Insufficiency fractures of the pelvis, which almost always occur in elderly women with osteoporosis, are often misinterpreted as metastatic disease. The initial symptom of such fractures is severe pain unassociated with an obvious history of trauma. The typical sites of involvement are the sacrum, the iliac bones, and the pubis. The plain film appearance of the sacral and iliac fractures is usually subtle and easily overlooked, and bone scans will show the abnormal areas more readily. Real-time ultrasound examination of the kidneys was performed in 11 patients with transitional cell carcinoma of the renal collecting system. Each sonogram revealed a zone in the renal sinus which was less echogenic than the normal sinus and which corresponded to the extent of the tumor. Although the specificity and sensitivity of this finding are not yet known, this appearance should suggest the diagnosis of transitional cell carcinoma.

In five pigs weighing approximately 85 kg. the pelvic pressure-volume relationship was investigated. The 39 cases of renal pelvic and ureteral tumors treated in our hospital from 1974 to 1983, were reviewed retrospectively. The 39 cases included 15 renal pelvic tumors, 21 ureter tumors and 3 ureteropelvic tumors. The incidence of these tumors was 0.15% among all new outpatients. A polypoid tumor located in the renal pelvis with both epithelial and sarcomatous elements is described. The epithelial component was histologically benign and resembled urothelium. The sarcomatous component showed undifferentiated cells and rhabdomyoblastic elements. The term mesonephric adenosarcoma is proposed to describe this biphasic tumor. Although this tumor might be histogenetically related to Wilms' tumor, it is morphologically distinct, and might behave more aggressively.

The authors have evaluated 22 patients with renal pelvis tumour (RPT) by means of excretory urography and ultrasonography. Six patients also underwent arteriography. A 63-year-old man who had undergone gastric resection for gastric carcinoma in June 1981, had a dull pain in the right flank and nausea on June 2, 1982. Excretory urograms showed spontaneous rupture of the right pelvis. Metastasis or invasion of the gastric carcinoma to the right ureter was suspected. On June 9, 1982, percutaneous nephrostomy guided by ultrasound was performed. After the nephrostomy, antegrade pyelogram revealed no extravasation of the contrast medium.

The extent of osseous pelvic injury in patients suffering multiple organ trauma is difficult to assess. However, accurate information is essential in order to determine an acceptable treatment regimen, either operative (external or internal fixation), or nonoperative (bed rest and early ambulation).
