Management of childhood spasticity: a neurosurgical perspective.

The management of childhood spasticity requires a multidisciplinary effort. With input from pediatricians, physical and occupational therapists, neurologists, orthotists, orthopedic surgeons, neurological surgeons, and other healthcare personnel, effective treatment for spasticity can be initiated and maintained that can lead to meaningful improvements in quality of life for vast numbers of children. Neurosurgical treatment of spasticity will continue to evolve and be refined as procedures and techniques are appropriately evaluated with reliable and validated outcome measures.

Embodied artificial intelligence in robotic orthopaedic surgery: from intelligent assistance to adaptive surgical autonomy.

Despite remarkable advances in robotic orthopaedic surgery over the past three decades, current commercial platforms remain precision-assistance technologies rather than truly adaptive intelligent systems. Embodied Artificial Intelligence (Embodied AI) represents a paradigm shift by enabling robots to perceive, reason, learn, and interact continuously with their physical environment through integrated perception-action loops. This structured narrative review examines the emerging role of Embodied AI in orthopaedic robotic surgery, highlights its conceptual differences from existing platforms,

Public information-seeking about orthopedic surgical robotics: a decade-scale infodemiological analysis and forecast through 2030.

Public information-seeking is an underused indicator of the diffusion of robotic-assisted surgery, yet no study has compared it jointly across the three highest-volume orthopedic domains or projected it toward the coming decade. Using Google Trends relative search volume (RSV: a term's share of all searches in a given month, rescaled 0-100 against the highest month of the study window, and not a count of searches) retrieved programmatically (Health category, worldwide, January 2014-June 2026; 150 monthly observations per term), we quantified and forecast public search interest in robotic knee,

Inconsistency in the Reporting Terminology of Adverse Events and Complications in Hallux Valgus Reconstruction: A Systematic Review.

Surgical complications are inevitable in any surgical subspecialty. Throughout the years, many classification systems have been developed to better understand and report such complications. The aim of this systematic review is to investigate the variability and frequency of reporting terms used to describe adverse events and complications in hallux valgus reconstruction. We hypothesized that the terms used would be highly inconsistent, which further promotes a need for a standardized terminology reporting system. Studies related to hallux valgus reconstruction outcomes that met our predetermin

Single- versus double-bundle patellar graft insertion for isolated MPFL reconstruction in patients with patellofemoral instability: a systematic review of the literature.

INTRODUCTION: The MPFL reconstruction is performed either via a single-bundle (SB) or double-bundle (DB) procedure. The purpose of this study is to perform a systematic review comparing SB versus DB graft for recurrent patellofemoral instability, to summarize current evidence, and to clarify the role of both techniques. We focused on clinical scores, physical examination, complications, revision surgeries, and failures. MATERIAL AND METHODS: In May of 2019 the main online databases were accessed. All the clinical studies treating isolated MPFL reconstruction for patellofemoral instability thr

[Secondary digit reconstruction of mutilated hand].

Traumatic digit amputation generates functional, aesthetic and psychological disabilities. Such post-traumatic consequences call for a rigorous therapeutic approach, aiming at an early secondary surgical procedure - which is ideally to be performed before the patients psychologically recover from their initial trauma. The therapeutic principle consists in improving the global function of the hand, basically aiming at re-creating a quality pollici-digital pinch, ideally thin, stable, sensitive, strong, mobile enough, but also, when possible, at improving the aesthetic aspect of the hand. The th

Characteristics and risk factors for proximal junctional kyphosis in adult spinal deformity after correction surgery: a systematic review and meta-analysis.

There are still controversies on characteristics and risk factors for proximal junctional kyphosis (PJK) in adult spinal deformity (ASD) patients. The objective of this study is to explore the characteristics and risk factors for PJK in ASD. A systematic online search in databases including PubMed, EMBASE, Web of Science, and the Cochrane Library was performed to identify eligible studies. OR and weight mean difference with 95% CI were used to evaluate characteristics and risk factors. A total of 31 studies were finally included. ASD patients with PJK had larger proximal junctional angle (PJA)

Percutaneous needle fasciotomy in Dupuytren's contracture: is it a viable technique?

The aim of this retrospective study was to evaluate the results of 44 percutaneous needle fasciotomies for Dupuytren's contracture, performed from March 2005 to June 2010. The mean age of the 36 patients was 58 years and the mean follow-up period was 28 months. The assessment was based on the clinical records and clinical evaluation. Pre-operative and postoperative total passive extension deficit and complications were registered. Recurrence and patient satisfaction were also noted. The results in stage I and II of Tubiana were interesting, with an average improvement of more than 70%. For mor

Beyond the bandeau: 4 variations on fronto-orbital advancements.

The ubiquitous tongue and groove osteotomy remains the standard for treating the anterior craniosynostoses, despite the fact that significant variations in three-dimensional fronto-orbital dystopia can occur with various sutural fusions. Four variations on bandeau design are described that allow surgeons to go beyond a simple forward advancements toward a precise three-dimensional positioning (forward advancement, vertical repositioning, and medial narrowing), or allowing the avoidance of sinus crossing osteotomies that can potentially reduce infection risk. These osteotomy designs permit more

Segmental vessel sparing during convex growth arrest surgery--a modified technique.

BACKGROUND CONTEXT: Further evidence of the importance of segmental vessel ligation in the development of neurological complications has been recently published. The more levels the ligation encompasses, the higher the risk of spinal cord damage. Therefore, caution should be taken when several segmental arteries are to be ligated in the clinical setting. PURPOSE: To prevent ligation of segmental vessels during convex growth arrest surgery and thus decrease the risk of spinal cord ischemia and neurological injury. STUDY DESIGN: A report of a modified technique of convex growth arrest surgery

Surgical anatomy and accuracy of percutaneous achilles tendon lengthening.

BACKGROUND: Percutaneous Achilles tendon lengthening is frequently done to treat gastrocsoleus equinus contracture. To our knowledge, no study has documented the proximity of tendinous or neurovascular structures to the nearest edges of each hemisection in a percutaneous Achilles tendon lengthening, the complication rates related to injury of such structures, or the Achilles tendon rupture rates from inaccurate cuts. Thus, our goal was to document these distances and determine the accuracy of this procedure. METHODS: We performed triple-hemisection percutaneous Achilles tendon lengthening (Ho

Surgical treatment of pectus carinatum.

Pectus carinatum, just like its sister deformity pectus excavatum, is a condition with an undefined developmental mechanism and debated surgical techniques. Elongation of the costal cartilages and elongation and anterior displacement of the sternum characterize the different varieties of pectus carinatum. Repair of the anomaly involves positional correction as well as the shortening of the sternum and the maintenance of its corrected position by action of the rectus abdominis and pectoralis muscles.
