Current medical, rehabilitation, and surgical management of vertebral compression fractures.

Approximately 25% of women over the age of 50 in the United States will suffer one or more vertebral compression fractures (VCFs) related to osteoporosis. VCFs are the most common of all osteoporotic fractures, with an incidence of approximately 700,000 annually. Such a fracture may cause significant pain, disability, and loss of general health and mobility and may lead to a progressive decline in quality of life. This is a review of the clinical literature on VCFs, including patient presentation, methods of diagnosis, and current rehabilitation and medical management. Much of the pain and dis

The influence of ankle-foot orthoses on gait and energy expenditure in spina bifida.

We examined the effect of ankle-foot orthoses (AFOs) on gait and energy expenditure in children with spina bifida. Nine boys and three girls, 6 to 16 years of age, took part in the study. There were four children each with L4, L5, and sacral level lesions. Each child underwent gait analysis and energy consumption studies with and without AFOs. Walking speed was faster with AFOs (mean, 58 m/min) than without (mean, 50 m/min, p < 0.01). Stride length improved significantly (p < 0.001) from 0.89 m barefoot to 1.08 m with AFOs. Double support time was decreased from 0.32 seconds barefoot to 0.28 s

Biomechanical analysis of the effect of orthotic shoe inserts: a review of the literature.

Physical activity is increasingly recognised as an important component of primary disease prevention. Overuse injuries are common sequelae of exercise and sporting activities in general, and of running in particular, frequently resulting in cessation of activity. It has been proposed that there is a link between foot shape, foot function and the occurrence of injury. As a means of treatment and prevention of further injury, orthoses and shoe inserts are widely prescribed in the belief that they can alter the pattern of lower extremity joints' alignment and movement. Although this is an assumpt

Interventions for preventing ankle ligament injuries.

BACKGROUND: Some sports, for example basketball and soccer, have a very high incidence of ankle injuries, mainly sprains. Consequently, ankle sprains are one of the most commonly treated injuries in acute care. OBJECTIVES: To assess the effects of interventions used for the prevention of ankle ligament injuries or sprains in physically active individuals from adolescence to middle age. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group's specialised register, MEDLINE, PubMed, EMBASE, CINAHL, the National Research Register and bibliographies of study reports. We also con

[Follow-up and ultrasound-controlled early treatment of hip dysplasia].

PROBLEM: The reliability of ultrasound of the hip is examined by the courses of mature and physiologically immature hips. The results of sonographically controlled therapy of the congenital dysplasia of the hip are investigated. METHOD: 470 children out of a hip-screening program could be controlled at least one more time clinically and by ultrasound. X-rays were taken at the end of treatment in children who were treated by an abduction device. The sonographical and radiological results were compared and grafically presented. RESULTS: None of 570 matured hips deteriorated. 88% of the physiol

An aggressive approach to limb dystonia: a case report.

A 15-year-old boy presented with a severe fluctuating foot and ankle dystonia resulting from a basal ganglia insult at the age of 4. This followed an embolic event related to an undiagnosed prolapsed mitral valve. Functionally, the patient was ambulatory with rocker bottom crutches and an ankle-foot orthosis, but there were periods of up to a year when pain and increased dystonic deformity required him to use a wheelchair. A new orthotic was made nearly every month because the orthotist could find no material that would withstand his tone without breaking, yet he could not ambulate without one

Effect of timing of stabilization on length of stay and medical complications following spinal cord injury.

This retrospective study examines length of acute hospital stay (LOS) and the development of medical complications in 64 patients with cervical, thoracolumbar or cauda equina injuries divided into two groups according to whether they underwent spinal stabilization < 24 hours after injury or > 24 hours after injury. The mean length of stay for the early stabilization group was 37.5 days (SD +/- 34.2) and for the late stabilization group 54.7 days (SD +/- 40.1). This difference was statistically significant by Mann Whitney U test (Z = 2.53, P = 0.01). There was no statistically significant diffe

Anatomical factors associated with overuse sports injuries.

Overuse injuries develop when repetitive stress to bone and musculotendinous structures damages tissue at a greater rate than that at which the body can repair itself. A combination of extrinsic factors, such as training errors and environmental factors, and intrinsic or anatomical factors, such as bony alignment of the extremities, flexibility deficits and ligamentous laxity, predispose athletes to develop overuse injuries. Malalignant of the lower extremity, including excess femoral anteversion, increased Q angle, lateral tibial torsion, tibia vara, genu varum or valgum, subtalar varus and e

Immediate effects of the toe spreader on the tonic toe flexion reflex.

BACKGROUND AND PURPOSE: The purpose of the study was to determine whether the use of a toe spreader to inhibit the tonic toe flexion reflex (TTFR) immediately alters temporal-distance gait characteristics, plantar surface contact, or muscle activity in the limb exhibiting the TTFR of subjects with hemiparesis secondary to supraspinal lesion. SUBJECTS: Eighteen adults with hemiparesis secondary to supraspinal lesions served as subjects for the standing portion of the study. Sixteen of the subjects participated in the gait portion of the study. METHODS: The study was a randomized, within-subje

Heel seats and shoe wear.

Seventy-two children with excessive heel valgus and markedly uneven shoe wear were treated by Helfet heel seats for 18-36 months. Of the 52 children reviewed in this study, 44 had improved shoe wear. Simple heel seats offer economical treatment for children whose foot deformities destroy their shoes.

Use of the Pavlik harness for hip displacements. When to abandon treatment.

Seven hundred twenty congenitally dislocated or subluxated hips in 550 patients less than one year old, treated with the Pavlik harness, were retrospectively reviewed. Overall, 11% of the hips proved irreducible by Pavlik harness treatment, 9% had dysplasia at the end of harness treatment, and 5% developed dysplasia by the age of two years two months (average). Fourteen percent of the hips positive for Ortolani's sign, 6% of the hips positive for Barlow's sign, and 2% of the congenitally subluxated hips required open or closed reduction followed by plaster hip spica immobilization. The medial heel skive technique. Improving pronation control in foot orthoses.

A new method of foot orthosis modification that enhances the pronation controlling ability of foot orthoses is presented. The medial heel skive technique involves selectively removing small amounts of the medial portion of the plantar heel of the positive cast of the foot to create a unique varus wedging effect within the heel cup of the foot orthosis. The resulting increase in supination moment across the subtalar joint axis of the foot clinically produces significantly improved pronation control on pediatric flexible flat feet, posterior tibial dysfunction, and other types of excessively pro

The weight-bearing abduction brace for the treatment of Legg-Perthes disease.

We reviewed the cases of thirty-one patients (thirty-four hips) who had had severe Legg-Perthes disease (Catterall group III, five hips and group IV, twenty-nine hips) and had been treated with a weight-bearing abduction orthosis, to determine the effectiveness of the brace. The mean age of the patients when they were first seen was six years (range, three to twelve years). The mean duration of follow-up was seven years (range, two to thirteen years). At the most recent follow-up, all hips were rated with the method of Mose and the classification of Stulberg et al. According to the criteria of

[Scoliosis, spondylolysis and lumbosacral spondylolisthesis. A study of their association apropos of 82 cases in children and adolescents].

The authors have reviewed the charts of 82 patients who presented the association of a scoliosis and a spondylolisthesis. They insist upon the necessity to treat each abnormality for itself. 26 patients have been simply followed in the clinic, they required no treatment because of the modicity of the scoliosis and the spondylolisthesis. 23 patients were treated with orthosis because of the scoliosis progression. In this group the spondylolisthesis was not a major concern and remained stable. 15 patients had to be operated on because their scoliosis was threatening. The existence of a spondylol

Idiopathic scoliosis: correction of lateral and rotational deformities using the Cotrel-Dubousset spinal instrumentation system.

The Cotrel-Dubousset instrumentation (CDI) system developed between 1982 and 1984 is now being used extensively for the posterior correction of idiopathic scoliosis. In this study we compared the lateral and rotational correction obtained with CDI to that obtained with Harrington rod instrumentation and Drummond wiring. We matched 14 patients who had had CDI by age and sex with 14 patients who had had either Harrington rod instrumentation (n = 5) or Drummond wiring (n = 9). Average lateral curve correction was significantly better with CDI than with the combination of the other systems (71.9%
