Guillain-Barre syndrome (GBS) is the most prevalent and severe form of acute paralytic neuropathy, commonly triggered by infections and characterized by an abnormal autoimmune response. Reports of multispace deep fascial infection (DFI) in the head and neck complicated by GBS are exceedingly rare. We report a 69-year-old woman with DFI who developed postoperative limbs weakness. GBS was diagnosed after neurology consultation and cerebrospinal fluid (CSF) puncture examination. Then the patient was treated with intravenous immunoglobulin, as well as supportive care and rehabilitation exercise. A

Necrotizing soft tissue infections (NSTIs) are characterized by "flesh-eating bacteria" that are rapidly progressive and require immediate diagnosis and aggressive surgical debridement along with broad-spectrum antibiotics. Common pathogens, such as Streptococcus pyogenes, Staphylococcus aureus, Enterococcus coli, and Clostridium, are the usual culprits of this disease. While NSTIs have a relatively low incidence, they are associated with higher rates of morbidity and mortality. Due to the aggressive nature of the pathogen, NSTI can cause widespread necrosis of soft tissue and muscle, leading

A previously well 54- year-old woman presented with a short history of diplopia, cognitive decline, hallucinations and hypersomnolence. The patient had progressive deterioration in short-term memory, ocular convergence spasm, tremor, myoclonus, gait apraxia, central fever, dream enactment and seizures. Results of investigations were normal including MRI brain, electroencephalogram, cerebrospinal fluid (CSF, including CSF prion protein markers) and brain biopsy. The patient died from pneumonia and pulmonary embolus. Brain postmortem analysis revealed neuropathological changes in keeping with Fa

This is a case of a 54-year-old woman managed as a case of osteogenesis imperfecta type 1 who sustained a left subtrochanteric fracture and eventual ankylosis of both hips after surgery and immobilisation. These injuries rendered her bedridden, maximally assisted in transitions and transfers, and unable to be positioned past 30° of backrest elevation. The patient underwent a bilateral Girdlestone procedure and had tailored progressive postoperative rehabilitation in both the inpatient and outpatient settings. The patient also continued to receive bisphosphonates during her preoperative and pos

Cervical spinal vascular abnormalities commonly present with progressive myelopathy as a result of venous congestion. They are not very prone to bleed and tend to be underdiagnosed due to their subtle clinical presentation. We came across a rare case of intracranial subarachnoid haemorrhage caused by cervical spinal dural fistula in the Imperial College Healthcare NHS Trust Hospitals/UK in June 2020. We diagnosed the patient under strict evidence base medicine guidance, which otherwise would have been missed. We discussed the case in several multidisciplinary team (MDT) meetings, and patient w

Transcatheter aortic valve replacement (TAVR) techniques are rapidly evolving, and results of published trials suggest that TAVR is emerging as the standard of care in certain patient subsets and a viable alternative to surgery in others. As TAVR is a relatively new procedure and continues to gain its acceptance, rare procedural complications will continue to appear. Our case is about an 89-year-old male with extensive past medical history who presented with progressive exertional dyspnea and angina secondary to severe aortic stenosis. Patient got TAVR and his postoperative course was complica

Fibrodysplasia ossificans progressiva (FOP) is a rare genetic disorder characterized by congenital malformations of the great toes and progressive heterotopic ossification of connective tissue that begins during the first decade of life. Our patient presented with intrauterine growth retardation, respiratory distress, neonatal onset soft tissue masses, bilateral hallux valgus, and congenital anomalies of the thyroid and uterus. She was initially diagnosed with atypical infantile myofibromatosis based on clinical and pathological findings. She underwent whole-exome sequencing (WES) as part of t

We evaluated a program for prevention of preterm birth involving early detection and aggressive intervention with subcutaneous terbutaline pump therapy in a high-risk, private patient population. Risk factor screening, frequent cervical examinations, and intensive patient education were used to detect preterm labor before it progressed to an advanced stage. Home terbutaline pump therapy was prescribed for patients with uterine contractions associated with progressive cervical change, after stabilization with IV magnesium sulfate. In this study of 51 patients, home terbutaline pump therapy was

UNLABELLED: Progressive dysphagia in older adults presents a diagnostic challenge with a broad differential including neuromuscular, structural and malignant causes. Dysphagia associated with aspiration may lead to significant respiratory compromise requiring intensive care management. We report the case of a 78-year-old patient with rheumatoid arthritis treated with methotrexate who presented with progressive dysphagia to solids and liquids, weight loss, and cough. She subsequently developed aspiration pneumonia complicated by repeated type 2 respiratory failure and required mechanical ventil

BACKGROUND: Hallux valgus (HV) is a common, progressive foot deformity associated with pain, functional limitation, and reduced quality of life. Patients are frequently referred for orthopaedic consultation contributing to prolonged waitlists. This study explored patient experiences, perceptions, and expectations of HV management while awaiting orthopaedic review.
METHODS: A convergent mixed-methods design was used involving patients on orthopaedic waitlists at Canberra Health Services and Northern Health, Australia. Participants completed a survey assessing demographics, HV severity, health-

Rheumatoid Arthritis-associated Interstitial Lung Disease (RA-ILD) is a serious extra-articular manifestation associated with significant morbidity and mortality. We present the case of a 48-year-old incarcerated male with seropositive rheumatoid arthritis who was clinically stable on tofacitinib and methotrexate for over one year. Following the discontinuation of tofacitinib, which the patient attributed to correctional facility formulary changes and restrictions, he developed progressive respiratory symptoms. He presented to the hospital two months later with acute hypoxic respiratory failur

OBJECTIVE: The diagnosis of secondary progressive multiple sclerosis (SPMS) is often established retrospectively leading to a delay in detection. This work presents a clinical care algorithm that aims to facilitate the recognition of the secondary progressive phase of the disease, analyzing its usefulness and the feasibility of its implementation in routine clinical practice.
METHODS: The algorithm was developed in four phases: 1) choice of validated diagnostic tools for the detection of progression; 2) assessment of these tools based on experience of use, applicability, time consumed, percei

INTRODUCTION: Multiple sclerosis (MS) is the most common cause of progressive neurological disability in young adults. The use of advance care planning (ACP) for people with progressive MS (pwPMS) remains limited. The ConCure-SM project aims to assess the effectiveness of a structured ACP intervention for pwPMS. The intervention consists of a training programme on ACP for healthcare professionals caring for pwPMS, and a booklet to be used during the ACP conversation. Herein, we describe the first two project phases.
METHODS: In phase 1 we translated and adapted, to the Italian legislation and

BACKGROUND: During the Coronavirus disease 2019 (COVID-19) pandemic, different neurological manifestations have been observed. However, only a few cases of Guillain-Barre syndrome (GBS) and COVID-19 have been reported. Therefore, the aim of this study is to investigate a case of concomitant GBS with COVID-19 in Colombia.
CASE PRESENTATION: A 39-year-old woman was admitted to a teaching hospital in Barranquilla, Colombia with a history of progressive general weakness with lower limb dominance. Previous symptoms such as ageusia, anosmia and intense headache were reported. Upon admission, facial

HISTORY AND ADMISSION FINDINGS: A 55-year old man suffers from progressive, distinctive dyspnoea and physical weakness since 5 days. Due to ST-segment changes in the ECG and a positive troponin-test, the primary care physician initiates an hospitalization.
INVESTIGATIONS: After admission, the laboratory tests confirm the elevated troponin-values, and show additionally elevated pro-brain-natriuric-peptide-values. The coronary angiography presents a highly reduced left ventricular function, an aortic insufficiency III° and a coronary heart disease.
DIAGNOSIS, TREATMENT AND COURSE: After clinic

Pembrolizumab, one of many novel immune checkpoint inhibitors (ICPi), is a monoclonal antibody that enhances immunity against cancer cells. Extensive escalation in immune activity predisposes to unsought immune-related adverse events. Due to progressive mesothelioma, a 67-year-old man was referred to the research unit and enrolled in a clinical trial with a cluster of differentiation (CD) 27 chemotherapeutic agent. He began crossover treatment and received just two doses of pembrolizumab, 33 and 16 days prior to admission. He subsequently presented to the emergency department with three days o

BACKGROUND: Carcinoid heart disease is a rare complication of neuroendocrine tumours caused by chronic exposure to vasoactive substances, leading to progressive valvular dysfunction and right heart failure. The use of veno-arterial extracorporeal membrane oxygenation (VA-ECMO) in this context remains controversial, as it may exacerbate carcinoid crisis by bypassing the pulmonary metabolism of these vasoactive mediators.
CASE SUMMARY: We describe a patient with clinical suspicion of a neuroendocrine tumour and severe carcinoid heart disease who developed refractory cardiogenic shock. VA-ECMO w

Acute epiglottitis in adults is an uncommon but potentially life-threatening condition that can rapidly progress to airway compromise. Epiglottic abscess represents a rare and severe complication associated with increased morbidity and diagnostic challenges. We report a case of a 36-year-old male admitted to the emergency department in Belgium with progressive odynophagia, dysphonia, and hypersalivation despite prior oral antibiotic therapy. Laboratory investigations revealed a marked inflammatory response, including leukocytosis and elevated C-reactive protein. Contrast-enhanced computed tomo
