Several indirect findings suggest that host-related factors influence susceptibility to Coxiella burnetii infection. We decided to explore the influence of genetic factors related to both innate and adaptive immunity in acute Q fever susceptibility. TLR2 (Arg753Gln) and TLR4 (Asp299Gly, Thr399Ile) polymorphisms, along with HLA-DRB1 alleles, were analyzed for 38 patients with acute Q fever, 38 matched controls, and 121 blood donors. No significant associations were found for TLR polymorphisms. However, HLA-DRB1*04 was more frequent in patients. HLA-DRB1 variants may play a role in Q fever susce…

BACKGROUND: Patients with bicuspid aortic valves (BAVs) are at increased risk of infective endocarditis. Coxiella burnetii is a rare cause o … valve replacement, and concurrent splenectomy. Serologic and tissue studies revealed C burnetii and Q fever.
DISCUSSION: Chronic Q fever is a clinical syndrome that presents months to years after C burnetii infection with nonspecific symptoms. Due to its intracellular and fastidious nature, treatment requires a prolonged antibiotic course and standard su …

Q fever is a zoonosis with a worldwide distribution caused by Coxiella burneti. Acute manifestations include a self-limited febrile illness which may associate headache, atypical pneumonia, or hepatitis. Neurological manifestations are rare, and they occur in less than half of the patients. Of these, approximately 1% present peripheral nervous system involvement, and, when present, it is difficult to diagnose because it has multiple manifestations such as mononeuritis multiplex, plexopathy or Guillain Barre syndrome. Due to the high rate of neurological sequelae, early diagnostic suspicion and…

Reported cases of Q fever in people living in urban areas after occasional contact with farm animals or infected pets such as dogs and cats have been increasing. The diagnosis of Q fever is usually laborious due to unspecific and variable clinical manifestations. The most common clinical presentation is an influenza-like illness with varying degrees of pneumonia and hepatitis. Acute hepatitis is more frequent than pneumonia in countries where the disease is endemic, such as in Portugal. We report a case of acute Q fever with hepatic and bone marrow involvement presented as fever of unknown ori…

BACKGROUND: After the Q fever outbreak in the Netherlands between 2007 and 2010, more than 300 patients with chronic Q fever have been identified. Some patients were also diagnosed with systemic sclerosis, a rare immune-mediated disease. We aimed to increase awareness of concomitant chronic Q fever infection and systemic sclerosis and to give insight into the course of systemic sclerosis during persistent Q fever infection.
MATERIALS AND METHODS: Chronic Q fever patients were identified after the Dutch Q fever outbreak in 2007-2010. Systemic sclerosis was diagnosed by a scleroderma expert and…

BACKGROUND: Coxiella burnetii, the causative agent of Q fever, causes abortions in animals. Its effects on pregnancy in humans and the management of Q fever in pregnancy are uncertain.
OBJECTIVES: To summarize data on the effects of Q fever on pregnancy in women, the effects of pregnancy on Q fever complications and the optimal screening and management of Q fever during pregnancy.
SOURCES: We searched for studies reporting on Q fever during pregnancy in women. We included randomized and observational studies, seroprevalence studies, case series and case reports, including clinical and histop…
