Transgastric jejunal intubation via gastrostomy (GJ) can be indicated when enteral nutrition via gastrostomy is not possible. Between 2001 and 2008, the authors prospectively assessed the outcomes in 29 patients (median age, 10 months) after GJ. Indications for jejunal feeding were severe gastro-oesophageal reflux (n=27) and intestinal dysmotility (n=2). The GJ was successfully placed in 27/29 patients. Experimental and clinical data indicate that ventilator strategies with permissive hypercapnia may reduce lung injury by a variety of mechanisms. Seven randomized controlled trials in preterm neonates suggest that permissive hypercapnia started early, before the initiation of mechanical ventilation (in conjunction with continuous positive airway pressure), followed by prolonged permissive hypercapnia if mechanical ventilation is needed is an alternative to early ventilation and surfactant. Permissive hypercapnia may improve pulmonary outcomes and survival.

INTRODUCTION: Despite statin monotherapy, many high-risk patients are not at recommended low-density lipoprotein cholesterol goals. Moreover, these patients are also likely to exhibit an atherogenic dyslipidemia characterized by decreased high-density lipoprotein cholesterol and elevated triglycerides. As a consequence, combination lipid-altering drug therapies are frequently required to improve the lipid profile.
