Shock was induced in sixty dogs by laparotomy, splenectomy and exteriorization of the small intestine for three hours. Resuscitation following different plasma substitutes was assessed by measuring hemodynamic parameters (mean arterial blood pressure, plasma volume, oxygen consumption, cardiac output and hematocrit). Gas gangrene which is caused by gram-negativ rod-shaped bacilli appeared longtime in war only. Now an increase of infection is noted by heavy traffic accidents. Reporting about 18 patients and treatment results we suggest for therapeutic proceeding: excision of wounds without primary amputation followed by hyperbaric oxygenation. In our opinion, surgical intervention and hyperbaric oxygenation should not be rival but complementary treatment of equal value.

In the last decade hyperbaric oxygen has been recognized as an important therapeutic tool in a variety of instances in which either destruction of anaerobic bacteria is urgent or an improvement in the oxygenation level is mandatory. The first case of ‘gas gangrene’ of the brain treated with hyperbaric oxygen is reported. It is suggested that there is no place in clostridial infection confined to the central nervous system for hyperbaric oxygen treatment. The early onset of symptoms in clostridial brain abscess with the presence of a gas bubble localizing the abscess were again confirmed. The importance of taking a careful history and obtaining skull radiographs in suspected penetrating wounds is stressed.
