Reframing Antimicrobial Stewardship as Systems Governance: Aligning Regulation, Diagnostics, Antibiotic Use Measurement, and Surveillance to Address Antimicrobial Resistance.

Antimicrobial stewardship (AMS) is often framed as a set of prescribing-focused interventions aimed at optimizing antibiotic selection, dose, route of administration, and duration. In practice, AMS also functions as a systems discipline. Embedded within governance and regulatory frameworks, diagnostic capacity, antimicrobial consumption and use patterns, and surveillance infrastructure, AMS operates within real-world structural and economic constraints, such as resource limitations, conflicting pharmaceutical market incentives, and health system fragmentation. In lower- and middle-income count

Lessons from the catastrophic floods in southern Brazil: geographic and environmental factors, vulnerabilities, and the resilience of nephrology services during natural disasters.

PURPOSE OF REVIEW: In April and May of 2024, the state of Rio Grande do Sul in southern Brazil faced one of the most devastating natural disasters in its history. Torrential rains caused catastrophic flooding, affecting over 2.4 million people and damaging critical infrastructure. Among the most vulnerable were individuals with chronic kidney disease, particularly those dependent on dialysis or immunosuppressive therapy following kidney transplantation. This narrative report describes the impact of the floods on nephrology services, highlights systemic vulnerabilities, and outlines the respons

Point of care tests for invasive fungal infections: a blueprint for increasing availability in Africa.

Invasive fungal infections (IFIs) such as cryptococcosis, disseminated histoplasmosis, and chronic pulmonary aspergillosis are significant causes of morbidity and mortality in Africa. Lack of laboratory infrastructure and laboratory personnel trained in diagnostic mycology hamper prompt detection and management of IFIs on the continent. Point-of-care tests (POCT) obviate the need for complex infrastructure, skilled technicians, and stable electricity and have had major impacts on the diagnosis of bacterial, viral, and parasitic infections in low- and middle-income countries. Over the last 10 y

The role of surveillance in polio eradication and identification of emerging viral encephalitis.

In 1988, when the World Health Assembly resolved to eradicate paralytic poliomyelitis, polio was endemic in 125 countries on 5 continents with an estimated 350,000 cases annually. By 2002, the number of countries was reduced to 7 and the number of cases by greater than 99%. Instrumental in this extraordinary progress is the timely detection and investigation of all cases of acute flaccid paralysis and the examination of stool samples in an accredited WHO Global Network laboratory. The Network consists of 124 National Poliovirus Laboratories, 15 Regional Reference Laboratories, and 7 Global Spe

Implementing genetics clinic for hereditary cancer in resource-constrained settings: a narrative review.

BACKGROUND: Hereditary cancer syndromes, driven by pathogenic germline mutations such as those in BRCA1 and BRCA2 and mismatch repair genes, represent a critical but under-addressed frontier in cancer care in low- and middle-income countries (LMICs), including India. Genetic clinics-multidisciplinary platforms offering counselling, testing, and cascade screening-have emerged globally as foundational to precision oncology. However, their implementation in resource-constrained settings remains highly uneven, hindered by infrastructural gaps, socioeconomic barriers, and sociocultural complexities

'A priori' external contextual factors and relationships with process indicators: a mixed methods study of the pre-implementation phase of 'Communities in Charge of Alcohol'.

BACKGROUND: It is widely recognised that complex public health interventions roll out in distinct phases, within which external contextual factors influence implementation. Less is known about relationships with external contextual factors identified a priori in the pre-implementation phase. We investigated which external contextual factors, prior to the implementation of a community-centred approach to reducing alcohol harm called 'Communities in Charge of Alcohol' (CICA), were related to one of the process indicators: numbers of Alcohol Health Champions (AHCs) trained. METHODS: A mixed meth

Taking the bus? Barriers and facilitators for adolescent use of public buses to school.

Transport to school can contribute significantly to adolescents' physical activity but in New Zealand - as in many other countries around the world - many adolescents are driven to school. Public transport offers an opportunity to integrate incidental active transport into school commutes. In this paper, we bring together multiple sources of data into a multi-method study to elucidate the barriers to and facilitators of public transport use by adolescents for school travel in Dunedin, New Zealand, a city with low rates of public transport use. The data include a public bus survey from Otago Sc
