Retroperitoneal fibrosis (RPF) is an uncommon disorder that may cause ureteric obstruction with renal damage. Pergolide, a dopaminergic agonist used in the treatment of Parkinson's disease, has rarely been related to the development of RPF. We report on a 78-year-old woman with Parkinson's disease who presented with hydroureteronephrosis and developed RPF and serosal fibrosis during treatment with pergolide. A 68-year-old male presented with abdominal pain and obstructive jaundice. CT revealed a large mass in the pancreatic head that was initially interpreted as pancreatic carcinoma. Needle biopsy revealed only fibrous tissue with signs of chronic inflammation. Together with typical findings of an idiopathic retroperitoneal fibrosis, the final diagnosis of multifocal idiopathic fibrosclerosis with focal pseudotumorous pancreatic head fibrosis could be made.

We report the incidental finding at surgery for retroperitoneal fibrosis of a carcinoid tumour causing complete right ureteric obstruction. Retroperitoneal fibrosis is an uncommon inflammatory disease that leads to extensive fibrosis throughout the retroperitoneum. A review of the literature of the last 5 years on retroperitoneal fibrosis is given. MRT seems to add to the exact extend of the disease and galliumscintigraphy can give new information on the activity of the process and the usefulness of medical therapy. Microinvasive cervical cancer presented in a woman with retroperitoneal fibrosis in remission following steroid therapy. The cervical lesion was treated surgically with good outcome. Review of the literature documenting this association reveals three other cases, one following and two preceding the diagnosis of retroperitoneal fibrosis. The case reports are reviewed and potential difficulties in the management of these patients are discussed.

Twelve patients with retroperitoneal fibrosis (RPF) over a 10-yr period (1980-1990) are reviewed. The clinical manifestations, radiographic findings as well as treatment and follow-up are discussed and a review of the literature is presented. A patient with mid-gut carcinoid tumor and the unusual complication of retroperitoneal fibrosis was also found to have a prolactinoma. A 68 years-old patient was followed-up during five years for idiopathic retroperitoneal fibrosis. The diagnosis was based on repeated examinations with two laparotomies as well as numerous biopsies. At autopsy a primary retroperitoneal sarcoma was found. This case points out the difficulty to differentiate idiopathic from malignant retroperitoneal fibrosis. Only one similar case was published in the literature.

The authors report a case of combined mesenteric and retroperitoneal fibrosis resulting in vascular and digestive compressions. Diagnosis was established by laparoscopy and biopsy. No etiologic factors were found. Such an association is exceptional and suggests that the disease is systemic. The possibility of fibrosis arising far from the mesenterium (mediastinum, extrahepatic biliary tract, thyroid, orbit) has led some authors to suggest that this condition be termed Idiopathic Systemic Fibrosis. An immunologic origin has been speculated in the literature.

The first case of a simultaneous retrocaval ureter and retroperitoneal fibrosis of the right ureter is presented. The diagnosis was made roentgenographically on the basis of the S-shaped course of the proximal right ureter and medial deviation of the tapered and obstructed mid ureteral segment. This was confirmed by exploration of the right ureter and retroperitoneal biopsy. The surgical treatment of this unusual entity is described.

Four patients with idiopathic retroperitoneal fibrosis were found to have characteristic obstruction and anterior displacement of the lumbar vena cava. Varying degrees of venous collateral circulation were present, depending on the degree of vena caval compression. All 4 patients underwent ureterolysis to relieve the hydronephrosis and to confirm diagnosis by biopsy. Two patients were treated with corticosteroids postoperatively. The valence of radiological methods of examination for the diagnosis and observation of retroperitoneal fibrosis is presented with reference to 2 case reports and an analysis of 48 of our own patients. The i.v. urogram with breathing and profile exposures is the most important method of examination. Retrograde pyelography in the AP and lateral positions is often indispensable for preoperative diagnosis. Cavography and arteriogram are necessary to determine the extent of the vascular involvement and the extent of the fibrosis caudally.

Three West-Indian black siblings with the sickle cell trait developed retroperitoneal fibrosis, a previously unreported association.
