Transversus abdominis plane block improved surgical conditions in neuromuscular blocker-free pediatric laparoscopic hernia repair: a randomized trial.

BACKGROUND: Pediatric laparoscopic hernia repair (LHR) is a short, commonly performed ambulatory procedure that can be conducted under general anesthesia without neuromuscular blocker (NMB). This prospective randomized controlled trial evaluated whether a transversus abdominis plane (TAP) block facilitates the implementation of NMB-free anesthesia in this surgical setting. METHODS: Eighty pediatric patients scheduled for LHR without NMB were stratified by age (≤ 3 or > 3 years) and randomized to receive either a bilateral TAP block after induction (TAPB group) or surgical-site local infiltrat

Supplemental intraoperative crystalloids for pediatric postoperative nausea and vomiting-A systematic review and meta-analysis.

BACKGROUND: Postoperative nausea and/or vomiting is a relatively frequent occurrence after general anesthesia in pediatric patients. Supplemental perioperative crystalloid fluid administration has been shown to have a positive effect on the incidence of nausea and/or vomiting in adults undergoing surgery. The question arises whether supplemental intraoperative intravenous fluids in pediatric patients offers beneficial results with regards to pediatric postoperative nausea and/or vomiting. METHODS: Pubmed, EMBASE, Google Scholar, and Web of Science were searched up to March 2022 to perform a s

Procedural Sedation in Minor Procedure Rooms for Pediatric Myringotomy and Tympanostomy: A Quality Improvement Initiative.

OBJECTIVE: Lengthy wait times for elective surgery is a widespread health care system conundrum that may increase patient distress and jeopardize health outcomes. The primary aim of this quality improvement project was to reduce the surgical wait time in patients undergoing tympanostomy tube insertion. METHODS: As of January 2018, our tertiary care institution implemented a novel protocol whereby healthy children may undergo tympanostomy tube insertion in a minor procedure room under ketamine sedation administered by pediatric emergency physicians to address lack of both physical and anesthes

Oxygen consumption following pediatric cardiac surgery.

Metabolic responses during recovery from cardiac operations for various congenital heart defects were studied in 30 mechanically ventilated pediatric patients in two groups: infants 1 year or less (group I) and children more than 1 year old (group II). Oxygen consumption (VO2) and carbon dioxide production (VCO2) were measured using a pediatric metabolic monitor intermittently after induction of anesthesia, after skin closure, 2 to 4 hours postoperatively, and on the first postoperative morning in the pediatric intensive care unit. Energy expenditure and respiratory quotient were determined fr

Navigating the Perils of Anesthesia: Managing Mediastinal Masses for Tru-Cut Biopsy.

Managing mediastinal masses during anesthesia presents formidable challenges, particularly in pediatric patients undergoing procedures such as tru-cut biopsy. These masses, both benign and malignant, can compress vital structures, leading to life-threatening complications. This article explores the complexities of managing anesthesia in patients with mediastinal masses, emphasizing the importance of meticulous preoperative assessment, understanding the relationship between the mass and surrounding anatomy, and employing lifesaving techniques such as inhalation induction and awake intubation. Error Traps in Pediatric Neuromuscular Block.

BACKGROUND: Neuromuscular blocking agents are essential for safe pediatric anesthesia but remain a frequent source of preventable morbidity when misused, inadequately monitored, or incompletely reversed. Children, particularly neonates and infants, are especially vulnerable to residual neuromuscular block due to developmental pharmacological variability and limited physiological reserve. AIMS: To describe common and preventable "error traps" in pediatric neuromuscular block management and to highlight strategies to improve safety across the perioperative continuum. METHODS: This narrative re…

Pharmacologic Considerations of Anesthetic Agents in Pediatric Patients: A Comprehensive Review.

Acute pain in the pediatric population has important differences in terms of biology, intrapopulation variation, and epidemiology. Discussion as to the pharmacologic considerations of anesthetic agents, such as induction agents, neuromuscular blockers, opioids, local anesthetics, and adjuvant agents, is presented in this article. Special considerations and concerns, such as risk for propofol infusion syndrome and adverse potential side effects of anesthesia agents, are discussed. Anesthesiologists managing pediatric patients need to have a firm understanding of physiologic and pharmacologic di

Sustainable Ophthalmology Applications: From the Perspective of Strabismus and Pediatric Ophthalmology.

Ophthalmology significantly contributes to the healthcare sector's carbon footprint. Despite recent increases in sustainability research in ophthalmology, there remains limited information on initiatives specific to pediatric ophthalmology and strabismus. This review aims to examine the existing literature and provide insights into sustainability and reducing carbon footprints in these areas. Although there has been no specific assessment of carbon emissions associated with congenital and developmental cataracts, measures related to cataract surgery and operating room practices remain relevant

[Structured data collection during neonatal anesthesia: Neo Somno Log - a self-developed digital event tracker].

INTRODUCTION: Documentation of anesthetic care in Hungary remains heterogeneous, with the scope of recorded data largely determined by local institutional protocols. Anaesthesia records are predominantly paper-based, which, with variability in data content, hinders uniform, structured, and systematic data processing. Anesthesia in newborns is associated with increased risk due to physiological immaturity and ongoing postnatal adaptation, therefore the standardized and systematic capture of perioperative data is essential to improve quality of care and enhance patient safety. OBJECTIVE: This s

Effect of Low Fresh Gas Flows on Intraoperative Hypothermia Among Neonates Undergoing Abdominal Surgeries: A Randomized Controlled Trial.

INTRODUCTION: Intraoperative hypothermia remains a frequent and under-recognized complication in neonates undergoing surgery despite standard preventive measures. Low-flow anesthesia, by increasing rebreathing and conserving heat and humidity, may offer a thermo-protective effect. In this study, we compare the incidence of intraoperative hypothermia between low-flow (1 L/min) vs. routine-flow (2 L/min) anesthesia in neonates undergoing abdominal surgery. METHODS: After obtaining written informed consent from parents or legal guardians, 160 neonates scheduled for elective or emergency abdomina

An anesthesiological complication related to bone cement use during reconstructive osteoplasty using the Masquelet technique in a pediatric patient: A case report.

A 16-year-old male (ASA II, BMI 31) underwent reconstructive osteoplasty with insertion of a polymethylmethacrylate (PMMA) spacer. Shortly after cement application, he developed sudden hypotension, tachycardia, and decreased end-tidal CO₂, consistent with Grade II BCIS. Supportive management with fluids, ketamine, and norepinephrine stabilized the patient. Postoperatively, he experienced recurrent hypotension, acute kidney injury, and elevated liver enzymes and creatine kinase. Intensive care treatment resulted in recovery, and he was discharged from the ICU on day five. Bone cement implantati

Anesthesia for interventional radiology.

Pediatric patients in the neurointerventional radiology setting pose the dual challenges of caring for relatively sick patients in the outfield environment. For safe and successful practice, the anesthesiologist must not only understand the nuances of pediatric anesthesia and the physiologic demands of the cerebral lesions. They must also help maintain a team-based approach to safe, compassionate care of the child in this challenging setting. In this review article, we summarize key aspects of success for several of these topics.

[Progress and problems in pediatric surgery--the Association of Pediatric Anesthesiologists].

From the first meeting of the Japanese Society of Pediatric Surgeons in 1964 a small group discussion on anesthetic problems and patient care was held by some anesthesiologists at night. The Association of Pediatric Anesthesiologists (started in 1971) has undertaken the night meeting ever since the twelfth Annual Meeting of the Society of Pediatric Surgeons in 1975. The problems about pediatric respiratory management, neonatal emergency surgery and pediatric anesthesia were discussed frequently by doctors in different specialties. The recent decrease of mortality in neonatal surgery is thought…

Preoperative fasting in children. The evolution of recommendations and guidelines, and the underlying evidence.

This review discusses the evolution of preoperative fasting guidelines and examines the incidence of pulmonary aspiration of gastric contents and suggested treatments. Nine guidelines developed by professional societies and published in peer-reviewed journals since 1994 were identified. The recommendations on preoperative fasting for various categories have undergone only small adaptations in the following three decades in pediatric anesthesia. We found twelve published studies of the incidence of pulmonary aspiration, which ranges from 0.6 to 12 in 10,000 anesthetics in children. However, thi…

Effects of xenon anesthesia on postoperative neurocognitive disorders: a systematic review and meta-analysis.

The latest clinical trials have reported conflicting outcomes regarding the effectiveness of xenon anesthesia in preventing postoperative neurocognitive dysfunction; thus, this study assessed the existing evidence. We searched the PubMed, Embase, Cochrane Library, and Web of Science databases from inception to April 9, 2023, for randomized controlled trials of xenon anesthesia in postoperative patients. We included English-language randomized controlled studies of adult patients undergoing surgery with xenon anesthesia that compared its effects to those of other anesthetics. Duplicate studies,

Evolution of and developments in simultaneous bilateral cataract surgery. Update 2020.

The debate on role of 'simultaneous bilateral cataract surgery' (SBCS) continues. The world population has exponentially increased during last fifty years and average human lifespan has increased by a decade during the last century. This translates to ever increasing geriatric population with its inherent problem of preventable blindness because of cataract formation in the elderly. We are adding to the backlog of cataract surgeries not only in the 'developing world' but also in the 'developed world.' Times demand that we reconsider our old fashioned approach of staggering bilateral cataract s
