Twenty-one patients with alkylator-resistant plasmacell neoplasms were treated with Peptichemio (PTC) at a dose of 40 mg/m2 for 3 days every 3 weeks or, in the case of persistent leukopenia and/or thrombocytopenia, at the single dose of 70 mg/m2 every 2-3 weeks according to haematological recovery. Seventeen patients, 10 with multiple myeloma and seven with extramedullary plasmacytoma (EMP), were fully evaluable. Ingestion of dimethylnitrosamine in a low dose and for a limited period by hamsters resulted in primary intrahepatic pylephlebitis. The process began at 24 h and progressed within 2-4 weeks to narrowing and, eventually, to partial obstruction of the portal bed. Five children with complicated appendiceal inflammatory disease are reported. They presented with nonspecific signs and symptoms, but three had liver abscess and two had inflammation of the portal vein. The inflamed portal vein may act as a conduit to the liver for bacteria, or it may become thrombosed and cause portal hypertension and hypersplenism. In a 49-year-old man with temporal pallor of the left optic nerve papilla and increasing spastic paraparesis and with slight lymphocytosis in the cerebrospinal fluid perivenous retinitis was diagnosed. The peripheral retinal veins were covered with characteristic white sheaths, focal formation of new blood vessels, retinal microscopic haemorrhages and residues after vitreal haemorrhage were found. The authors discuss the possibility of a connection between disseminated sclerosis and perivenous retinitis and their possible common aetiology.
