Radiologic diagnosis of Hirschsprung's disease utilizing rectosphincteric reflex.

Artificial balloon distension of the rectum caused a reflex opening of the anal canal as seen during barium enema in 10 out of 10 normal children and in 7 of 7 children with idiopathic constipation. Reflex opening of the anal canal was not observed in any of 15 children with proven Hirschsprung's disease. This lack of response was independent of the length of aganglionosis or previous diverting colostomy and corresponded to the fact that on manometric study the intraluminal pressure of the anal canal fell in normal children but not in the children with Hirschsprung's disease. This finding grea…

Auditory brain-stem response and acoustic reflex test.

Auditory brain-stem response (ABR) and acoustic reflex test (ART) results were reviewed for 30 patients with cerebellopontile angle tumors and for 30 patients without tumor. Patients included in the nontumor group were selected to match the audiometric configurations for individual patients in the tumor group. The ABR waveforms were abnormal for all but one of the patients with tumor and for seven of the patients without tumor. The ART results were abnormal for 25 of the patients with tumor and two of the patients without tumor. Patients without tumor yielding abnormal ABR results usually had …

Altered cardiovascular reflex responses during positive pressure breathing.

Cardiovascular responses during hyperinflation produced by positive end-expiratory pressure (PEEP) are considered to be reflexly influenced by pulmonary mechanoreceptors. Numerous studies have indicated heart and vascular effects attributed to mechanical events and cardiopulmonary mechanoreflexes. Yet interactions of these modalities with the systemic baroreflexes are not clear. We examined aspects of these modulatory interactions by distinguishing changes in pulmonary, heart, and vascular responses during PEEP-hyperinflation before and after progressive elimination of chemo-, mechano-, and ba…

Bainbridge reflex in conscious, unrestrained, and tranquilized baboons.

The effects of volume loading were examined in conscious, unrestrained, and tranquilized baboons (20-25 kg) 1-3 mo after instrumentation wit … heart rates, volume loading did not alter heart rate. These experiments suggest that the Bainbridge reflex exists in larger primates with relatively low spontaneous heart rates.

Bone scintigraphy in the reflex sympathetic dystrophy syndrome.

Sixty-four consecutive patients were studied for possible reflex sympathetic dystrophy syndrome (RSDS). They were divided into five groups, based upon specific clinical criteria, and the radiographic and scintigraphic findings in each group were examined. Osteoporosis was the most common radiographic abnormality, present in 69% of subjects with definite, probable, or possible RSDS, as compared with 21% opf those with RSDS. Scintigraphic abnormalities were noted in 60% of RSDS patients but in only 7% of the others. These findings included increased blood flow and enhanced periarticular radionuc…

Laryngeal reflex apnea in the chemodenervated newborn piglet.

Effects of peripheral arterial chemodernervation on laryngeal reflex-induced apnea were studied in 18 piglets of either sex varying in age from 4 to 63 days. The distal trachea was cannulated with a cuffed endotracheal tube to secure a free airway and permit ventilatory measurements with a pneumotachograph. The proximal trachea was used to introduce fluids into the larynx. Water elicited apnea, bradycardia, and arterial hypertension, whereas saline caused only transient disturbances. Electrical stimulation of the superior laryngeal nerves reproduced, and conduction anesthesia ablated, the effe…

Effects of chemoreceptor stimulating agents on reflex bradycardia.

Effects of carotid body chemoreceptor stimulation on reflex bradycardia were studied by injecting various pharmacological and physiological agents into the external carotid artery of rabbits. Reflex bradycardia was induced by ACh and nicotine in doses from 3 micrograms to 10 micrograms. The response to ACh was markedly potentiated by physostigmine and small doses of atropine, while it was completely blocked bymecamylamine and a large dose of atropine. The nicotine-induced bradycardia was also inhibited by mecamylamine and a large dose of atropine. After reserpinization, the responses to ACh an…

The neuronal pathway subserving the pupillary light reflex.

The pupillary light reflex was investigated using electrical stimulation along the pathway and recording in the short ciliary nerves. The discharge of single units in the ciliary ganglion was compared during diffuse light stimuli and electrical stimuli. It was concluded that the early reflex discharge in the short ciliary nerves following electrical stimulation on the optic tract is due to excitation of fibres active during the pupillary light reflex. The light reflex is conveyed by slow (less than 10 m/sec) optic tract fibres which synapase in the medial part of the pretectal area. In turn, p…

Acoustic reflex threshold measurement in hearing aid selection.

Aided speech discrimination scores were obtained for 18 patients with sensorineural hearing loss. Monosyllabic word discrimination lists with a competing message of connected discourse at a signal-to-noise ratio of +10 dB were used as the test stimuli, and measurements were made at four hearing aid gain settings for each subject. One of the gain settings was determined by measuring the acoustic reflex threshold for filtered noise in the ear contralateral to the aided ear. Two other settings were at +/- 10 dB relative to the reflex threshold gain setting; the fourth setting was determined with …

Mechanisms of reflex cardiac arrest in tetraplegic patients.

Four patients with physiologically complete high cervical spinal-cord lesions, sustained within the previous 6 weeks, were observed. All needed intermittent positive-pressure ventilation. In the stage of spinal shock, stimuli to the trachea induced bradycardia, and in two patients cardiac arrest resulted. The bradycardia occurred when the patients were hypoxic, and seemed to be due to a vaso-vagal reflex. Normally this reflex is opposed by sympathetic activity, and during hypoxia by increased pulmonary (inflation) vagal reflex activity due to increased breathing. In these patients, however, co…

Lower limb reflex changes in segmental epidural analgesia.

Sensory changes and lower limb reflexes were observed in 35 patients receiving segmental epidural analgesia in the mid-thoracic region. Sensory blockade was confined to the thoracic and upper lumbar segments. The lower limb reflexes changed to an "upper motor neurone' pattern, with the onset and development of segmental blockade. The sequence of changes observed was hypertonic tendon reflexes, then the onset of ankle clonus, and finally the plantar response changed to a positive Babinski sign. With regression of analgesia the reflexes returned to normal in the reverse order. The significance o…
