MRSA peritonitis secondary to perforation of sigmoid diverticulitis.

The occurrence of methicillin-resistant Staphylococcus aureus (MRSA) is well documented, but the pathology is usually associated with post surgical infections or long-term peritoneal dialysis. We report the case of a 50-year-old Caucasian man who presented with a one week history of left lower quadrant abdominal pain, poor appetite and nausea due to MRSA peritonitis secondary to perforated sigmoid diverticulitis. Despite a thorough search of the medical literature, we could not find that this problem has been previously described. We report this case to demonstrate the robust nature of MRSA, w…

Misdiagnosis of fetus-in-fetu as meconium peritonitis.

Fetus-in-fetu (FIF) is a rare congenital condition in which a fetiform mass is detected in the host abdomen and also in other sites such as the intracranium, thorax, head, and neck. This condition has been rarely reported in the literature. Herein, we report the case of a fetus presenting with abdominal cystic mass and ascites and prenatally diagnosed as meconium pseudocyst. Explorative laparotomy revealed an irregular fetiform mass in the retroperitoneum within a fluid-filled cyst. The mass contained intestinal tract, liver, pancreas, and finger. Fetal abdominal cystic mass has been identifie

[Clinical review of 8 patients with tuberculous peritonitis].

OBJECTIVE: With the progress of anti-tuberculous therapy, tuberculous peritonitis (TBP) has become a rare manifestation of active tuberculosis. Its early diagnosis is difficult due to lack of pathognomonic findings and specific symptoms. However, early diagnosis is important for effective treatment and for reducing fatality. MATERIALS AND METHOD: We retrospectively reviewed medical records of eight patients who were hospitalized with TBP in National Hospital Organization Omuta National Hospital during the periods between 2001 and 2009. RESULTS: Three patients were males and five were females…

Two infant deaths linked to intussusception without peritonitis.

We report two infant deaths attributable to intussusception, but without clear evidence of peritonitis. In the first instance, a 3-year-old girl had presented with abdominal pain, vomiting, and melena before her demise. Aspirated vomitus was subsequently ascertained as the immediate cause of death, due to intussusception-induced ileus. The other infant, a 2-month-old male, showed autopsy evidence of intussusception at two sites, with likely aspiration of gastric mucus. Since the circumstances surrounding his death were vague, timing of the intussusception was difficult to pinpoint. Thus, an in…

Schistosomal peritonitis: a rare cause of acute abdomen.

Schistosomiasis is a major parasitic infection of tropical areas. We present a 26-year-old male immigrant from Egypt with acute abdomen due to schistosomial infection, mimicking symptoms of acute appendicitis, with histopathological detection and representation of schistosomial eggs in the unusual area of the greater omentum. This is a rare case with findings of intra-peritoneal schistosoma ova. We review the pertinent literature and indicate the need for a new approach to infectious diseases with unusual clinical presentation, due to pathogens coming from distal geographical regions, in which

Clinical manifestations in infants with symptomatic meconium peritonitis.

BACKGROUND: Meconium peritonitis (MP) develops when bowel perforation occurs in utero or soon after birth, resulting in leakage of meconium into the peritoneal cavity. The clinical features are often variable and prenatal ultrasonography plays an important role in prenatal diagnosis. METHODS: We conducted a retrospective review of neonates diagnosed with MP in our hospital from January 1998 to December 2007. Prenatal examinations, postnatal presentations, investigations, management, patient outcomes, and possible causes were analyzed. RESULTS: Ten patients (five boys, five girls) diagnosed w…

Splenic abscess in a patient with fecal peritonitis.

Splenic abscess is a rare entity normally associated with underlying diseases. We report a case of splenic abscess with large gas formation in a non-diabetic and non-immunosuppressed patient after surgery for colon perforation. The most frequent cause of splenic abscess is septic embolism arising from bacterial endocarditis. Splenic abscess has a high rate of mortality when it is diagnosed late. Computed tomography resolved any diagnostic doubt, and subsequent surgery confirmed the diagnosis.

A case of peritonitis caused by Rhizopus microsporus.

We report a case of a 62-year-old female patient who developed peritonitis after receiving a renal transplant. Candida glabrata was detected and treated with voriconazole. As the patient did not improve under therapy, laparotomy was performed. Mould-like plaques were found on the peritoneum. Using culture as well as pan-fungal polymerase chain reaction (PCR) followed by DNA microarray hybridisation of the amplicon, the causative agent was identified as Rhizopus microsporus. Despite aggressive surgical treatment, intravenous therapy with amphotericin B and topical administration of Lavasept (po…

Tuberculous peritonitis defying diagnosis: report of a case.

A case of tuberculous peritonitis, which has been scarcely encountered in clinical practice in recent years, is reported. A 32-year-old man was admitted to our hospital complaining of abdominal fullness, anorexia, and a 15 kg weight loss. His abdomen was distended. There was neither any previous history nor recent contact with tuberculosis. The laboratory data indicated increased C-reactive protein and erythrocyte sedimentation rate, but the white blood cell count was normal. A chest X-ray examination revealed no abnormalities. Abdominal X-ray showed scattered, small-intestinal gas shadows. Ab…

Granulomatous peritonitis after laparoscopic cholecystectomy mimicking pelvic endometriosis.

BACKGROUND: Spillage of bile or gallstones, a relatively common occurrence during laparoscopic cholecystectomy, is believed to be inconsequential, but we describe a case of postlaparoscopic granulomatous peritonitis mimicking endometriosis. CASE: A 20-year-old gravida 0 presented with chronic lower abdominal pain and dysmenorrhea. Laparoscopy showed red-brown pigmented pelvic adhesions, which on biopsy showed foreign-body-type granulomatous reaction, both suggesting endometriosis, but a history of laparoscopic cholecystectomy and a positive Fouchet's stain for bile gave the true etiology of t…

[Treatment of peritonitis using ozone and hydropressive technology].

In the experiment on 144 mongrel dogs and 546 white rats the methods of ozone therapy and hydropressive treatment of the abdominal cavity have been worked out. These methods were used in the treatment of 94 patients aged from 13 to 89 years with severe forms of acute peritonitis. The control group consisted of 174 patients with peritonitis. A high antimicrobial effect was detected in the treatment of the abdominal cavity with microdisperse stream of the ozonated solution under pressure. After 1-2 procedures of the intestinal ozone dialysis the number of microbial bodies in the lumen of the sma…

[A South American tapir (Tapirus terrestris) with peritonitis].

A 6-year-old male tapir was admitted because it had been anorexic for 1 day. On admission, the tapir weighed 160 kg. Its rectal temperature … a day) intravenously was unsuccessful and the tapir died 4 days after hospitalization. At necropsy, peritonitis due to a colon infarct as well as chronic bronchopneumonia and renal tubulonecrosis were found. The antibiotic susceptibility of the bacteria isolated (Streptoc. sp., E. coli, and K. pneumoniae) from the tapir post-mortem was assessed.

Peritonitis following percutaneous gastrostomy in children: management guidelines.

BACKGROUND: To establish the incidence, timing and outcome of peritonitis following percutaneous gastrostomy (PEG) insertion in children. METHODS: Patients developing peritonitis after PEG insertion during a 5-year period (1990-95) were identified. Variables analysed included clinical presentation, management, operative findings and outcome. RESULTS: One hundred and twenty paediatric patients received 130 PEG in the 5-year period. Eight children developed peritonitis: 4 within 24 h of PEG insertion and 4 following routine PEG tube change (3-18 months later). All four patients developing earl…

[Treatment of undeveloped intestinal fistulas in purulent peritonitis].

For treatment of two consecutively opened into abdominal cavity perforating fistulas of small bowel in conditions of purulent peritonitis the author successfully used funnel-shaped obturators by Kolchenogov. In the same patients in later adhesive intestinal obstruction, which could not be eliminated by common surgical mode, continuous introduction of semifluid food and intestinal chyme collected from enterostoma (located above the obstruction) were carried out through enterostoma located above the obstruction. After resorption of inflammatory infiltrate bowel continuity was restored. Based on …

[Relaparotomy in the treatment of generalized postoperative peritonitis].

The results of treatment of 46 patients with postoperative peritonitis, caused by intestinal sutures incompetence (41.4%) and break of the abscess into the abdominal cavity (32.6%) are analysed. The mortality rate after the single relaparotomy is 30.6%, and in repeated surgeries-65.6%. Monitoring of clinical and laboratory parameters, and the use of X-ray examination, laparoscopy, ultrasound examination and CT promotes early diagnosis of postoperative peritonitis. Removal of the major cause of peritonitis, detoxication of a body and correction of metabolic disorders are the main targets of the…
