PFO-closure for secondary stroke prevention proved to be superior over medical therapy. This was shown in several meta-analyses of the three large randomized trials. According to the current evidence, PFO-closure should be the preferred therapy in this setting. PFO-closure for primary prevention makes sense and should be considered in patients with a disease associated with PFO (e.g. migraine, sleep apnea).

There are no current therapies that specifically target IgE production in human allergic disease. In the United States, a longstanding legal rule exists against patenting natural phenomena. The Supreme Court recently had an opportunity to help define the boundaries and clarify the implications of this "natural phenomenon doctrine" in Laboratory Corporation of America v. Metabolite Labs., dismissed as improvidently granted. This article argues that the natural phenomenon doctrine renders both the patent claim at issue in LabCorp, and the patents that directly or indirectly claim biological correlations between genotypes and medical phenotypes, invalid or unenforceable under U.S. patent law.

BACKGROUND: A patent ductus arteriosus (PDA) complicates the clinical course of preterm infants, increasing their risks of developing chronic lung disease (CLD), necrotizing enterocolitis (NEC), and intraventricular hemorrhage (IVH). Indomethacin is used as standard therapy to close a PDA, but is associated with reduced blood flow to the brain, kidneys and gastrointestinal tract. The author describes his first-hand experience of a technique to keep patients with a suprapubic catheter free from urine leakage. He modified a suprapubic catheter by adding an external phalange to secure the catheter and balloon placement within the bladder. To date, this modification has prevented leakage around the catheter site for all patients in which it was used. In a followup article, the author will explain how he obtained a patent for the new design and worked with companies to manufacture the new product.
