Dual inhibitory pathways link antral somatostatin and histamine secretion in human, dog, and rat stomach.

BACKGROUND & AIMS: The secretion and function of antral histamine are not known. The aims of this study were to characterize the mechanisms of histamine release from the gastric antrum of humans, dogs, and rats and to determine whether histamine can influence the secretion of somatostatin and gastrin. METHODS: Somatostatin, gastrin, and histamine secretion from superfused antral segments was measured using radioimmunoassay. RESULTS: Superfusion with thioperamide (H3 antagonist) increased somatostatin and decreased gastrin and histamine secretion in all three species; superfusion with (r)-alp

Electrophysiologic effects and antiarrhythmic efficacy of recainam in patients with supraventricular tachycardia.

Recainam is a new antiarrhythmic agent with class Ic properties. To evaluate its electrophysiologic effects and antiarrhythmic efficacy in patients with recurrent supraventricular tachycardia (SVT), programmed electrical stimulation was performed in 10 patients before and after intravenous recainam (loading dose 0.8 mg/kg, infusion 1 mg/kg/h), and in four patients on oral recainam 1,200 mg/day. Five patients had atrioventricular (AV) node reentrant tachycardia; five had AV-reciprocating tachycardia. There were no significant changes in electrocardiographic and intracardiac intervals after eith

Multikinase inhibitors in refractory metastatic colorectal cancer: an optimal sequence?

Despite major advances in management strategies, metastatic colorectal cancer remains an important clinical challenge because most patients experience progression after standard first- and secondline treatments. In the setting of refractory disease, defined as disease that progresses after 2 or more lines of treatment, the therapeutic landscape is growing. Options include regorafenib, trifluridine plus tipiracil (FTD/TPI) with or without bevacizumab, and fruquintinib, all of which received approval from the US Food and Drug Administration after showing modest survival benefits in phase 3 trial

Lenvatinib in radioiodine-refractory differentiated thyroid cancer: a real-world institutional analysis.

Lenvatinib is a standard systemic therapy for radioiodine-refractory differentiated thyroid cancer (RAI-R DTC). Although pivotal trials such as SELECT demonstrated significant efficacy, real-world evidence remains limited, particularly regarding treatment timing and the role of planned drug holidays. We retrospectively analyzed 44 consecutive patients with RAI-R DTC treated with lenvatinib between 2015 and 2024. All patients initiated therapy at 24 mg/day, with dose reductions and treatment interruptions-including planned drug holidays-implemented according to toxicity. Efficacy outcomes inclu

Lactylation and targeted therapy resistance in hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) persists as a leading cause of cancer-related mortality globally. While targeted therapies, including sorafenib and lenvatinib, serve as standard first-line treatments, their efficacy is frequently compromised by low response rates and the rapid development of resistance, posing a significant clinical challenge. Lactylation (Kla), a metabolism-driven post-translational modification stemming from lactate accumulation, has recently emerged as a pivotal regulator of HCC progression and therapeutic resistance. The extent of lactylation is governed by lactate availabi

ACSS2 drives Lenvatinib resistance in hepatocellular carcinoma through palmitoylation.

Lenvatinib, a first-line tyrosine kinase inhibitor for advanced hepatocellular carcinoma (HCC), faces clinical challenges due to acquired drug resistance. While metabolic reprogramming has been implicated in therapeutic resistance, the precise mechanistic links remain elusive. Here, we identified ACSS2-mediated metabolic-epigenetic crosstalk as a critical driver of Lenvatinib resistance. Transcriptomic and metabolomic profiling identified enhanced pyruvate metabolism in resistant HCC cells, with ACSS2 expression showing the strongest association with Lenvatinib resistance. Genetic manipulation

The KRT15 and KRT81 complex promotes lenvatinib resistance in thyroid cancer by upregulating DGKB mediated lipid metabolism.

Lenvatinib resistance is a major clinical obstacle in the treatment of radioiodine-refractory papillary thyroid cancer (PTC). Clarification of the molecular mechanisms of this resistance is of utmost importance to devise effective therapeutic strategies. We investigated the role of Keratin 15 (KRT15) in lenvatinib resistance through comprehensive in vitro and in vivo studies. Tumor and normal thyroid tissues were analyzed for KRT15 expression and correlation with patient survival. Metabolic profiling was performed to investigate KRT15-dependent alterations in lipid metabolism, namely fatty aci

Efficacy and safety of infigratinib in patients with refractory advanced gastric or gastroesophageal junction adenocarcinoma harboring FGFR2 gene amplification: a single-arm, multicenter phase 2 trial.

BACKGROUND: FGFR2 has garnered attention as a promising therapeutic target for gastric cancer (GC) because of its role in GC progression. Infigratinib, an FGFR1-3 selective tyrosine kinase inhibitor, has shown potential in preclinical GC models. METHODS: Infigratinib was evaluated in a phase 2 trial for patients with FGFR2-amplified GC or gastroesophageal junction (GEJ) adenocarcinoma who had failed two or more lines of systemic treatment for locally advanced or metastatic disease. A total of 21 patients received 125 mg of infigratinib orally once daily on a "3 weeks on, 1 week off" schedule.

Bioequivalence of a Fixed-Dose Combination of Relugolix, Estradiol, and Norethindrone Acetate: An Open-Label, Randomized, Fully Replicate Crossover Study in Healthy Postmenopausal Women Under Fasting Conditions.

Uterine fibroids are the most common benign gynecological tumors, often causing heavy menstrual bleeding (HMB), anemia, and impaired quality of life. A fixed-dose combination (FDC) of relugolix (40 mg), estradiol (1 mg), and norethindrone acetate (0.5 mg) was developed to address these limitations. This open-label, randomized, two-treatment, two-sequence, four-period, crossover bioequivalence study was conducted in 48 healthy postmenopausal women under fasting conditions. Participants received a single oral dose of either the RELUEASE (Sun Pharmaceutical Industries Limited, India) or the refer

Effects of Lenvatinib treatment for advanced differentiated thyroid cancer on cortisol deficiency.

BACKGROUND: Lenvatinib, a multi-kinase inhibitor widely used in the treatment of radioiodine-refractory differentiated thyroid carcinoma (RR-DTC), has shown remarkable efficacy and improvement in progression-free survival (PFS), although its use is associated with a variety of side effects. Among them, adrenal insufficiency (AI) remains under-recognized and potentially underestimated, and it may be involved in fatigue, one of the most frequent adverse events (AEs) Lenvatinib-related. In this prospective study, we report the incidence, development, and time course of primary AI (PAI) during Len

Activation of TREK-1 and TREK-2 Two-Pore Domain Potassium Channels by the Kv4 Channel Modulator, NS5806.

NS5806 is a diaryl-urea small molecule developed for the treatment of pain and neurological disorders. A potent activator of Kv4.3 potassium channels, it is widely used to study Kv4 channel physiology in excitable cells, including trigeminal neurons. Surprisingly, we found that NS5806 produced a significant, robust hyperpolarization of the resting membrane potential of all trigeminal neurons tested. Given the structural similarity of NS5806 to other negatively charged activator compounds with broad potassium channel activity, we investigated whether NS5806 might also modulate the two-pore doma

AMPK Pathway Activation Markers in GBM: Expression Patterns and Clinical Associations in a Real-World Study.

Phosphorylated ACC (pACC), a downstream effector of the LKB1/AMPK pathway, may support tumor bioenergetics and exert a tumor-promoting role in glioblastoma (GBM). In the randomized REGOMA trial, pACC expression was associated with improved overall survival (OS) in GBM patients treated with regorafenib, but its validity in large cohorts is unclear. This study aimed to characterize pACC and phosphorylated AMPK (pAMPK) expression in a real-world GBM cohort and to assess their prognostic and predictive value in patients receiving second-line regorafenib or fotemustine/lomustine. In this retrospect
