Unravelling the challenges in diagnosing Caffey disease.

Caffey disease, also known as Infantile Cortical Hyperostosis, is a rare yet significant self-limiting disorder that primarily affects infants. The condition is characterised by acute inflammation of the periosteum, cortical hyperostosis, and soft tissue swelling, often accompanied by systemic manifestations such as fever and irritability. The case describes a male infant with progressive left thigh swelling appearing 10 days post-immunisation, with no systemic symptoms. Examination revealed a bony-hard swelling with elevated inflammatory markers and alkaline phosphatase. Radiographs showed co…

Filum Terminale: A Comprehensive Review with Anatomical, Pathological, and Surgical Considerations.

The conus medullaris is the distal tapering end of the spinal cord, and the filum terminale (FT) is regarded as a bundle of nonfunctional fibrous tissue; therefore, some scholars call it the spinal ligament, while others describe the human FT as "remnants of the spinal cord." It was later found that in the human spinal cord, the FT is composed of an intradural segment and an epidural segment, and the end of the FT is connected to the coccyx periosteum. Because some nerve tissue is also found in the FT, as research progresses, FT may have the potential for transplantation. A lack of exhaustive …

A new surgical strategy for reconstruction of claw nail deformity.

Claw nail deformity is common in patients with fingertip injury. The optimal reconstruction remains unclear. We devised a unique strategy for reconstruction of claw nail deformity. We divided the approach into three parts: soft tissue reconstruction, bone graft and nail bed graft. In the soft-tissue reconstruction, a reverse digital arterial finger flap for the finger or an extended palmar flap advancement with V-Y plasty for the thumb was selected. A part of the distal phalanx of the second toe including periosteum was harvested as a bone graft. A nail bed graft from the big toe was performed…

Bilateral Proximal Tibial Sleeve Fractures in a Child: A Case Report.

INTRODUCTION: A sleeve fracture classically describes an avulsion of cartilage or periosteum with or without osseous fragments and usually occurs at the inferior margin of the patella. Tibial tubercle sleeve fractures in the skeletally immature are extremely rare. CASE PRESENTATION: In this report the authors describe a 12-year-old boy with no systemic disease and no steroid use who sustained bilateral proximal tibial sleeve fractures whilst playing football. Both ruptures were associated with rupture of the medial patellofemoral ligament and tear of the medial retinaculum. Treatment was perf…

Mini-implants and miniplates generate sub-absolute and absolute anchorage.

The functional demand imposed on bone promotes changes in the spatial properties of osteocytes as well as in their extensions uniformly distributed throughout the mineralized surface. Once spatial deformation is established, osteocytes create the need for structural adaptations that result in bone formation and resorption that happen to meet the functional demands. The endosteum and the periosteum are the effectors responsible for stimulating adaptive osteocytes in the inner and outer surfaces. Changes in shape, volume and position of the jaws as a result of skeletal correction of the maxilla …

Unusual presentation of obstructive sleep apnoea syndrome due to a giant mandible osteoma: case report and literature review.

Osteomas are benign lesions composed of mature compact and/or cancellous bone that grow continuously. Their pathogenesis is unknown. It has been considered to be a neoplasm, a developmental or reactive osteogenic lesion resulting from muscle traction on the periosteum, or due to trauma. Herein, we report an unusual case of giant osteoma of the mandible depressing the lateral pharyngeal wall, interfering with normal respiration during the night. The uniqueness of this case is related to snoring and sleep apnoea symptoms. In fact, he presented to our Department because daytime sleepiness hindere…

[Diagnostics and therapy of spinal metastases].

Out of all skeletal metastases 30% are located in the spine as are 10% of primary bone tumors, whereby 52% of metastases occur in the lumbar region, 36% in the thoracic spine and 12% in the cervical spine. Patients suffer from local pain caused by irritation of the periosteum due to rapid growth of the tumor or subsequent pathologic fractures which may lead to compression and neurological impairment with paresthesia, paresis and paraplegia. If the diagnosis cannot be confirmed exactly by radiological imaging and laboratory tests, a biopsy should be performed. A precise diagnosis of the tumor e…

A single injection technique for midface rejuvenation.

The correction of volume loss has become an important part of facial rejuvenation treatments, particularly in the midface region. Techniques to date have largely relied on multiple injections, fanning techniques and deep placement of product under muscle or on periosteum. This article describes a new technique to enhance volume in the midface using a single injection of cross-linked hyaluronic acid at the subdermal level and above the muscle. The technique approaches midface rejuvenation with reference to both the bony skeleton and the medial malar fat compartment. After appropriately marking …

Early diagnosis and surgical treatment of the lower labial frenum in infancy: a case report.

The lower labial frenum attached to the free gingival margin can promote local tension, resulting in tissue ischemia, promoting the development of gingival recession, as well as complicating oral hygiene, resulting in a local biofilm accumulation and chronic inflammation. In such cases, periodontal surgery is recommended and the local anatomic characteristics will be improved as early as school age. In this case report, a 7 years old patient had the lower labial frenum repositioned. After this procedure, the suture of the mucosa to the periosteum was performed around the surgical wound to prov…

Patterns of mandibular invasion by gingival squamous cell carcinoma.

PURPOSE: This study examined patterns of invasion of lower gingiva cancer into the mandible. Our goal was to clarify the most common routes for gingival squamous cell carcinoma to enter and spread within the mandible. PATIENTS AND METHODS: The subjects were 176 patients with lower gingival squamous cell carcinoma who underwent mandibulectomy; all tumors were examined histopathologically. The method of mandibulectomy was decided on the basis of conventional plain radiographic and computed tomographic findings. RESULTS: The tumor extended to the periosteum in 33%, to the cortical bone in 23%, …

Intraosseous tophaceous pseudotumor in the trigonal process of the talus.

Primary gout with destructive tophaceous pseudotumor development rarely occurs in juvenile patients with asymptomatic hyperuricemia. An expansile, intraosseous tophaceous pseudotumor on the posterior process of the talus was verified in a 14-year-old boy with posterior ankle impingement and asymptomatic hyperuricemia. A review of the literature found no similar cases had been reported. The pathogenesis of this intraosseous tophaceous pseudotumor on the trigonal process probably was caused by the penetration of urate crystals from the periosteum into the posterior process of the talus. However,…

Palsy of the deep peroneal nerve after proximal tibial osteotomy. An anatomical study.

Iatrogenic, isolated weakness or paralysis of the extensor hallucis longus muscle is a common complication in patients who have had a proxim … an area seventy to 150 millimeters distal to the fibular head; all of them ran close to the fibular periosteum. We suggest that, in some patients, the nerve supply to the extensor hallucis longus is at high risk for injury during a tibial osteotomy because of the proximity of the bone to the motor branches.

The isolated digital cord in Dupuytren's contracture: anatomy and clinical significance.

This article describes the isolated occurrence of Dupuytren's pathologic fascial cords within digits of the hand. Thirty-seven cords were found in 32 patients with nearly half (45.6%) occurring in digits other than the small finger. Almost all patients (97.3%) had other clinical evidence of Dupuytren's disease. Cords were either single (83.8%) or double (16.2%) and originated from the periosteum at the base of the proximal phalanx in conjunction with adjacent ligaments and intrinsic tendons. They proceeded in an oblique direction to displace and then cross the neurovascular bundles before inse…
