RATIONALE: Some patients with severe asthma are immunologically sensitized to one or more fungi, a clinical entity categorized as severe asthma with fungal sensitization (SAFS). It is not known whether SAFS responds to antifungal therapy.

The studies reported in this review indicate that, whereas the expectations from the molecular approach help to excite and enlighten us, the realities suggest that even some less modern approaches may provide the necessary practical solutions to problems of serology and vaccination against coccidioidomycosis. OBJECTIVE: To determine the prevalence of myositis specific autoantibodies (MSAs) and several myositis associated autoantibodies (MAAs) in a large group of patients with myositis.
METHODS: A total of 417 patients with myositis from 11 European countries (198 patients with polymyositis (PM), 181 with dermatomyositis (DM), and 38 with inclusion body myositis (IBM)) were serologically analysed by immunoblot, enzyme linked immunosorbent assay (ELISA) and/or immunoprecipitation.

A monoclonal lambda IgM euglobulin with peculiar precipitable characteristics in vitro was found in the serum of a patient with Waldenström macroglobulinaemia. The precipitation was observed at room temperature only when serum was exposed to air. Covalent disulphide bonds probably contribute to the mechanism involved in the formation of precipitate in this case.

Sera of 16 of 89 patients with systemic lupus erythematosus contained precipitating antibodies to RNA. These antibodies were found infrequently in patients with rheumatoid arthritis and Sjögren's syndrome, and not at all in patients with juvenile rheumatoid arthritis, myeloma, nor in normal individuals. These studies present evidence for the presence of antibodies to double-stranded RNA (both synthetic and viral). The fact that mammalian RNA's are probably single-stranded nucleotides suggests that these anti-RNA antibodies are directed primarily to a foreign, double-stranded RNA antigen.
