Fundamental mechanisms of host immune responses to infection.

Chronic inflammatory periodontal disease is caused by host immune responses to periodontal microorganisms. Although periodontal disease seems to progress locally in the oral cavity, the actual immune responses proceed at two sites, one being the local area peripheral to the periodontium, and the other being in secondary lymphoid tissues, such as lymph nodes and the spleen. To investigate the pathogenesis of periodontal diseases, it is essential to understand exactly how the immune system works against microbial infections. The past decade has produced remarkable advances in our understanding o…

Periodontal changes in patients undergoing radiotherapy.

BACKGROUND: The purpose of this study was to evaluate changes in the periodontium in patients who received head and neck radiation therapy. METHODS: Periodontal clinical parameters (probing depth, clinical attachment level, gingival recession, plaque index, and bleeding on probing) were assessed on 27 patients before and 6 to 8 months following radiation therapy in the head and neck area. RESULTS: The greatest changes occurred in clinical attachment level: overall, 70.3% of the patients showed a loss, with 92% evincing loss in the mandible. Attachment loss was directly related to the field o…

Microbiology and management of periodontal infections.

The term periodontal disease refers to all diseases that involve the supportive structures of the periodontium. Peridontal diseases commonly begin as a gingivitis and progress to periodontitis. Necrotizing ulcerative gingivitis (NUG) is the most fulminate form of gingivitis. The two main forms of periodontitis are chronic periodontitis (also known as adult periodontitis) and aggressive periodontitis (also known as early onset periodontitis, destructive periodontitis, and juvenile periodontitis). Gingivitis treatment involves removing dental plaques and maintaining good oral hygiene. Periodonti…

Dietary shifts may explain the incidence of periodontitis in industrialized countries.

A new hypothesis for the etiology of periodontal disease is presented. We suggest that the formation of calcifying dental plaque, together with an established gingival sulcus, leads to circumstances which favor the growth of pathogenic microbes associated with the destruction of the periodontium. We assume that, in ancient humans, this problem was avoided by the nonexistence of sulci around the teeth. Such a situation may have resulted from the usage of a hard and gritty diet which caused occlusal attrition compensated by tooth eruption. Unfortunately, in modern humans, sulcus formation is nor…

Periodontal health and disease in children and adolescents.

The prevention and treatment of periodontal diseases in children and adolescents are most desirable and the physician should be involved in the achievement of these goals. Furthermore, cooperation between the physician and the dentist may be imperative for the comprehensive treatment of some patients. One should be aware of the influence of the general growth and development of the periodontium in order to be able to differentiate between health and disease. Periodontal diseases in children and adolescents include chronic gingivitis, gingival recession, chronic periodontitis, prepubertal perio…

Acute nonlymphocytic leukemia, monocytic variant. Report of a case.

This report documents a case of a patient presenting unusual oral features of one of the rarer variants of acute nonlymphocytic leukemia, a … leukemia. Since the leukemias may imitate other oral conditions, especially various diseases of the periodontium, it is of paramount importance that the dental clinician recognize their clinical manifestations. As future treatment modalities improve, the dental clinician is seeing more leukemic patients in remission as well as more cases that have relapsed.

Engineered Delivery of Dental Stem-Cell-Derived Extracellular Vesicles for Periodontal Tissue Regeneration.

Periodontal disease begins as an inflammatory response to a bacterial biofilm deposited around the teeth, which over time leads to the destruction of tooth-supporting structures and consequently tooth loss. Conventional treatment strategies show limited efficacy in promoting regeneration of damaged periodontal tissues. Here, a delivery platform is developed for small extracellular vesicles (sEVs) derived from gingival mesenchymal stem cells (GMSCs) to treat periodontitis. EVs can achieve comparable therapeutic effects to their cells of origin. However, the short half-lives of EVs after their a

Application of Propolis in Protecting Skeletal and Periodontal Health-A Systematic Review.

Chronic inflammation and oxidative stress are two major mechanisms leading to the imbalance between bone resorption and bone formation rate, and subsequently, bone loss. Thus, functional foods and dietary compounds with antioxidant and anti-inflammatory could protect skeletal health. This review aims to examine the current evidence on the skeletal protective effects of propolis, a resin produced by bees, known to possess antioxidant and anti-inflammatory activities. A literature search was performed using Pubmed, Scopus, and Web of Science to identify studies on the effects of propolis on bone

Living cell-based regenerative medicine technologies for periodontal soft tissue augmentation.

The cultivation of human living cells into scaffolding matrices has progressively gained popularity in the field of periodontal wound healing and regeneration. Living cellular constructs based on fibroblasts, keratinocytes alone or in combination have been developed and used as alternatives to autogenous soft tissue grafts in keratinized tissue augmentation and in root coverage procedures. Their promising advantages include reduced patient morbidity, unlimited graft availability, and comparable esthetics. This manuscript reviews soft tissue augmentation and root coverage procedures using bioen

Parotid salivary secretion induced by stimulation of periodontal regions with toothbrush in humans.

The act of brushing teeth induces salivary secretion. However, there is no evidence to show which particular regions of the teeth and oral cavity in brushing induces the most effective salivary secretion. We investigated the effects of tooth brushing in different oral regions on parotid salivary secretion in humans. Saliva was collected using a modified Lashley cup. Brushing of the gingival margin was performed according to the Bass method, which may stimulate periodontal mechanoreceptors. The occlusal surface of the molars, gingiva, tongue, and palatal rugae were also brushed with a toothbrus

Dental tissues involved in exarticulation, root resorption and factors influencing prognosis in relation to replanted teeth. A review.

Prognosis in relation to an exarticulated, replanted tooth is poor. Late complications are frequent and often lead to tooth loss. However, the root of a replanted, avulsed tooth may be slowly resorbed preserving alveolar width and postpones replacement of the tooth to a more convenient time. During healing following transplantation or replantation of teeth, various dental and oral structures are involved. Late complications relate to various factors. Tissues and factors involved are reviewed. Knowledge of this subject derives mostly from results of transplantation studies in animals.

[Histologic observation of upper central incisors after surgical immediate torsion].

The purpose of this study is to verify the treatment results of rotated teeth by using surgical immediate torsion method. Seven upper central incisors in four monkeys were chosen in our experiment. The surgical treatment was accomplished by rotating the teeth in about 45 degree. The animals were sacrificed at 2, 3, 4 and 12 weeks after operation. The results showed that two of seven dental pulps were necrosis, three were injured and two were normal. Absorption of dental cementum and dentine occurred at the apex and the middle third of the root. There was no evidence of ankylosis between the to
