A previously healthy 15-year-old girl was bitten by a cat in her left thumb. Despite initial treatment with penicillin she developed osteomyelitis after a period of four months. Pasteurella multocida was cultured from the necrotic bone. The infection showed low activity and was successfully treated with surgery combined with penicillin.

Pyogenic vertebral osteomyelitis may follow any process which initiates bacteremia and should be considered in a differential diagnosis of back pain occurring in the convalescent phase of septicemia. The two cases discussed here, following abdominal stab wounds, illustrate the principles of diagnosis and management of this problem. Diagnosis is based on gradual onset of low back pain with spinal tenderness and paravertebral muscle spasms, without neurologic symptoms. Serial X-rays are helpful, as are technetium phosphate or gallium bone scans. Appropriate antibiotic plus immobilization are rec…

Hemophilus influenzae may account for 10% or more of cases of acute hematogenous osteomyelitis in infants. H. influenzae osteomyelitis is more often seen in the upper extremities. Two patients with H. influenzae acute osteomyelitis seen at a community hospital are described. The second patient is the first known case of osteomyelitis secondary to ampicillin-resistant H. influenzae. A review of 22 additional cases revealed that meningitis accompanied osteomyelitis in 10% (2/22).

Analysis of three cases and a review of the recent literature on squamous cell carcinoma arising in osteomyelitis confirm that this is an uncommon condition which may run a less benign course than was earlier believed. The disease affects chiefly middle-aged and older men, usually involves the lower extremity and most often occurs in the tibia. Development of a malignant tumour is heralded by increased pain and foul-smelling drainage in the presence of a fungating ulcer. Increased bone destruction is seen on the roentgenograms. A consistent finding is the long duration of infection but the aut…

Gel-diffusion and the enzyme-linked immunosorbent assay (ELISA) were used to quantify and to identify the immunoglobulin class of teichoic acid antibodies in patients with chronic staphylococcal osteomyelitis and a wide variety of other infections. Teichoic acid antibodies were identified by gel-diffusion in 14 of 23 patients with staphylococcal endocarditis, six of 30 with staphylococcal bacteremia without endocarditis, four of 35 with staphylococcal skeletal infections, and one of 45 with nonstaphylococcal infections. None of the 20 patients with chronic staphylococcal osteomyelitis had posi…

In spite of modern therapeutical concepts chronic osteomyelitis is a disease, which can be resistant to all trials with antibiotics, surgical or other treatments. Since the percentage of bone infections increases by increasing accidents the possibilities of classical treatment are discussed and new therapeutical ideas developed by considerations regarding the pathomechanism. The basical reflections for this new treatment can be summarized with the fact that a chronic infection can only develop in an organism with a lack of immune resistance. As the immune response of an organism can be stimula…

Diffuse sclerosing osteomyelitis (DSO) is a non-bacterial disease of the jawbone, characterized by intermittent pain, swelling, and a mixture of osteosclerosis and osteolysis on radiographs. Its etiology remains unclear, and a standard treatment, based on clear diagnostic criteria, has not been established. We present the case of a 48-year-old male patient, who was initially diagnosed with chronic mandibular osteomyelitis due to apical periodontitis in the right lower second premolar, and underwent antimicrobial medication and surgical therapy based on computed tomography (CT), magnetic resona…

Osteomyelitis remains a serious inflammatory bone disease that affects millions of individuals worldwide and for which there is no effective treatment. Despite scientific evidence that Staphylococcus bacteria are the most common causative species for human bacterial chondronecrosis with osteomyelitis (BCO), much remains to be understood about the underlying virulence mechanisms. Herein, we show increased levels of double-stranded RNA (dsRNA) in infected bone in a Staphylococcus-induced chicken BCO model and in human osteomyelitis samples. Administration of synthetic [poly(I:C)] or genetic (Alu…

Osteomyelitis can result from the direct inoculation of pathogens into bone during injury or surgery or from spread via the bloodstream, a condition called hematogenous osteomyelitis (HOM). HOM disproportionally affects children, and more than half of cases are caused by Staphylococcus aureus. Laboratory models of osteomyelitis mostly utilize direct injection of bacteria into the bone or implantation of foreign material and therefore do not directly interrogate the pathogenesis of pediatric hematogenous osteomyelitis. In this study, we inoculated mice intravenously and characterized the result…

BACKGROUND: Chronic Non-bacterial Osteomyelitis (CNO) is a rare autoinflammatory bone disease primarily affecting children and adolescents. The disease presents with a wide spectrum of severity, ranging from mild unifocal lesions to severe, recurrent multifocal bone inflammation. Its etiology remains unclear, making diagnosis challenging due to nonspecific symptoms.
METHODS: We report the case of a 14-year-old girl who presented with recurrent swelling and pain in the left clavicle. After multiple admissions, the patient underwent extensive diagnostic workup, including laboratory tests, imagi…

Fungal osteomyelitis is an uncommon infection that can present with nonspecific symptoms, leading to delays in diagnosis. We report the case of a 27-year-old woman who presented with chronic right knee pain and intermittent swelling for three months. Imaging revealed a subcortical osteolytic lesion in the lateral condyle of the distal femur. The patient underwent surgical curettage and biopsy, which confirmed fungal osteomyelitis caused by Candida auris. Antifungal susceptibility testing showed resistance to multiple agents, with sensitivity to voriconazole. Following surgical debridement, the…

Chronic sclerosing osteomyelitis (CSO) is a rare, non-suppurative inflammatory condition of the bone characterized by progressive sclerosis and fibrosis, often requiring a multifaceted diagnostic approach. This case highlights an eight-year-old female with progressive mandibular swelling and trismus unresponsive to prior treatments. Imaging and histopathological evaluation revealed features consistent with CSO, excluding infectious or neoplastic causes. Intraoperative findings prompted a shift from planned segmental resection to conservative bone shaving, preserving mandibular integrity while …

This case presents the first documented successful medical management of chronic nonbacterial osteomyelitis (CNO) in a small dog-a condition rarely described in veterinary medicine. A 4-year-old castrated male Pomeranian dog weighing 4.6 kg was presented with a 3-week history of right hindlimb lameness. Radiographic examination revealed osteolytic lesions, periosteal reactions, and decreased muscle mass in the affected limb. Microbial and blood culture tests showed negative results, whereas cytological and histopathological analyses of the right distal femur confirmed neutrophilic inflammation…

CLINICAL/METHODOLOGICAL ISSUE: Hematogenously disseminated osteomyelitis in children and adolescents is an acute disease with a substantial threat to the health of a child or adolescent. Due to the destruction of bones or joints within hours to days and the risk of bacterial sepsis, it is a feared pediatric emergency that requires hospitalization in a children's hospital and rapid action.
STANDARD RADIOLOGICAL METHODS: The standard radiological procedures are X‑ray, sonography and magnetic resonance imaging (MRI).
METHODOLOGICAL INNOVATIONS/PERFORMANCE: Contrast-enhanced MRI has established …

INTRODUCTION: Congenital talipes equinovarus (CTEV), or clubfoot, is a common congenital lower limb disorder. Pre-natal diagnosis using ultrasound (US) enables early identification, facilitating timely intervention and improving outcomes. Understanding the diagnostic accuracy and implications of pre-natal CTEV detection is essential for effective clinical management.
MATERIALS AND METHODS: This observational cohort study evaluated pre-natal US accuracy in detecting CTEV. Two thousand singleton pregnancies between 18 and 24 weeks of gestation were included. Exclusions applied to complicate and

Lactobacillusspecies are generally considered beneficial and are not typically pathogenic in humans. Bloodstream and tissue infections are uncommon and usually occur in immunocompromised patients. We present a case of a 63-year-old female with Lactobacillus jensenii lumbosacral discitis and osteomyelitis, complicated by small bilateral psoas abscesses and myositis. L4-L5 lumbar biopsy confirmed the presence of L. jensenii via next-generation sequencing after conventional tissue and blood cultures were negative. The presumed etiology of this infection was a genitourinary source. The patient rec…
