Effects of positive end-expiratory pressure on intracranial pressure and compliance in brain-injured patients.

Hypoxic pulmonary disorders and head injuries associated with increased intracranial pressure (ICP) frequently co-exist. Positive end-expiratory pressure (PEEP) improves hypoxemia but has been reported to impede cerebral venous return, potentially causing a further increase in ICP. This study examined the effects of PEEP on ICP at different levels of brain compliance. continuous ICP recordings were obtained after insertion of Scott cannulas to the lateral ventricles of seven comatose patients. Brain compliance was assessed by calculation of the pressure volume index. Patients were maintained i…

Under pressure - a sensor-based analysis of simulated prehospital pressure dressing application for hemorrhage management.

BACKGROUND: The concept of "x-ABCDE", with "X" representing exsanguinating hemorrhage, highlights the priority of immediate bleeding control in trauma care. Despite being a fundamental intervention, pressure dressing techniques lack standardization. Studies show high variability in achieved pressures, often below therapeutic targets or exceeding harmful levels, with no correlation between subjective estimates and objective measurements. OBJECTIVES: To objectively assess current practices of pressure bandage application and identify influencing factors including material type, technique, and p…

Jet ventilation in severe airway stenosis: how pressure and frequency shape airway pressure and ventilation.

BACKGROUND: Jet ventilation is widely used during rigid bronchoscopy, particularly in patients with airway narrowing, yet evidence guiding the selection of driving pressure and frequency in severely narrowed airways remains limited. OBJECTIVES: To investigate how jet driving pressure and frequency interact with airway geometry to determine airway pressure, ventilation, and gas exchange during normal-frequency jet ventilation (NFJV) and high-frequency jet ventilation (HFJV) in normal and severely narrowed airways. METHODS: Jet ventilation was applied to a 3D printed adult airway model under t…

Ambulatory Blood Pressure Profiles in Children With Primary Headache Disorders Despite Normal Office Blood Pressure.

OBJECTIVES: Headache is a common complaint in children and may indicate early hypertension (HT), an emerging concern in pediatrics. Children with primary headache (PH) may exhibit elevated blood pressure (BP) load or impaired nocturnal dipping despite normal office BP. This study aimed to evaluate the utility of ambulatory blood pressure monitoring (ABPM) in children with PH. METHODS: A case-control study was conducted including 37 children aged 8-17 years diagnosed with PH according to the International Classification of Headache Disorders (ICHD-3) criteria, all with office BP below the 95th…

Wearable blood pressure sensors for cardiovascular monitoring and machine learning algorithms for blood pressure estimation.

With advances in materials science and medical technology, wearable sensors have become crucial tools for the early diagnosis and continuous monitoring of numerous cardiovascular diseases, including arrhythmias, hypertension and coronary artery disease. These devices employ various sensing mechanisms, such as mechanoelectric, optoelectronic, ultrasonic and electrophysiological methods, to measure vital biosignals, including pulse rate, blood pressure and changes in heart rhythm. In this Review, we provide a comprehensive overview of the current state of wearable cardiovascular sensors, focusin…

Correlation and consistency analysis between various indirect portal pressure gradients and actual portal pressure gradient.

OBJECTIVE: To evaluate the correlation and consistency between hepatic venous pressure gradient(F-HVPG) calculated as the wedged hepatic venous pressure (WHVP) minus free hepatic venous pressure (FHVP), I-HVPG calculated as WHVP minus inferior vena cava pressure (IVCP) in the hepatic segment, and portal pressure gradient (PPG). METHODS: Data were collected from 112 patients with portal hypertension undergoing transjugular intrahepatic portosystemic shunt (TIPS) along with HVPG measurement. FHVP, IVCP, WHVP, and portal venous pressure (PVP) were collected intraoperatively. Pearson's correlatio…

Validation of a noninvasive device for measuring filling pressure against invasive pulmonary capillary wedge pressure.

BACKGROUND: Intravascular congestion, particularly elevated cardiac filling pressures, is a key component of heart failure and often precedes symptom onset. Early intervention is critical, yet non-invasive tools to detect congestion before symptoms appear are limited. OBJECTIVES: This study evaluates the accuracy of a non-invasive filling pressure (NIFP) device in assessing cardiac filling pressure, compared to pulmonary capillary wedge pressure (PCWP) obtained via Swan-Ganz catheterization. METHODS: Ninety-eight patients undergoing cardiac surgery had NIFP device measurements immediately be…

Tricuspid regurgitation velocity reflects pulmonary artery mean pressure better than systolic pressure in cardiogenic shock.

In this exploratory analysis of 35 patients with ischemic cardiogenic shock, we evaluated the correlation between tricuspid regurgitation velocity (TRV) and pulmonary artery catheter-derived pressures. TRV correlated best with pulmonary artery mean pressure (r = 0.54, p = 0.0009), more than with systolic or diastolic pressures. A TRV threshold of 2.3 m/s identified mean pressure >25 mmHg with 81% sensitivity and 62% specificity. These findings suggest that TRV may serve as a non-invasive indicator of elevated pulmonary pressure in critically ill patients when invasive monitoring is unavailable…

Treatment of headache reduces blood pressure among most patients with migraine and elevated blood pressure.

BACKGROUND: It is unclear whether patients with elevated BP and acute headache require treatment with anti-hypertensive medication, anti-headache medication, or both. We determined the impact of migraine medication on blood pressure and pain scores among emergency department patients with moderate or severe migraine and elevated blood pressure. METHODS: This study uses data collected from four emergency department-based migraine clinical trials. Patients were included if they had moderate or severe migraine. Patients received one of the following medications or medication combinations: Metocl…

Age-Related Blood Pressure Gradients Are Associated With Blood Pressure Control and Global Population Outcomes.

BACKGROUND: The strong relationship between blood pressure (BP) and age is well known. Limited evidence suggests that a steeper age-BP slope may be associated with an increased risk of adverse outcomes. The May Measurement Month campaign enables an investigation of geographic, socioeconomic, and sex differences in age-related BP gradients and their association with public-health outcomes. METHODS: Cross-sectional, annual global BP May Measurement Month screening data were analyzed. Average systolic BP and age-related BP slopes across different age groups were calculated to assess regional, so…

Center of Pressure Measurement Accuracy via Insoles with a Reduced Pressure Sensor Number during Gaits.

The objective was to compare simplified pressure insoles integrating different sensor numbers and to identify a promising range of sensor numbers for accurate center of pressure (CoP) measurement. Twelve participants wore a 99-sensor Pedar-X insole (100 Hz) during walking, jogging, and running. Eight simplified layouts were simulated, integrating 3-17 sensors. Concordance correlation coefficients (CCC) and root mean square errors (RMSE) between the original and simplified layouts were calculated for time-series mediolateral (ML) and anteroposterior (AP) CoP. Differences between layouts and bet…

Intraoperative Intracranial Pressure Monitoring as an Intraoperative Guide During Operations for Relieving Elevated Intracranial Pressure.

BACKGROUND: Intracranial pressure (ICP) is a well-established measure in managing not only traumatic brain injury but also nontraumatic intracranial bleeding or edema. When ICP increases despite nursing or medical management, ICP may be reduced via surgical measures. Deciding whether to perform a craniotomy vs. craniectomy (whether the bone flap is replaced or not, respectively) is commonly made intraoperatively following preoperative planning. While ICP monitoring (ICPm) is standard pre- and postoperatively, its intraoperative utility remains understudied. METHODS: We conducted a study utili…

Closed-incision negative pressure wound therapy (NPWT) in elderly patients following sacral pressure sore reconstruction.

BACKGROUND: Pressure sores significantly affect elderly patients, with the sacrum being especially vulnerable, often due to its proximity to the anus and potential for fecal contamination. Despite preventive measures such as frequent repositioning and specialized cushions, some sores still necessitate surgical intervention. Postoperative care focuses on monitoring, hygiene, and pressure alleviation. This study assessed the efficacy of Closed-Incision Negative Pressure Wound Therapy (CI-NPWT) for postoperative wound management in patients with sacral pressure sores treated with local flaps. ME…
