The floating mass transducer for external auditory canal and middle ear malformations.

OBJECTIVE: The indications for the Med-El Vibrant Soundbridge, currently limited to patients with sensorineural hearing loss and normal middle ear function, have been extended to include patients with conductive or mixed hearing loss because of severe acquired or congenital ossicular chain defects. Patients with congenital aural atresia have combined malformations of the external auditory canal and the middle ear, often accompanied by severe mixed hearing impairment. Long-term results of traditional surgical techniques for treatment of congenital aural atresia show a persistent air-bone gap in

Results obtained with a new superelastic nitinol stapes prosthesis in stapes surgery.

OBJECTIVE: The trimmable nitinol-polytetrafluoroethylene stapes prosthesis is a novel prosthesis that features a band-shaped nitinol loop. Unlike the SMart prosthesis, which uses the heat-activated self-crimping property of the alloy for fixation to the long process of the incus, this new prosthesis uses the property of superelasticity. The objective of the present study was to assess the hearing outcomes and intraoperative handling of the first 31 prostheses to be implanted in a primary stapes operation and to compare the results and experience thus obtained with those previously achieved wit

First experience with a new titanium clip stapes prosthesis and a comparison with the earlier model used in stapes surgery.

OBJECTIVES/HYPOTHESIS: The aim of the study was to gain the first clinical experience with a new titanium clip prosthesis in stapes surgery, and to compare this model with its predecessor. We placed particular emphasis on the practicability of fixing the prosthesis to the long process of the incus and on the postoperative improvement in hearing. STUDY DESIGN: Retrospective chart review. METHODS: The study included 23 patients who had a CliP Piston àWengen fitted and 21 patients with a Soft CliP Piston (both from Kurz Medizintechnik, Dusslingen, Germany). Air and bone conduction were tested p

Reparative granuloma related to perilymphatic fistula.

Reparative granuloma is defined as an 'exaggeration of the normal reparative process' (Schuknecht) after stapes surgery, often resulting in a destruction of the labyrinth. It is ascribed to a foreign body reaction, yet there are insufficient histological data to prove this and although rare, the problem is still with us. We encountered two patients in whom the history contains elements evoking a perilymphatic fistula. Cerebrospinal fluid causes irritation of the middle ear mucosa. This is demonstrated in an exemplary way in case of a spontaneously developed cerebrospinal fluid leak from a defe

Long-term functional outcome and satisfaction of patients with an active middle ear implant for sensorineural hearing loss compared to a matched population with conventional hearing aids.

UNLABELLED: The objectives of the study were to review the results of an active middle ear implant for sensorineural hearing loss in patients who were unable to wear or did not benefit from conventional hearing aids in comparison to patients with a matched degree of hearing loss successfully fitted with a conventional hearing aid. A retrospective chart review of 10 patients (10 ears) after implantation of an active middle ear implant and 12 patients (13 ears) with conventional hearing aids in one tertiary referral center was performed. Intervention for sensorineural hearing loss was the implan

Integration of 3D-printed middle ear models and middle ear prostheses in otosurgical training.

BACKGROUND: In otosurgical training, cadaveric temporal bones are primarily used to provide a realistic tactile experience. However, using cadaveric temporal bones is challenging due to their limited availability, high cost, and potential for infection. Utilizing current three-dimensional (3D) technologies could overcome the limitations associated with cadaveric bones. This study focused on how a 3D-printed middle ear model can be used in otosurgical training. METHODS: A cadaveric temporal bone was imaged using microcomputed tomography (micro-CT) to generate a 3D model of the middle ear. The

Management of conductive and mixed hearing loss intolerant to air-conduction hearing aids: A stepwise algorithm and narrative review from a Japanese perspective.

OBJECTIVE: Conductive and mixed hearing loss in the complicated ear, including chronically inflamed ears with recurrent otorrhea, postoperative cavities, tympanic membrane lateralization, canal stenosis/atresia, and congenital malformations, remains a frequent, consequential problem in cases where conventional air-conduction hearing aids (ACHAs) are unusable or provide insufficient functional benefits. The expansion of the therapeutic landscape from non-implantable (e.g., cartilage conduction hearing aids [CCHA] and adhesive bone-conduction systems) to implantable (e.g., bone-conduction implan

Impact of coupling techniques of an active middle ear device to the round window membrane for the backward stimulation of the cochlea.

HYPOTHESIS: Interposed cartilage and the round window coupler (RWC) increase the efficiency of cochlea stimulation with the floating mass transducer (FMT) of a single active middle ear implant (AMEI) placed against the round window membrane. BACKGROUND: Treatment of mixed and conductive hearing loss with an AMEI attached to the round window is effective, yet the best placement technique of its FMT for the most efficient stimulation of the cochlea remains to be determined. METHODS: Experimental study on human temporal bones with the FMT placed against firstly the unaltered round window niche

Cartilage tympanoplasty: literature review.

There has been renewed interest in the use of cartilage for middle-ear reconstructions. The aim of the present review is to examine the indications, techniques and surgical outcomes of cartilage tympanoplasties reported in the literature. There have been concerns regarding weakening of cartilage struts, from histological studies involving explants; as a result, cartilage struts for ossiculoplasty have not gained popularity. On the other hand, cartilage tympanoplasty is now an established procedure for tympanic membrane and attic reconstruction. The commonest techniques involve cartilage palisa

Temporal bone fracture with ossicular dislocation caused by a blow to the opposite side of the head.

We report an indirect temporal bone fracture with ossicular chain injury occurring only on the side opposite a facial bone fracture caused by a direct blow. A 45-year-old female was referred to our clinic complaining of right hearing loss. She had suffered a left facial bone fracture in a traffic accident 6 months earlier. Her right hearing impairment was conductive. Exploration of her right middle ear revealed a temporal bone fracture, with ossicular chain separation and a dislocated stapes with a perilymphatic fistula. These injuries were considered to be caused by an indirect force followin

Surgical advances in treating otitis media.

Surgical methods of treating otitis media and its sequelae are discussed, according to the classification of otitis media presented in an earlier report. Surgical management of otitis media with effusion and recurrent purulent otitis media includes myringotomy and use of ventilation tubes. Occasionally, otitis media with effusion will lead to structural and other pathologic changes in the middle ear, and conservative treatments such as use of medication or tubes will not suffice. Indications and methods for exploratory tympanotomy and reconstruction of the middle ear are discussed. In such ins

Realities in ossiculoplasty.

The results of ossiculoplasty are frequently reported in terms of closure of the air-bone gap. This parameter is a reliable indicator of the degree of technical success, and is useful in comparing different materials and types of reconstructions. However, assessment of the operated ear alone does not evaluate the effect of surgery on binaural hearing ability, leading to the situation where sub-optimal advice may be given to patients pre-operatively. This article advocates a more patient orientated method of assessing the results of ossiculoplasty. Previous studies have indicated that the opera

Surgery for congenital conductive deafness in Klippel-Feil syndrome.

A wide variety of anomalies of the middle and inner ear has been reported in association with congenital deafness in Klippel-Feil syndrome. Findings of six new exploratory tympanotomies are added to a review of 14 previously reported; for this entire group, better hearing was achieved in eight ears. When the only anomaly is a disconnection of the incudostapedial joint, good results can be expected from reconstruction of the ossicular chain. With the addition of the six newly reported stapedectomies, the success rate for this procedure now equals 50%. None of the fenestrations of the promontory

Some observations concerning 200 stapedectomies: fifteen years postoperatively.

In this study, a statistical evaluation of the hearing improvement, the incidence of postoperative tinnitus and vertigo, and the permanency of the surgically restored hearing of 120 women and 80 men operated on for otosclerosis between 1970 and 1971 was undertaken. The standardized operating procedure in all of these cases involved the use of a Gelfoam and wire prosthesis (Schuknecht 3). All patients underwent normal audiological examinations, including determination of bone and air conduction values. Pure tone thresholds were determined by averaging the values for 500, 1,000, 2,000 and 4,000
