In a patient with psoriatic erythroderma treated with Methotrexate and later with Buthiopurine, porphyrinuria and later porphyria cutanea tarda developed, following a toxic reaction with bone marrow suppression. Hepatological examination before therapy did not reveal any signs of active liver disease.

Hydroxychloroquine and phlebotomy were compared in the treatment of porphyria cutanea tarda (PCT). Thirty patients received hydroxychloroquine (200 mg twice weekly) for 1 year and thirty-one underwent twice-monthly phlebotomies of 400 ml whole blood each, also for 1 year. Clinical signs of disease improved equally in both groups. At the end of the year, urinary porphyrin excretion had significantly improved in twenty-two out of thirty hydroxychloroquine-treated subjects, but in only eight out of the thirty-one patients who received phlebotomy. Liver histology showed significant regression of s

A 49-year-old man with porphyria cutanea tarda was treated with repeated small volume plasmapheresis. After the circadian change of urinary porphyrins was found to be higher between midnight and noon, approximately 250 mL of plasma was removed at 8 AM and was replaced by physiological saline. Clinical disease manifestations and abnormal laboratory findings rapidly improved following a total of 20 plasmapheresis during a 24-week period. The blood hemoglobin and plasma total protein levels were unchanged.

Fifteen cases of PCT were followed up during 1-4 years after the chloroquine treatment with 250 mg daily for 5 days. Clinical improvement was noted in 10 patients, and in 7 there was a full remission. The levels of uroporphyrins in the urine did not correlate with the clinical response whereas characteristic immunopathological findings in the skin became negative in six. The immunofluorescence tests on the skin biopsies of patients with PCT may be not only of supplementary significance for the diagnosis, but also in the evaluation of the course of the disease and the effects of the chloroquine

Celiac disease was diagnosed in one patient with variegate porphyria, and dermatitis herpetiformis in two patients, one with acute intermittent porphyria and the other with erythropoietic protoporphyria. The probability that celiac disease or dermatitis herpetiformis should occur in three patients with porphyria in Finland is less than 0.2%. Neither a consistent HLA pattern nor any other explanation can be offered for the association between these diseases.

Porphyria involving more than 4,000 persons occurred in Eastern Turkey from 1956 to 1961 due to ingestion of hexachlorobenzene (HCB), a fungicide added to wheat seedlings. During a site visit in 1977, 32 porphyric Turks (mean age, 29 years) were examined. Porphyrin concentrations were determined with samples of urine and stool from 29 porphyric and 10 control Turks and compared with results from 40 subjects in the United States. Distinctive clinical features were found in porphyric subjects, including hyperpigmentation, hirsutism, severe scarring, short stature, pinched facies, small hands, pa

In 2 children, both 7 years old, and with an obvious presence of clinical symptoms, the diagnosis of porphyria cutanea tarda was corroborated by the characteristic patterns of urinary and fecal prophyrin excretion. Familial studies allowed us to consider both cases as hereditary.

During hexachlorobenzene feeding of rats the following biochemical signs of a chronic hepatic porphyria developed: porphyrinuria with increase of uro- and hexacarboxyporphyrin, hepatic prophyrin accumulation of uro- and heptacarboxyporphyrin and a diminished activity of uroporphyrinogen decarboxylase in the liver, but nut in the red cells. During the 5.3 days of the intoxication the behaviour of metabolite constellation and enzyme activities was inverse. Hexachlorobenzene porphyria in rats is a pathobiochemical model of chronic hepatic prophyria, which in man becomes clinically manifest as por

Symptomatology and pathogenesis of porphyria cutanea tarda (PCT) are shortly reviewed. The different possibilities in the treatment of PCT are discussed with special reference to the phlebotomy according to Ippen and to the chloroquine administration according to the regimen described by Kordac and Semradova in 1974. 75 PCT patients were treated with chloroquine (Resochin, Nivaquine). After 9.3 +/- 3.4 months (means +/- SE) all patients excreted normal amounts of porphyrins, and the clinical symptoms disappeared. 14 of these patients relapsed after a mean period of 21 months after the withdraw

The patient in this report had typical retinitis pigmentosa which apparently was of recessive inheritance. In addition, she had an exacerbation of porphyria cutanea tarda (PCT) while taking oral contraceptives. In view of the lack of evidence for hereditary transmission of PCT it is also of interest that the patient's mother had an exacerbation of PCT while taking estrogenic medication.

We examined conjugal and blood relatives of one kindred for evidence of porphyria cutanea tarda. The disease was identified in eight family members in three generations. Four were classified as having overt porphyria cutanea tarda because of four criteria: photo-enhanced dermatosis; excessive urinary excretion of uroporphyrins; characteristic thin-layer chromatographic pattern of urinary porphyrins; and decreased activity of erythrocyte uroporphyrinogen decarboxylase. Two patients with excessive excretion of uroporphyrins or characteristic chromatograms, or both, and decreased uroporphyrinogen
