The prevalence of serious mental illnesses in prisons is estimated to be significantly higher than in the community. The antipsychotic medication clozapine is very effective in managing treatment-resistant psychosis and may also reduce suicidal and self-injurious behaviors but is underused due to several logistic challenges. A partnership between the North Carolina prison system and University of North Carolina School of Medicine established a consultative system for clozapine initiations that has led to a 390% increase in the number of incarcerated people using clozapine over a 5-year period.…

Introduction: German prisons face organizational and time-consuming difficulties in access to medical specialties. Since 2019, our institute offers interdisciplinary video consultations with spatially independent dermatological support for German prisons. Methods: Documentation of n = 200 consultations between February 2020 and July 2021 with retrospective analysis of dermatological conditions and consultation requests. Results: Most cases (98.0%; 196 of 200) were performed during a regular weekly teleclinic and only few cases on urgent demand. The average duration of the skin disease before r…

Background: Opioid use disorder (OUD) is common among incarcerated persons and risk of overdose and other adverse drug-related consequences is high after release. Recognizing their potential to reduce these risks, some correctional systems are expanding access to medication for opioid use disorder (MOUD). This study explored the experiences and perspectives of formerly incarcerated individuals on MOUD use while incarcerated and after release. Methods: We interviewed 53 individuals with self-reported OUD who were released from New Jersey state prisons. Interviews explored motivations to use MOU…

The COVID-19 pandemic has had far-reaching consequences, resulting in millions of infections and deaths worldwide. Given the vast impact of the virus and how it has highlighted health care access/treatment concerns within the general public, this study examined how these issues specifically impacted COVID-19 cases and deaths among incarcerated individuals. In particular, we examined how the method of medical care delivery (i.e., provided by correctional versus contracted practitioners) impacted the reported rates of COVID-19 cases/deaths within correctional institutions. Findings indicated tha

Although widespread vaccination in correctional facilities is crucial for preventing COVID-19 morbidity and mortality in these institutions and their surrounding communities, there are little data on how to effectively perform vaccine outreach to people experiencing incarceration who remain unvaccinated. In this article, we describe lessons learned from a successful vaccine education initiative in California state prisons and describe opportunities for application to other correctional settings. (Am J Public Health. 2022;112(11):1543-1545. https://doi.org/10.2105/AJPH.2022.307042).

PURPOSE: Suicide prevention and intervention in prisons is a challenge. Prisons were not designed to be clinical facilities, yet with the growing numbers of people who face mental health challenges in prisons, staff require knowledge and skills to adequately address mental health crises, especially suicide. This study aims to: describe trends in suicide attempts and completions within one state's prison system and measure staff knowledge and preparedness to address suicide.
DESIGN/METHODOLOGY/APPROACH: This research uses a nonexperimental research design and two data sources. Administrative d…

This study examined factors influencing health information and mental health support seeking among individuals in U.S. prisons using the 2014 data from the Programme for the International Assessment of Adult Competencies. Results showed that not only are age, race, health status, receiving health information (e.g., from health experts, friends, and other incarcerated individuals), and readiness to learn associated with health information seeking from the media, but so too are competencies such as literacy, numeracy, and problem-solving skills. Years of education moderated the association betwe…

Objectives. To examine SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) epidemiology and risk factors among Federal Bureau of Prisons (BOP) staff in the United States.Methods. We calculated the SARS-CoV-2 case rate among 37 640 BOP staff from March 12 to June 17, 2020, using payroll and COVID-19-specific data. We compared occupational factors among staff with and without known SARS-CoV-2 using multiple logistic regression, controlling for demographic characteristics. We calculated relative risk among staff in stand-alone institutions versus complexes (> 1 institution).Results. SARS…

BACKGROUND: While the delivery of healthcare services within prison systems is underpinned by different models, access to timely and optimal healthcare is often constrained by multifaceted factors. Telehealth has been used as an alternative approach to conventional care. To date, much of the focus has been on evaluation of telehealth interventions within certain geographical contexts such as rural and remote communities. Therefore, the aim of this systematic review was to synthesise the evidence base to date for the impacts of, and outcomes from, telehealth delivered in prisons.
METHODS: This…

With a prison population of approximately 9000 women in England, it is estimated that approximately 600 pregnancies and 100 births occur annually. Despite an extensive literature on the sociology of reproduction, pregnancy and childbirth among women prisoners is under-researched. This article reports an ethnographic study in three English prisons undertaken in 2015-2016, including interviews with 22 prisoners, six women released from prison and 10 staff members. Pregnant prisoners experience numerous additional difficulties in prison including the ambiguous status of a pregnant prisoner, physi…
