Opioid Craving in Chronic Pain Management: A Structural Equation Model of Veterans in Pain Rehabilitation.

INTRODUCTION: Craving is a significant factor in the use and misuse of various substances, but its relevance to prescription opioids in indi … wledge gap by providing rare prospective data on craving in patients using prescription opioids for pain management. We hypothesized that pre-treatment opioid craving would predict changes in opioid use during treatment. In this observational study (N = 104), we aim to expand on previous research by examining the relationship between opioid craving and opioid tap …

The unheard pain: why patient-related barriers remain the weakest link in cancer pain management.

Cancer pain remains a major challenge in oncology and palliative care despite the availability of clinical guidelines, effective analgesics, opioids, educational interventions, and non-pharmacological strategies. This Perspective argues that patient-related barriers remain one of the weakest links in cancer pain management because they are often treated as secondary obstacles rather than core determinants of treatment effectiveness. These barriers include fear of addiction, concerns about adverse effects, fatalistic beliefs, limited pain communication, low adherence, insufficient knowledge, an…

Pain Management Strategies for Chronic Pain after Thoracic Surgery: From Pharmacological Therapy to Regenerative Approaches.

PURPOSE: Thoracic surgery is commonly performed for the management of conditions involving the lungs, mediastinum, pleura, and chest wall. Despite advances in surgical techniques and perioperative care, postoperative pain remains a frequent complication. In some patients, pain may persist and progress into chronic post-thoracotomy pain syndrome (CPTPS), defined as pain lasting more than 2 months after surgery. The prevalence of chronic pain ranges from 21% to 91%, with lower rates observed in minimally invasive approaches such as video-assisted thoracoscopic surgery (VATS) (~35.5%) and robotic

Chronic stress, cortisol dysfunction, and pain: a psychoneuroendocrine rationale for stress management in pain rehabilitation.

Pain is a primary symptom driving patients to seek physical therapy, and its attenuation commonly defines a successful outcome. A large body of evidence is dedicated to elucidating the relationship between chronic stress and pain; however, stress is rarely addressed in pain rehabilitation. A physiologic stress response may be evoked by fear or perceived threat to safety, status, or well-being and elicits the secretion of sympathetic catecholamines (epinephrine and norepinepherine) and neuroendocrine hormones (cortisol) to promote survival and motivate success. Cortisol is a potent anti-inflamm

Cost-Effectiveness of Integrative Pain Management for Chronic Pain: A Systematic Review of Economic Evaluation.

Chronic pain significantly impacts quality of life and productivity, with Integrative Pain Management (IPM) offering a potential solution. However, its cost-effectiveness compared to usual care remains unclear. This systematic review assesses the economic evaluations of IPM interventions for chronic pain. Seven randomized controlled trials (RCTs) were included, focusing on cost-effectiveness measures such as the Incremental Cost-Effectiveness Ratio (ICER) and quality-adjusted life years (QALYs). The results showed a broad range of ICER values, from $2,808 to $74,868 per QALY, depending on the …

Silent Pain or Silent Records? Pain Visibility, Documentation Ethics, and Nursing Management in Neurocritical Care.

Pain assessment in critical care is organised primarily around patient self-report and numerical scoring, an arrangement that protects the patient's first-person authority. In neurocritical care, however, critically ill patients with stroke may be unable to communicate because of aphasia, impaired consciousness, sedation, mechanical ventilation, or delirium. Under these conditions, the absence of a numerical score may reflect limitations in the systems through which pain is made visible rather than the absence of pain. This paper offers an empirically informed philosophical analysis of what a

Enhancing Pain Management: The Role of Superior Hypogastric Plexus Block in Chronic Pelvic Pain Treatment.

Chronic pelvic pain (CPP) is a multifactorial condition characterized by persistent pain in the pelvic region, significantly impacting the quality of life and psychological wellbeing of affected individuals. The superior hypogastric plexus block (SHPB) has emerged as a promising interventional therapy for patients with refractory CPP. This procedure involves the targeted administration of anesthetic agents to the superior hypogastric plexus, a key neural network responsible for transmitting visceral pain from the pelvic organs. A thorough analysis of existing literature demonstrates that SHPB

Pain management considerations in patients living with both pain syndromes and cardiovascular diseases and disorders.

Concomitant pain syndromes and cardiovascular disease (CVD) and disorders are associated with significant morbidity, impaired quality of lif … ients with CVD and/or disorders. However, the literature surrounding the use of pharmacotherapy for pain management is limited to retrospective studies and there is a lack of well-designed, prospective, randomized trials; this also includes head-to-head comparator studies. The expanding role of chronic pain interventions in multimodal perioperative pain management: a narrative review.

Surgery is a risk factor for chronic pain and long-term opioid use. As perioperative pain management continues to evolve, treatment modalities traditionally used for chronic pain therapy may provide additional benefit to patients undergoing surgery. Interventions such as radiofrequency ablation, cryoneurolysis, and neuromodulation may potentially be used in conjunction with acute pain procedures such as nerve blocks and multimodal analgesia. Pharmacological agents associated with chronic pain medicine, including gabapentinoids, ketamine, and selective serotonin reuptake inhibitors, may be usef…

Multidisciplinary pain management program for patients with chronic musculoskeletal pain in Japan: a cohort study.

INTRODUCTION: Multidisciplinary pain management is a useful method for treating chronic musculoskeletal pain. Currently, few facilities in Japan offer multidisciplinary pain treatment, especially in the inpatient setting. We implemented a multidisciplinary pain management program based on International Association for the Study of Pain recommendations. This study described our initial efforts in implementing the program, and reported 3- and 6-month follow-up results. MATERIALS AND METHODS: Our pain management team included orthopedic surgeons, psychiatrists, nurses, physical therapists, clini…

Does pain mediate the pain interference with sleep problem in chronic pain? Findings from studies for management of diabetic peripheral neuropathic pain with duloxetine.

Although sleep problems are common in patients with chronic pain, it is unclear whether pain mediates (causes) impaired sleep. The relationship between pain and sleep has been difficult to investigate because of the potential confounds of depression and somnolence. This report used clinical trials data for duloxetine in the management of diabetic peripheral neuropathic pain (DPNP) to investigate the direction of this association. Data were pooled from three double-blind, randomized, placebo-controlled, 12-week trials of patients with DPNP without mood disorder (n=1,139). DPNP patients reportin
