Unilateral diaphragmatic paralysis after laparoscopic cholecystectomy.

Laparoscopic cholecystectomy is currently the gold standard treatment for acute biliary tract pathology. Despite its many advantages compared to open surgery, it is not without complications. We present the case of an 82-year-old man who, after a diagnosis of gangrenous cholecystitis, underwent urgent laparoscopic cholecystectomy. During the first 24hours after the surgery, he had an episode of acute respiratory failure, for which he was admitted to the critical care unit. Studies performed later showed paralysis of the right diaphragm that was probably related to the surgery.

Paradoxical abdominal wall movement in bilateral diaphragmatic paralysis.

A 56-year-old man gave a 6-month history of progressive dyspnoea and orthopnoea. During breathing, his abdominal muscles showed paradoxical movement and he rapidly and reproducibly developed difficulty breathing when lying supine. The most likely explanation was severe weakness or paralysis of both hemidiaphragms. This was confirmed with electrophysiology and ultrasonography. Extensive investigation identified no underlying cause, suggesting this is a case of bilateral isolated phrenic neuropathy. We present a video showing how easily bilateral diaphragmatic palsy can be detected clinically th

[Patients with high spinal cord injuries: evaluation of diaphragmatic function, indication of electrophrenic ventilation].

Electrophrenic Respiration (REP) is a technic specially reserved to the patients with a first motoneuron disease but having moreover normal phrenic nerve, diaphragm and lungs. Electrical pulse trains are applied to the phrenic nerve by a surgically implanted electrode. They produce rhythmic inspiratory diaphragm contractions; expiratory time is passive. The energy is delivered by a transcutaneous radiofrequency external pacer. Before to decide to implant a REP device, it should be wait for a sufficient time to avoid either a possible return to a spontaneous breathing or, on the contrary, a per

Endoscopic percutaneous suture lateralization with syringe needles for neonatal bilateral vocal cord paralysis.

OBJECTIVE: To explore the novel technique of percutaneous endoscopic suture lateralization for bilateral vocal cord paralysis (BVCP) in neonates from Shenzhen, China, and to evaluate the safety and efficacy of the operation. METHODS: In this retrospective case series, we present four neonates with BVCP diagnosed within 3 days after birth from Shenzhen Children's Hospital. All had stridor, respiratory distress and hypoxemia requiring respiratory support at diagnosis. Endoscopic vocal fold lateralization was performed under general anesthesia using 3.0 mm endotracheal intubation through the imp

Anesthetic Management of a Patient With Spinocerebellar Ataxia Type 1.

Spinocerebellar ataxia type 1 (SCA1) is one of the autosomal dominant spinocerebellar degeneration (SCD) diseases characterized by progressive cerebellar ataxia, muscle atrophy, and peripheral neuropathy. We report the management of a 43-year-old man with SCA1 who underwent general anesthesia for open reduction and internal fixation of a mandibular fracture. Although anesthesia-induced vocal cord paralysis has been reported in patients with SCD, nasotracheal intubation was performed uneventfully with video laryngoscope. After taking into consideration the increased risk of postoperative respir

Rare case of a newborn baby with left-sided Erb's palsy and a contralateral/right-sided paralysis of the diaphragm.

Brachial plexus birth injury (BPBI) and phrenic nerve injury can sometimes occur concurrently in neonates following difficult deliveries like breech presentation, shoulder dystocia, forceps or vacuum extraction. Phrenic nerve palsy should be suspected in a newborn with respiratory distress and an elevated hemidiaphragm on the imaging studies in presence of the associated risk factors. The right side is affected more often than the left side and most of it is associated with BPBI. We present here a rare case of a newborn baby with a left-sided Erb's palsy and a contralateral/right-sided diaphra

[Rupture of larynx or trachea resulting from injuries sustained at birth].

Four newborn boys developed respiratory insufficiency and pneumothorax, pneumomediastinum or subcutaneous emphysema as the result of a laryngeal or tracheal rupture. These ruptures were due to birth injuries after difficult labour resulting from shoulder dystocia or a large lymphangioma and to a birth weight of at least 4500 g. The three children with shoulder dystocia also had a clavicular fracture, a Horner's syndrome, Erb paralysis or phrenic nerve paresis. Treatment consisted of surgical repair followed by a few days' intubation. The children with a shoulder dystocia recovered well, althou

Risk Factors and Clinical Outcomes of Perioperative Complications Following Cervical Spine Surgery.

Study DesignProspective multicenter cohort study.ObjectivesTo determine the incidence and risk factors for postoperative complications following cervical spine surgery and to explore their association with long-term clinical outcomes.MethodsA total of 1482 patients with degenerative cervical disorders who underwent surgery at 10 high-volume institutions in Japan were prospectively enrolled. Perioperative complications were defined as events occurring within 30 days postoperatively. Risk factors were analyzed using univariate and logistic regression analyses. Clinical outcomes were assessed usi

Airway stenosis following immediate bilateral recurrent laryngeal nerve reconstruction: report of two cases successfully treated with laser submucosal intermediate transverse resection of the arytenoid cartilage.

BACKGROUND: While the usefulness of immediate intraoperative reconstruction of the unilateral recurrent laryngeal nerve (RLN) has been reported, there are few reports of bilateral reconstruction. Furthermore, detailed evaluation of laryngeal function before and after surgical treatment for airway compromise caused by bilateral RLN paralysis remains limited. This report aimed to highlight a preventable complication of immediate bilateral RLN reconstruction and a salvage treatment option. CASE DESCRIPTION: We report two women who underwent immediate bilateral RLN reconstruction during surgery f

Rewiring the Lung-CNS Axis After Spinal Cord Injury.

Spinal cord injury (SCI) is a catastrophic disorder of the central nervous system, most commonly resulting from traumatic events such as motor vehicle collisions or falls, but it can also arise from non-traumatic causes including neoplastic, infectious, or degenerative diseases. Respiratory complications are among the most frequent and life-threatening sequelae of SCI. In the acute phase, up to 80% of patients experience respiratory dysfunction, including pneumonia, atelectasis, and respiratory failure. These issues are particularly pronounced in individuals with high cervical injuries, where

[Transient acute respiratory failure and thoracic epidural anesthesia].

A case is reported of acute respiratory failure occurring during upper abdominal surgery in a patient not previously known to have chronic respiratory failure. Preoperatively, this 68 year old patient presented with mild obesity, slight effort dyspnoea and paralysis of the right hemidiaphragm, a sequela of polytrauma she suffered the year before. Respiratory tests were not considered useful with regard to the results of clinical examination. Moreover, she had already several previous general anaesthetics without any problems. A thoracic epidural anaesthesia was performed with a mixture of 150

Affection of Respiratory Muscles in ALS and SMA.

Respiratory dysfunction is a common cause of morbidity and mortality in motor neuron disease (MND). However, classical volitional measures of respiratory function in these patients are impeded by, e.g., bulbar paralysis or progressive disability. Diaphragm ultrasound imaging might be a valuable tool for assessing respiratory impairment, albeit different ultrasound measures have not been systematically investigated in adult MND patients and, in particular, have not yet been comparatively applied in adult patients with amyotrophic lateral sclerosis (ALS) and spinal muscular atrophy (SMA). We hyp

[Emergencies in neuromuscular pathology].

Acute muscle weakness (AMW) is the predominant symptom of neuromuscular emergencies, especially if it affects the respiratory or oropharyngeal musculature . AMW is a multi-etiological syndrome, with different lesion levels in the motor unit. Within the broad group of neuromuscular diseases, those that most frequently provoke AMW and respiratory failure are Guillain-Barré syndrome (GBS) and myasthenia gravis (MG). GBS is the most frequent cause of acute flaccid paralysis; it can cause respiratory failure in a third of cases, making mechanical ventilation necessary. Accurate diagnosis of this sy
