[Unusual involvement of the CNS in a case of neuro-sarcoidosis (author's transl)].

A peculiar case of neuro-sarcoidosis is reported. Clinical symptoms associated ocular dysfunction, hypopituitarism due to hypothalamic disorders, hypersomnia and alveolar hypoventilation. Dynamic tests have proved hypothalamic involvement. The patient died swiftly and suddenly. Neuropathological examination showed typical granuloma consistent with sarcoidosis mainly distributed to the area of the third ventricle and spread hyperplastic gliosis of pons and medulla.

Pneumoencephalo-roulette tomography of operated primary pontine haemorrhage with long survival: report of two cases.

Pre- and postoperative pneumoencephalo-roulette tomography has been carried out in two cases of primary pontine haemorrhage with long survival. A pontine or cerebellar atrophy was revealed in case 1, in whom an intrapontine haematoma was removed. A markedly hollowed pons on the affected side has been demonstrated nearly five months after ventriculoatrial shunting in case 2.

Locked-in syndrome associated with ocular bobbing.

Locked-in syndrome together with an ocular bobbing phenomenon was observed in a 41-year-old man, an association which has not been reported earlier. Computerized tomography demonstrated an infarction of the medio-ventral portion of the pons. The patient survived and is still alive ten months after the onset of the disease with severe neurological deficit.

Upper brainstem cholinergic neurons project to ascending and descending circuits.

BACKGROUND: Based on their anatomical location, rostral projections of nuclei are classified as ascending circuits, while caudal projections are classified as descending circuits. Upper brainstem neurons participate in complex information processing and specific sub-populations preferentially project to participating ascending or descending circuits. Cholinergic neurons in the upper brainstem have extensive collateralizations in both ascending and descending circuits; however, their single-cell projection patterns remain unclear because of the lack of comprehensive characterization of individu

Hyponatremia: cerebral symptoms and role in central pontine myelinolysis.

Central nervous system symptoms due to hyponatremia is highly dependent on its acuteness and cause. Severe acute hyponatremia (serum sodium less than 125 mEq/l) often causes confusion, lethargy, seizures or frank coma due to brain oedema. If therapy is delayed, hyponatremia carries a high mortality rate, and risk of irreversible brain damage. Hyponatremia should probably be corrected to 125-130 mEq/l at a rate of 1.5-2.0 mEq/l/h. Malnourished alcoholic patients with hyponatremia may represent a special case with possible dangers of central pontine myelinolysis if a very low serum sodium is cor

Foramen of Monro mass: MRI appearances permit diagnosis of cavernous haemangioma.

Cavernous haemangiomas are most commonly found in the subcortical white matter, the pons and the external capsule. We present a case of a cavernous haemangioma that appeared as an intraventricular mass at the foramen of Monro. Despite the unusual location the diagnosis was established by MRI because of the typical appearance. The differential diagnosis included primary and secondary neoplasms at the foramen of Monro.

Initial and follow-up MRI in a case of early diagnosed Reye's syndrome.

Early MRI in a case of clinically established Reye's syndrome confirmed CT findings of compressed ventricles and additionally demonstrated signal alterations in the thalamus, mesencephalon and pons. On follow-up MRI the pontine lesion had vanished by 1 week later, while the thalamic lesion persisted for more than 2 months. The patient, however, recovered without neurological sequelae.

[Oculo-palatal myoclonus and multiple sclerosis].

A 33 year-old woman with multiple sclerosis developed bilateral palatal myoclonus. Rotatory pendular nystagmus and gaze-evoked nystagmus were present and analyzed by electro-oculographic recordings. Magnetic resonance imaging demonstrated abnormal signals in the pons. To our knowledge, this is the first report of palatal myoclonus in the course of a definite case of multiple sclerosis.

Contralateral trigeminal neuralgia as a false localizing sign in calcified chronic subdural hematoma: a case report.

A case of trigeminal neuralgia caused by a contralateral calcified chronic subdural hematoma is reported. Preoperative magnetic resonance imaging revealed rotational displacement of the pons caused by huge chronic subdural hematoma. The neuralgia was completely alleviated after removal of the hematoma. The clinical and radiological features are described, and the possible mechanism of the neuralgia is briefly discussed.

Brainstem auditory hallucinosis.

We studied three patients with findings suggesting that auditory hallucinations may occur with lesions of the tegmentum of the pons and lower midbrain. The evidence was clinical (indicating location of the lesion), radiologic (CT), pathologic in one case, and physiologic (affirming integrity of the cochleas and auditory nerves). The condition is comparable with the Lhermitte peduncular-diencephalic visual hallucinosis.

Central nervous system vasculitis in rheumatoid arthritis.

The history of a patient with rheumatoid arthritis and necrotizing vasculitis affecting only the central nervous system is reported. Clinical and pathological involvement by this process was present in both cerebral hemispheres, the pons and spinal cord. Review of the literature revealed that cerebral vasculitis in rheumatoid arthritis has been reported rarely and spinal cord vasculitis not at all.

Clinical observations on sensory effects of trigeminal dorsal root section.

Further clinical and operative support is presented for Dandy's discovery in 1929 of the presence of accessory sensory fibres of the fifth cranial nerve, running a separate course from the main sensory root at its exit from the pons. The advantages are stressed of Dandy's subcerebellar approach in selected cases for sparing those fibres as well as the motor root.

False-Negative Initial Magnetic Resonance Imaging in Acute Posterior Circulation Stroke: A Case Report Describing Locked-in Syndrome.

Locked-in syndrome is defined as quadriplegia and anarthria with the preservation of consciousness. Typically, locked-in syndrome is caused by an insult to the ventral pons secondary to trauma or vascular disease. Presented herein is a case of a locked-in syndrome with an initial MRI with no restricted diffusion and clinical deterioration over the course of four days. Repeat interval MRI demonstrated bilateral pontine ischemia.

Isolated facial myokymia as a presenting feature of pontine neurocysticercosis.

A 45-year-old healthy man presented with 2 weeks history of continuous rippling and quivering movements of his right side of face and neck suggestive of myokymia. MRI scan of the head revealed neurocysticercus in the pons. Treatment with steroids and carbamezapine produced a significant benefit. This is the first report of pontine neurocysticercosis presenting as an isolated facial myokymia.
