BACKGROUND: Onychomycosis is one of the most prevalent fungal infections in the population with a higher rate of treatment failures. Photodynamic therapy (PDT) is based on the use of photosensitizers with light of an accurate wavelength, and in the presence of oxygen, reactive oxygen species are produced which induce cell death. PDT is widely used to treat non-melanoma skin cancer, and other indications are being investigated, especially in superficial infections.

BACKGROUND: Photodynamic therapy (PDT) has been explored for the treatment of recalcitrant viral warts (VW) with good results in recent publications.
METHODS: We carried out a descriptive and observational study in 19 patients with recalcitrant VW. Methyl aminolaevulinic acid (MAL) was occluded for 3 hours and pulsed dye laser (PDL) was used as the light source. VW were treated weekly until they were clear or for a maximum of six sessions.

A 74-year-old man was referred to our department for the treatment of a 15 x 15 cm superficial basal cell carcinoma (BCC) on his lumbar region. As surgical excision was considered too destructive, photodynamic therapy (PDT) was proposed. Methyl 5-aminolevulinate (MAL) cream was applied under occlusion for 3 h before illumination with a light-emitting diode lamp with an emission peak of 632 nm, a fluence rate of 83.3 mW/cm, and a light dose of 37 J/cm. Seventy-six patients with severe alopecia areata were treated with either local or oral 8-methoxypsoralen and UVA irradiations. In 43 cases (57%) a good-to-excellent result was obtained. The number of irradiations required for regrowth varied, but 20-40 treatments were sufficient in most cases with a positive response.
