Splenic syndrome at mountain altitudes in sickle cell trait. Its occurrence in nonblack persons.

Six consecutive cases of splenic syndrome at mountain altitudes in persons with sickle cell trait are reported and the literature is reviewed. All six cases occurred in men who experienced the acute onset of severe left-upper-quadrant abdominal pain within 48 hours of arrival in Colorado from lower altitudes. All six patients were phenotypically nonblack. Three patients experienced their symptoms at moderate altitudes of 1,609 to 2,134 m (5,280 to 7,000 ft) above sea level. All recovered with medical management and none required splenectomy, although functional hyposplenia was a sequela in at …

National study on natural history of renal allografts in sickle cell disease or trait.

The natural history of renal allograft in recipients who have sickle cell disease or trait has not been widely studied. This survey attempted to collect all the case histories known at present and to assess the relationship of sickle cell disease or trait to the natural history of renal allograft. A questionnaire was sent to 120 transplant centers in North America. 106 centers responded to the study, compliance rate being 88%. 88 centers reported not having any experience with such a case, whereas 18 centers transplanted 34 kidneys in 30 patients with this disease or trait. 9 of these had sick

Red cell survival after aortic valve replacement with Björk-Shiley prosthesis in presence of sickle-cell trait.

A case of aortic valve replacement with a Björk-Shiley disc prosthesis in a patient with sickle-cell trait is presented. Six months after operation red cell survival was not significantly reduced. On theoretical and clinical grounds it is concluded that the presence of sickle-cell trait does not contribute to late postoperative haemolysis after heart valve replacement.

Hemolysis Following Intraoperative Cell Salvage Replacement in a Scoliosis Patient With Sickle Cell Trait: A Case Report.

UNLABELLED: MINI: Intraoperative cell salvage (ICS) is used to reduce blood loss in scoliosis spinal fusion. We report one case of hemolysis induced acute renal failure following ICS. This is the first reported case of acute renal failure associated with hemolysis following ICS in a scoliosis patient with sickle cell trait. STUDY DESIGN: Case report. OBJECTIVE: To describe a novel presentation of acute renal failure associated with hemolysis after intraoperative cell salvage (ICS) in a neuromuscular scoliosis patient with sickle cell trait (SCT). SUMMARY OF BACKGROUND DATA: Hemolysis-associ…

Blood cell exchange in the treatment of exercise-induced rhabdomyolysis in a patient with sickle cell trait.

We report the use of red blood cell exchange (RBCex) to treat rhabdomyolysis complicated by acute kidney injury in a 16-year-old African-American female with sickle cell trait (SCT). Treatment with aggressive fluid and electrolyte management failed to stem the rise in her creatine kinase, and RBCex was instituted 27 hours after symptom onset. She had a transient improvement in her creatine kinase following this treatment although it failed to resolve a developing lower extremity compartment syndrome, requiring bilateral lower extremity fasciotomies. The mechanism of exercise-induced rhabdomyol…

Epinephrine modulates BCAM/Lu and ICAM-4 expression on the sickle cell trait red blood cell membrane.

Collapse and sudden death in physical training are the most serious complications of sickle cell trait (SCT). There is evidence that erythrocytes in SCT patients aggregate during strenuous exercise, likely because of adhesive interactions with the extracellular matrix (ECM) and endothelial cells, and because of their irregular viscoelastic properties. This results in inflammation, blood flow impairment, and vaso-occlusive events. However, the exact role of stress conditions and how they lead to these complications is virtually unknown. Using single-molecule atomic force microscopy experiments,…

Does higher red blood cell (RBC) lactate transporter activity explain impaired RBC deformability in sickle cell trait?

Lactate and H(+) are suggested to promote the sickling process in red blood cells (RBCs) containing hemoglobin S. We demonstrated greater activity of the RBC monocarboxylate cotransporter MCT-1, lower RBC deformability and impaired hematological indices in sickle cell trait (SCT) carriers compared to control subjects, suggesting an involvement of MCT-1 in hemorheological disturbances in SCT carriers.

Prevalence of sickle cell disease and sickle cell traits among children and adolescents in Nigeria: a systematic review and meta-analysis.

BACKGROUND: Sickle cell disease (SCD) is one of the most common genetic disorders in the world, with Nigeria believed to have the highest burden. Despite the high burden in the country, the true prevalence of SCD and its main driver-the sickle cell trait (SCT)-is unknown, which is important for planning and resource allocation. Hence, we conducted a systematic review and meta-analysis of the prevalence of SCD, SCT, and associated factors in Nigerian children and adolescents. METHODS: Using the PRISMA guidelines [ID: CRD42024556354], we systematically search the following major databases: AJOL…

Is sickle cell trait truly benign? A case-control study of physical and mental health outcomes from the sickle cell belt of Eastern India.

Sickle cell trait (SCT) has long been considered a benign carrier condition; however, emerging evidence suggests that individuals with SCT may experience adverse health outcomes, both physical and mental. These findings challenge the conventional notion of SCT as clinically insignificant. Therefore, the present study aims to capture the physical and mental health of SCT individuals among scheduled caste (SC) and scheduled tribe (ST) populations of Odisha, India. This cross-sectional study was conducted in the hard-to-reach regions, primarily located in the mountainous terraces of Koraput distr…

COVID-19 outcomes among patients with sickle cell disease or sickle cell trait compared to the general population: a systematic review and meta-analyses.

Individuals with sickle cell disease (SCD) and sickle cell trait (SCT) face an increased risk of complications from COVID-19 due to their susceptibility to infections and venous thromboembolism. We selected 28 studies from 3228 references in bibliographic databases to compare COVID-19 outcomes (hospitalization, ICU admission, need for ventilatory support, thromboembolic events, and mortality) between patients with SCD or SCT and control patients. Compared to control patients, the pooled risk of hospitalization was not significantly higher in those with SCT (odds ratio [OR] 1.13, 95% confidence…

Exercise and hypoxia increase sickling in venous blood from an exercising limb in individuals with sickle cell trait.

PURPOSE: The association between sickle cell trait (SCT) and complications related to exercise may be explained if exercise-induced sickling interferes with capillary blood flow and causes tissue ischemia and functional abnormalities. To test this hypothesis, we measured sickling and blood gas values in venous and arterial blood of an exercising limb in subjects with SCT and in controls. SUBJECTS AND METHODS: The study consisted of 15 subjects with hemoglobin AS (SCT group) and 15 subjects with hemoglobin AA (control group). Each performed two maximal arm crank exercise tests, one at 1,270 me…

Variation and impact of polygenic hematologic traits in monogenic sickle cell disease.

Several of the complications observed in sickle cell disease (SCD) are influenced by variation in hematologic traits (HT), such as fetal hemoglobin (HbF) level and neutrophil count. Previous large-scale genome-wide association studies carried out in largely healthy individuals have identified thousands of variants associated with HT, which have then been used to develop multi-ancestry polygenic trait scores (PTS). Here, we tested whether these PTS associate with HT in SCD patients and if they can improve statistical models associated with SCD-related complications. In 2,056 SCD patients, we fo
