Nonunion of a carpal scaphoid fracture in a child.

We report a case of a nonunion of a carpal scaphoid in a child treated successfully by cast immobilization. Contrary to the general opinion that nonunion of the scaphoid bone should be treated operatively, even in children, we showed that healing can be obtained by nonoperative treatment methods.

Is arthroscopic-assisted treatment applicable for the treatment of pediatric scaphoid nonunion?

Pediatric scaphoid nonunion is uncommon. Immobilization is initially attempted; however, it causes delays in both intervention and healing. Therefore, surgical intervention has gained broader acceptance and has been implemented in recent years. Among these, arthroscopic management has emerged as a contemporary technique. This study evaluated the clinical outcomes of pediatric scaphoid nonunion treated with arthroscopic-assisted bone grafting and Kirschner (K) wire fixation. Six pediatric patients with scaphoid nonunion treated with arthroscopic bone grafting and K-wire fixation between Novembe

Scaphoid non-union: factors affecting the functional outcome of open reduction and wedge grafting with Herbert screw fixation.

50 patients with scaphoid non-union were treated by open reduction, anterior wedge bone grafting and internal fixation using the Herbert screw. Intra-operative image intensifier control enabled us to insert the screw into the scaphoid accurately. An excellent or good functional outcome was less likely when more than 5 years had elapsed since injury, the non-union was in the proximal third, when sclerosis of the proximal fragment was present, and when reduction of carpal and scaphoid deformity was unsatisfactory. These four factors are believed to be the primary determinants affecting the funct

Operative Management for Pediatric and Adolescent Scaphoid Nonunions: A Meta-analysis.

INTRODUCTION: Scaphoid fractures in the pediatric population represent ∼3% of all hand and carpal fractures. Cast immobilization has been shown to yield excellent results in the acute phase, however some patients develop nonunions. Currently, there is no consensus regarding the best surgical treatment after development of a pediatric/adolescent scaphoid nonunion. METHODS: A comprehensive literature review was performed utilizing Medline, Ovid, and Embase databases to compare surgical techniques for adolescent scaphoid nonunions on the basis of union rates, functional outcomes, and operative c

Scaphoid Reconstruction.

Surgical strategies for scaphoid nonunions become more complex based on time from injury to treatment. The decision-making process, however, can follow a logical sequence. Scaphoid nonunions less than 1 year after trauma and no carpal malalignment can be treated with percutaneous screw fixation under fluoroscopic and/or arthroscopic guidance. Reinterventions or patients with avascular proximal poles that do not need substantial grafts are reliably treated with pedicle vascularized bone grafts. Resection of the distal pole of the scaphoid is a good option in old nonunions or after a failed scap

Computer-assisted 3-dimensional anthropometry of the scaphoid.

Scaphoid fracture fixation using a cannulated headless compression screw and the Matti-Russe procedure for the treatment of scaphoid nonunions are performed routinely. Surgeons performing these procedures need to be familiar with the anatomy of the scaphoid. A literature review reveals relatively few articles on this subject. The goal of this anatomical study was to measure the scaphoid using current technology and to discuss the findings with respect to the current, relevant literature.Computed tomography scans of 30 wrists were performed using a 64-slice SOMATOM Sensation CT system (resoluti

Gadolinium enhanced MR assessment of proximal fragment vascularity in nonunions after scaphoid fracture: does it predict the outcome of reconstructive surgery?

This study investigated whether the outcome of bone graft and internal fixation surgery for nonunion of scaphoid fractures could be predicted by gadolinium-enhanced MR assessments of proximal fragment vascularity. Sixteen established scaphoid fracture nonunions underwent gadolinium-enhanced MR scanning before surgical treatment with bone grafting and internal fixation. No relationship was found between MR enhancement and the outcome of surgery. Union was achieved in eight of the 12 nonunions with more than 50% enhancement, and three of the four with less than 50% enhancement, of the proximal p

Scaphoid fractures and Kienbock's disease of the lunate: MR imaging with histopathologic correlation.

Thirteen patients with scaphoid fractures and four patients with Kienbock's disease of the lunate underwent magnetic resonance imaging (MRI) prior to surgery. A total of 28 specimens had MR-histologic correlation. Biopsy specimens obtained by curettage provided pathologic correlation. MRI proved accurate in prospective evaluation of bone viability for both scaphoid fractures and Kienbock's disease. Normal marrow signal was shown to correlate with the presence of osteoid and osteocytes on light microscopy and a surface layer of fluorescence reflecting tetracycline uptake in viable bone. Decreas

Idiopathic avascular necrosis of the scaphoid (Preiser's disease): a case report and review of the literature.

Idiopathic avascular necrosis of the scaphoid bone, Preiser's disease, was originally described as a deteriorative pathology whereby the osseous structure necroses due to loss of blood supply. It may present with multifactorial etiology, which is still largely not well understood. We describe a case of Preiser's disease in a 70-year-old female, with worsening pain and loss of range of motion in her right wrist over a two-year period. Past medical history was significant for Sjogren's disease, fibromyalgia, and dystonia. Pain began several months following traumatic right dorsal wrist injury. D…

Displaced scaphoid waist fractures: the use of a week 4 CT scan to predict the likelihood of union with nonoperative treatment.

This study assessed whether nonunion of displaced scaphoid waist fractures with nonoperative treatment could be predicted from 4 week CT scans. Thirty-one patients with unilateral displaced scaphoid waist fractures and adequate follow-up were included. CT scans in the longitudinal axis of the scaphoid with sagittal and coronal slices were done 4 weeks after the index injury. The effects of fracture gap, sclerosis and bone resorption on union were assessed. Fracture union was observed in all 13 displaced fractures with a < 2 mm gap, four of the seven with a gap of 2-3 mm and only four of the 11

Reduction of the scaphoid fracture with DISI alignment.

Eight cases of symptomatic malunion and nonunion of scaphoid fractures with dorsiflexed intercalated segment instability (DISI) alignment were treated by open reduction, anterior wedge-shaped bone grafting, and internal fixation using Herbert's screw. In four malunited fractures, osteotomy at the fracture line was done before the reduction. The DISI alignment was reduced by insertion of the Kirschner wire into the lunate and flexing it anteriorly. After the bone grafting to the anterior bony defect of the scaphoid, which was created by the Kirschner wire reduction, internal fixation was accomp

Scaphoid fractures in children.

Fracture of the carpal scaphoid before skeletal maturity represents 0.34% of all fractures in children and 0.45% of children's upper limb fractures. Seventy-seven children, with a mean age of 11 years (range 8-14 years), suffered a scaphoid fracture in Nottingham, England, between 1977 and 1984 inclusive. Sixty-four of them have been personally reviewed. The mechanism of injury was usually a fall onto the outstretched hand and in the majority was an undisplaced fracture of the distal third of the bone. Immobilization in a scaphoid type of forearm-hand cast led to union in virtually all instanc

[Use of a cannulated 3.0 mm AO screw with an intraosseous support washer in osteosynthesis of the scaphoid: results and analysis of problems in 28 cases].

The cannulated 3.0 mm AO/ASIF screw with threaded washer is a new implant for scaphoid osteosynthesis. After insertion into the distal scaphoid pole, the washer serves as an intraosseous support for the head of the cannulated lag screw. From June 1997 to March 1998 the new implant was used in 28 male patients between 14 and 50 years of age. In fourteen patients, acute scaphoid fractures were operated on, including five transscaphoid perilunate fracture dislocations (Herbert type B2 and B4, respectively). Fourteen patients had scaphoid pseudarthroses, among those two re-pseudarthroses after pri

The Use of the Proximal Hamate as an Autograft for Proximal Pole Scaphoid Fractures: Clinical Outcomes and Biomechanical Implications.

Scaphoid proximal pole fractures remain a surgical challenge because of high propensity for nonunion, osteonecrosis, and ultimately carpal collapse. Options for management of nonsalvageable proximal pole fractures include non-vascularized bone grafts, vascularized pedicled bone grafts, free vascularized bone flaps, and rib cartilage grafts. The proximal pole of the hamate can also serve as a replacement arthroplasty in the setting of proximal pole scaphoid nonunions with collapse, bone loss, and/or osteonecrosis. This novel graft addresses shortcomings of other graft choices by providing a loc

Scaphoid metastasis as the first sign of occult gastroesophageal junction cancer: A case report.

BACKGROUND: Metastatic tumors of the hand are very rare. They are usually late manifestations of massive advanced malignancies. However, a small portion of acrometastases are indications of occult primary cancer. Here, we report an extremely rare case in which a scaphoid bone lesion was the initial manifestation and was found to be a metastasis from gastroesophageal junction (GEJ) cancer. CASE SUMMARY: A 57-year-old male patient presented with ongoing left wrist pain and swelling after trauma. He was initially misdiagnosed with infection of the scaphoid bone and treated with antibiotics and a…

Treatment of scaphoid fractures with carpal instability.

Since 1981, I treated 81 scaphoid fractures: 14 wrists (group A) with acute unstable fracture, 37 (B) with ununited fracture accompanied by dorsiflexed intercalated segment instability (DISI), and 30 (C) with ununited fracture without DISI. The methods and results were evaluated retrospectively. Bone union occurred in 100% of group A wrists, 92% of group B, and 90% of group C. By a modification of Cooney's clinical scoring chart, mean scores were 91 in group A, 84 in group B, and 87 in group C. In 43% of group A, a scapholunate gap remained after surgery. Of 21 wrists in group B treated by bon

Vascularised bone graft applications in upper extremity problems.

Between 1990 and 1996, 16 cases of bone defects were treated by vascularised bone grafting by the authors. Free vascularised fibula was used in 10 cases and one free iliac crest graft was used for upper extremity bone defects. Four vascular pedicled first metacarpal bone and one radial styloid bone were used for scaphoid nonunion. Average follow-up was 26 months (6-78 months) and success rate was 94%. We recommend vascularised bone grafts in the upper extremity when there is risk of infection; the defect is greater than five centimeters when the forearm rotation is unlimited. The avascularity

Management of trans-scaphoid perilunate dislocations. Herbert screw fixation, ligamentous repair and early wrist mobilization.

A retrospective review of 28 patients with 29 trans-scaphoid perilunate dislocations who underwent open reduction and Herbert screw fixation is presented. The majority of the patients had satisfactory results at 24 months of follow-up. A significantly better range of wrist motion was obtained in postoperative patients treated with cast immobilization for 4 weeks compared with those treated for longer than 5 weeks. The scaphoid fractures united well, with proper alignment of the carpal bones, regardless of the length of cast immobilization. We recommend open reduction, internal scaphoid fixatio
