CASE: We present the case of a 36-year-old patient with a 4-part proximal humerus fracture with subcoracoid dislocation and devascularization of the humeral head after a fall onto his right shoulder.
CONCLUSION: The patient was successfully treated with open reduction and locking plate fixation to demonstrate that a successful postoperative functional outcome with humeral head survival can be achieved in these complex situations.

BACKGROUND: Clavicle fractures are a common injury in young and active patients. Nonoperative management has historically been preferred; however, recent studies have challenged this paradigm with evidence of lower nonunion rates and improved functional outcomes following surgical treatment. This article describes a limited incision approach to clavicle plating and reports long-term functional and radiographic outcomes for a large cohort of operative midshaft clavicle fractures.
METHODS: A prospectively maintained database was used to identify 1,038 patients who underwent surgical treatment f

Complex injuries and conditions of the shoulder may necessitate wide exposure during the operative procedure. The commonly used approach today is a wide anterior deltopectoral approach with extensive detachment of the deltoid muscle, leading to muscle damage, which may result in difficult postoperative rehabilitation and deteriorated function. We describe a simple combined approach that consists of a deltopectoral approach in conjunction with a subcutaneously performed lateral deltoid split with a single skin incision. We believe this surgical technique provides another useful tool to achieve

Between 1980 and 1997, 27 patients underwent shoulder arthroplasty because of pain or functional impairment due to a proximal humeral nonunion. Twenty-five of the 27 shoulders with a mean 6-year follow-up period (range, 2-15 years) were included in the study. There were 19 women and 6 men, with a mean age of 65 years. The most frequent original fracture types were 2-part surgical neck (64%) and 3-part greater tuberosity fractures (28%). Twenty-one shoulders underwent hemiarthroplasty, and 4 underwent total shoulder arthroplasty. Shoulder arthroplasty resulted in significant pain relief, with m

BACKGROUND: The floating shoulder (ipsilateral fractures of the clavicular shaft and the scapular neck) is thought to be an unstable injury pattern requiring operative stabilization in most instances. This recommendation has been made with little biomechanical data to support it. The purpose of this study was to determine the osseous and ligamentous contributions to the stability of experimentally created scapular neck fractures in a cadaver model.
METHODS: Standardized scapular neck fractures were made in twelve fresh-frozen human cadaveric shoulders. Each specimen was mounted in a specially

PURPOSE: Proximal humerus fractures in adults are approached with a high suspicion for potential associated glenohumeral dislocation. Axillary views of the shoulder can be painful and possibly even lead to dynamic angulation of the proximal humerus fracture. The incidence of associated glenohumeral dislocation in the paediatric population is unclear and it would be useful to determine whether children with proximal humerus fracture require specific axillary view imaging to rule out dislocation.
METHODS: We retrospectively reviewed 220 proximal humerus fractures in 218 total children. Imaging

BACKGROUND: The popularity of bicycle riding for recreation, exercise and transportation has grown enormously in recent years, which has led to an increased incidence of bicycle-related injuries. While these injuries involve mainly the musculoskeletal system, data on shoulder-specific injuries incurred while bike riding are lacking. Classifying these shoulder injuries may provide insight and assistance in the creation and implementation of effective protective gear and measures.
OBJECTIVES: To investigate the types and mechanisms of shoulder injuries among cyclists.
METHODS: This study retro

INTRODUCTION: Sternal fractures are rare with 3-8 % out of the total number of trauma cases mostly caused by direct impact to the anterior chest wall. Most cases described are due to motor vehicle crash either caused by direct impact to the steering wheel or by the seat belt. Fractures mainly occur to the sternal body. Only rarely are cases of manubrium fractures described in literature, for example, in relationship with a direct impact to the shoulder which caused an oblique fracture near to the sternoclavicular joint. Three patients with profoundly dislocated oblique manubrium fracture were

The increasing shift towards patient-centred healthcare has lead to an emergence of patient-reported outcome instruments to quantify functional outcomes in orthopaedic patients. Unfortunately, selecting an instrument for use in a shoulder trauma population is often problematic because most shoulder instruments were initially designed for use with chronic shoulder pathology patients. To ensure an instrument is valid, reliable, and sensitive to clinical changes, it is important to obtain psychometric evidence of its use in the target population. Four commonly used shoulder outcome instruments ar

BACKGROUND: Although various implants exist for the fixation of isolated greater tuberosity fractures, few implants are specifically designed for such fractures. The purpose of this study was to investigate the clinical and radiologic outcomes of open reduction-internal fixation with a low-profile anatomic locking plate for comminuted greater tuberosity fractures of the proximal humerus.
METHODS: From November 2012 to February 2018, 24 patients with displaced and comminuted isolated greater tuberosity fractures were treated with the new low-profile anatomic locking plate. To determine clinica

The primary aims when performing revision arthroplasty of periprosthetic humeral fractures (PHF) are preservation of bone stock, achieving fracture healing and preserving a stable prosthesis with the focus on regaining the preoperative shoulder-arm function. The indications for revision arthroplasty are given in PHF in combination with loosening of the stem. In addition, further factors must be independently clarified in the case of an anatomical arthroplasty. In this context secondary glenoid erosion as well as rotator cuff insufficiency are potential factors for an extended revision procedur

The prevalence of periprosthetic humeral fractures (PHF) is currently low and accounts for 0.6-2.4%. Due to an increase in the rate of primary implantations a quantitative increase of PHF is to be expected in the near future. The majority of PHF occur intraoperatively during implantation with an increased risk for cementless stems and when performing total arthroplasty. Additional risk factors are in particular female gender and the severity of comorbidities. In contrast, postoperative PHF mostly due to low-energy falls, have a prevalence between 0.6% and 0.9% and are significantly less common
