Subclinical coeliac disease.

The prevalence of coeliac disease (CD) was investigated in an epidemiological study of school children from a district of Rome. Mass screening of IgA-AGA levels (anti-gliadin antibody) were determined in blood samples from the study group and the results were positive in 19 children. Eighteen of these agreed to undergo endoscopic biopsy. Histological study of the duodenal specimens revealed villous atrophy in twelve children none of whom had the characteristic symptoms of CD. The most frequent complaints were abdominal pain, aphtous stomatitis, and atopic dermatitis. The high prevalence of CD …

Impact of the COVID-19 pandemic on the Emergency Department of a tertiary children's hospital.

BACKGROUND: Italy was the first country in Europe affected by COVID-19: the emergency started on February 20, 2020, culminating with national lockdown on March 11, which terminated on May 4, 2020. We describe how the pandemic affected Emergency Department (ED) accesses in a tertiary children's hospital, composed by two different pediatric centers, one located in Rome's city center and the second, Palidoro (regional COVID-19 center), in its surrounding metropolitan area, both in the Lazio region, analyzing the profile of admitted patients during the pandemic period in terms of their general cha…

Gastroesophageal Reflux Disease and Reflux-like Phenotypes: Using Lyon Consensus 2.0 to Guide Clinical Approach.

Gastroesophageal reflux disease (GERD) is common worldwide, but troublesome symptoms alone are not sufficient for a conclusive diagnosis. The Montreal Consensus introduced a symptom-based definition of GERD and defined mucosal injury phenotypes of the disease. The Lyon Consensus adapted mucosal injury findings as well as defined abnormal reflux burden criteria for the diagnosis of conclusive GERD. The diagnosis of esophageal disorders of gut-brain interaction (E-DGBI) using Rome V criteria requires exclusion of GERD, which is also described in the Lyon Consensus 2.0 criteria. Collectively util…

Epigastric pain syndrome overlaps with gastroesophageal reflux disease, while postprandial distress syndrome with esophageal disorders of gut-brain interaction.

BACKGROUND: Functional dyspepsia (FD) and gastroesophageal reflux disease (GERD) frequently coexist. Studies suggest that patients with the Epigastric Pain Syndrome (EPS) subtype of FD have higher esophageal acid exposure and respond better to proton pump inhibitors (PPIs) than those with Postprandial Distress Syndrome (PDS). AIM: To evaluate multichannel intraluminal impedance-pH (MII-pH) monitoring findings in FD patients and assess potential overlap with GERD and esophageal disorders of gut-brain interaction (DGBI). METHODS: Patients fulfilling Rome IV criteria for EPS and PDS, both with …

Exploring new subgroups for irritable bowel syndrome using a machine learning algorithm.

Irritable bowel syndrome (IBS) is a complicated gut-brain axis disorder that has typically been classified into subgroups based on the major abnormal stool consistency and frequency. The presence of components other than lower gastrointestinal (GI) symptoms, such as psychological burden, has also been observed in IBS manifestations. The purpose of this research is to redefine IBS subgroups based on upper GI symptoms and psychological factors in addition to lower GI symptoms using an unsupervised machine learning algorithm. The clustering of 988 individuals who met the Rome III criteria for dia…

Management of functional dyspepsia: state of the art and emerging therapies.

Patients with functional dyspepsia, defined in the 2016 Rome IV criteria as bothersome clinical dyspepsia symptoms, experience markedly reduced quality of life. Several etiologies have been associated with the disorder. In the Rome IV criteria, the brain-gut axis was acknowledged as an important factor in the etiology of functional gastrointestinal (GI) disorders. The distinct subgroups of functional dyspepsia, epigastric pain syndrome (EPS) and postprandial distress syndrome (PDS), are treated differently: acid secretion inhibitors are recommended with patients with EPS, whereas prokinetic dr…

Development of a digestive health status instrument: tests of scaling assumptions, structure and reliability in a primary care population.

BACKGROUND: The absence of valid and reliable health status measures for functional gastrointestinal illness has limited research and patient care for this common group of disorders. A self-report survey has been developed. METHODS: Initial development focused on extensive pre-testing of patients, primary care physicians and gastroenterologists. The disease-specific portion included the Rome criteria for dyspepsia subgroups and the Manning and Rome criteria for irritable bowel syndrome. The Short Form-36 was added. Psychometric analyses included techniques of multitrait scaling, scale interna…

Exploring the Impact of Yoga on Autonomic Nervous System Function, Social Functioning, and Quality of Life in People with IBS.

As per the Rome IV criteria, a disorder of gut-brain interaction such as irritable bowel syndrome (IBS) is characterized by recurrent abdominal pain occurring at least 1 day per week for the previous 3 months and associated changes in bowel movements (defecation, stool frequency, or stool form). These symptoms must have begun at least 6 months prior to diagnosis. IBS affects 4% to 7% of the Indian population, with a higher prevalence observed in women and younger individuals. The present randomized controlled trial investigated the effects of a 3-month yoga intervention on IBS management. The …

The subject is the object: On the emergence of subjective science in thirteenth-century theology.

This article examines a fundamental transformation in thirteenth-century theory of science by tracing the shifting role of the subiectum scientiae in scholastic theology. While Aristotelian epistemics defined a science through a unitary subject conceived as a logical genus, medieval debates-especially within the Parisian university context-revealed increasing tensions when applying this framework to theology. Focusing on key figures such as Robert Kilwardby, Thomas Aquinas, and Giles of Rome, the article reconstructs an epistemological turn whereby the subject of science was progressively rein…
