standardize the care of optic neuritis. Recognizing that clinicians handle these cases differently depending on local resources, the guidelines offer a clear roadmap for distinguishing true optic nerve inflammation from its common clinical mimics. The primary focus is on identifying early "red flags," optimizing neuroimaging protocols, and using targeted cell-based antibody testing to pinpoint the exact cause of the disease. Beyond immediate diagnosis, the paper outlines clear strategie…

A Practical Approach to the Diagnosis and Management of Optic Neuritis.

Optic neuritis (ON), as an umbrella term, refers to a spectrum of inflammatory optic neuropathies arising from a myriad of potential causes. In its most common form, "typical" ON presents as a unilateral, painful subacute vision loss event in young Caucasian women. The Optic Neuritis Treatment Trial (ONTT) has historically guided our treatment of ON, and taught us important lessons about the clinical presentation, visual prognosis, and future risk of multiple sclerosis (MS) diagnosis associated with this condition. However, in the decades since the ONTT, several immune-mediated conditions such…

[Is it efficacy for the treatment of idiopathic optic neuritis with glucocorticosteroid].

Glucocorticosteroid (steroid) therapy is the major treatment for some important neuro-ophthalmological diseases. However, its efficacy, safety and abuse raise great controversy and debate for a long time, many clinical problems also remain to be solved. This paper discusses the issues relevant to the treatment of idiopathic optic neuritis by steroid and strongly advocate carrying out an evidence-based multicenter randomized clinical trial in China. It is suggested to establish a standard guideline or consensus for the treatment of neuro-ophthalmological diseases with steroid.

An unusual presentation of optic neuritis and the Pulfrich phenomenon.

The Pulfrich effect (named after Carl Pulfrich) is a well described visual stereoillusion observed when a swinging pendulum bob is viewed through a neutral density filter in front of one eye. The Pulfrich effect can be demonstrated spontaneously (ie, without a neutral density filter) in patients with multiple sclerosis because of the delayed optic nerve conduction seen after unilateral optic neuritis in multiple sclerosis. Here, a Pulfrich effect is described in a man with subclinical multiple sclerosis who only became symptomatic when playing squash.

[To improve understanding of etiology of optic neuritis in China].

Domestic ophthalmologists have different understandings on the causes of optic neuritis. Some of them consider infection is the main cause, some believe that so-called idiopathic optic neuritis caused by central nervous system primary demyelinating disease, such as multiple sclerosis is the most common type of optic neuritis, while others suggest that optic neuritis is attributed to autoimmune disease. Based on the review of literatures, we describe the development of definition and etiology of optic neuritis in Western countries and in China, and then provide some suggestion to a better under…
