Pulmonary nodules and nodular scleritis in a teenager with superficial granulomatous pyoderma gangrenosum.

Superficial granulomatous pyoderma gangrenosum, a rare variant of pyoderma gangrenosum, has been considered to be the most benign form of the disease. We present the case of a 15-year-old boy with pulmonary involvement and nodular scleritis associated with this unusual type of pyoderma gangrenosum and discuss its differential diagnosis.

[Clinical characteristics of 4 cases of scleritis associated with systemic lupus erythematosus].

Episcleritis and scleritis are relatively rare ocular diseases, which are commonly associated with rheumatic diseases including systemic lupus erythematosus (SLE). To investigate clinical and laboratory features of SLE-associated episcleritis and scleritis, we now report 4 cases of inpatients who were diagnosed with episcleritis or scleritis secondary to SLE from September 2005 to July 2016 in the Department of Rheumatology and Immunology in Peking University People's Hospital. Demographic, clinical and laboratory characteristics were summarized together with the treatment regimen and the prog…

[Anterior uveitis in the absence of scleritis in a patient with rheumatoid arthritis: case report].

Rheumatoid arthritis is the most common collagenosis and affects almost 0.6% of Brazilian population. It is an important cause of articular deformities. The main ocular manifestation of rheumatoid arthritis is dry eyes (secondary Sjögren's syndrome), followed by scleritis, peripheral ulcerative keratitis and uveitis. The aim of this paper is to present a case of anterior uveitis in the absence of scleritis in a patient with rheumatoid arthritis, a very rare presentation in this type of patient. Female patient, 55 years old, with rheumatoid arthritis, presenting suddenly ocular pain and low vis…

Excision of granulation tissue and free conjunctival autograft in the management of necrotizing scleritis.

PURPOSE: To describe a novel use of conjunctival autograft in the management of surgically induced scleral necrosis that occurred after squint surgery. METHODS: Interventional case report. RESULTS: A 22-year-old man underwent bilateral adjustable squint surgery. Two weeks postoperatively, he developed conjunctival retraction and an area of scleral necrosis/melting with 80% thinning. The surrounding conjunctiva was markedly inflamed with raised edges. Screening for autoimmune and vasculitic conditions did not provide any positive results. The conjunctiva surrounding the area of scleral melt w

Trabeculectomy with mitomycin C in refractory glaucoma associated with nonnecrotizing anterior scleritis.

We report a case of refractory secondary-glaucoma associated with scleritis managed successfully with Mitomycin-C augmented trabeculectomies and preoperative transscleral cyclodiode laser(TSCL). Intraocular pressures (IOPs) were uncontrolled on maximal medical therapy (70mmHgOU).Primary emergency TSCL was performed using reduced parameters whilst optimal immunosuppression was achieved. Uncomplicated elective secondary MMC-trabeculectomies (MMC 0.2mg/ml) were later performed. Over four years postoperative follow-up IOPs have remained within normal range without IOP-lowering medication or bleb m…

Scleritis associated with rheumatoid arthritis and with other systemic immune-mediated diseases.

PURPOSE: Rheumatoid arthritis is the most common systemic immune-mediated condition associated with scleritis. The purpose of this study is to determine whether or not scleritis is more severe in patients with rheumatoid arthritis than in those without associated disease ("idiopathic scleritis"), or than in patients with other systemic immune-mediated diseases. METHODS: Patient characteristics, type of scleritis, and ocular complications of 32 patients with scleritis associated with rheumatoid arthritis were compared with those of 74 patients with idiopathic scleritis and with those of 50 pat…

Cyclosporine A for the treatment of necrotizing scleritis and corneal melting in patients with rheumatoid arthritis.

Two of the ocular complications of rheumatoid arthritis (RA), necrotizing scleritis and keratolysis, have been associated with poor response to standard ocular therapy, and thus poor ocular outcome. We describe 6 patients with active ocular disease and active RA who failed to respond to disease modifying agents or immunosuppressive therapy and whose ocular and arthritic disease subsequently responded to cyclosporine A. Drug related complications were consistent with those reported for patients receiving similar doses for RA.

Nodular scleritis, episcleritis and anterior uveitis as ocular complications of Crohn's disease.

Ocular complications of Crohn's disease (granulomatous enterocolitis) are reported to be as high as 13 percent. However, the association between ocular inflammation and intestinal inflammation is not always recognized by optometrists. The onset of ocular symptoms can often be a precursor to exacerbation of bowel involvement. A case report of episcleritis, anterior nodular scleritis and anterior uveitis secondary to Crohn's disease is discussed.

Microscopical studies of necrotising scleritis. II. Collagen degradation in the scleral stroma.

Electron microscopy of tissue excised during surgery in eight cases of advanced anterior necrotising scleritis showed degradation of collagen in the scleral stroma by both intracellular and extracellular mechanisms. In the first of these mechanisms cells resembling active fibroblasts and macrophages were observed in the process of phagocytosis of collagen fibrils into vacuoles associated with dense cytoplasmic granules. In the second mechanism collagen fibrils in large areas of the scleral stroma appeared swollen and unravelled, or completely solubilised, without close association with stromal…

Adalimumab plus Conventional Therapy versus Conventional Therapy in Refractory Non-Infectious Scleritis.

Long-term systemic glucocorticoids and non-specific immunosuppressants remain the mainstay of treatment for refractory scleritis, and result in serious side-effects and repeated inflammation flares. To assess the efficacy and safety of additional adalimumab, patients diagnosed with refractory non-infectious scleritis were enrolled. They were assigned to the conventional-therapy (CT, using systemic glucocorticoids and other immunosuppressants) group or the adalimumab-plus-conventional-therapy (ACT) group according to the treatments they received. The primary outcome was time to achieve sustaine…

Surgically-induced necrotising scleritis complicated by Nocardia infection following routine cataract surgery.

Necrotising scleritis is a rare, inflammatory condition with potentially devastating visual consequences. It can be associated with inflammatory and infectious causes, and has been linked to several different ocular procedures. We present a difficult case of a patient with surgically-induced necrotising scleritis following routine phacoemulsification cataract surgery, who developed a secondary Nocardia bacterial infection. He required a number of surgical interventions and prolonged antibiotic therapy, suffering recurrent scleral abscesses. A literature review accompanies our case report. Prom…
