Background: Active abdominal compression-decompression cardiopulmonary resuscitation (AACD-CPR) is potentially more effective for cardiac arrest (CA) with multiple rib fractures. However, its effect on survival rates and neurological outcomes remains unknown. This study aimed to assess if AACD-CPR improves survival rates and neurological outcomes in a rat model of asphyctic CA with multiple rib fractures. Methods: Adult male Sprague-Dawley rats were randomized into three groups-AACD group (n = 15), standard cardiopulmonary resuscitation (STD-CPR) group (n = 15), and sham group (n = 10)-after b…

BACKGROUND: Resuscitative thoracotomy (RT) is a critical intervention for patients in traumatic cardiac arrest or hemorrhagic shock, where survival is highly dependent on the time required to perform the procedure. Despite its urgency, RT is still conducted using traditional thoracic retractors originally designed for scheduled surgeries, which pose challenges in emergency settings. To address these limitations, we developed a novel thoracic retractor optimized for RT and evaluated its performance compared to a conventional model.
METHODS: The novel retractor was designed with an arrow-shaped

Terminating cardiopulmonary resuscitation (ToR) in pediatric emergency department settings requires complicated clinical and ethical decision-making. This review focuses on providing physicians and advanced practice providers with practical guidance for assessing indicators of prognosis and navigating ethical considerations during the resuscitation of pediatric patients. Clinical indicators such as physical exam findings, arrest etiology, cardiopulmonary resuscitation (CPR) duration, and end-tidal carbon dioxide levels serve as benchmarks to guide resuscitation efforts. Ethical principles, inc…

The protocol for the management of hypotension in the setting of septic shock has been long debated. Septic shock is a type of distributive shock where disseminated infection causes widespread vasodilation in the vascular network, leading to inefficient perfusion of organs and ultimately multiorgan failure, which can cause death if not compensated for. In the setting of severe sepsis, acute kidney injury may occur, leading to electrolyte and acid-base abnormalities, which can further contribute to mortality. Traditionally, fluid resuscitation and vasopressor administration have been utilized i…

The initial management of septic shock necessitates adequate fluid resuscitation to restore tissue perfusion. However, excessive or sustained fluid administration may precipitate fluid accumulation syndrome (FAS), significantly exacerbating the risk of organ dysfunction and mortality. Consequently, proactive prevention and management of FAS are paramount for optimizing patient outcomes. With the continuous optimization of fluid resuscitation strategies in the treatment of septic shock, de-resuscitation, as a key stage for preventing or correcting FAS, provides an important opportunity to impro…
