Automatic 3D Video Analysis of Upper and Lower Body Movements to Identify Isolated REM Sleep Behavior Disorder: A Pilot Study.

Rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia characterized by dream enactment, abnormal jerks and movements during REM sleep. Isolated RBD (iRBD) is recognized as the early stage of alpha-synucleinopathies, i.e. dementia with Lewy bodies, Parkinson's disease and multiple system atrophy. The certain diagnosis of iRBD requires video-polysomnography, evaluated by experts with time-consuming visual analyses. In this study, we propose automatic analysis of movements detected with 3D contactless video as a promising technology to assist sleep experts in the identification o

Associations between probable REM sleep behavior disorder, olfactory disturbance, and clinical symptoms in Parkinson's disease: A multicenter cross-sectional study.

BACKGROUND: Rapid eye movement sleep behavior disorder (RBD) and olfactory dysfunction are useful for early diagnosis of Parkinson's disease (PD). RBD and severe olfactory dysfunction are also regarded as risk factors for cognitive impairment in PD. This study aimed to assess the associations between RBD, olfactory function, and clinical symptoms in patients with PD. METHODS: The participants were 404 patients with non-demented PD. Probable RBD (pRBD) was determined using the Japanese version of the RBD screening questionnaire (RBDSQ-J) and the RBD Single-Question Screen (RBD1Q). Olfactory fu

Structural and functional magnetic resonance imaging in isolated REM sleep behavior disorder: A systematic review of studies using neuroimaging software.

Isolated rapid eye movement sleep behavior disorder (iRBD) is a harbinger for developing clinical synucleinopathies. Magnetic resonance imaging (MRI) has been suggested as a tool for understanding the brain bases of iRBD and its evolution. This review systematically analyzed original full text articles on structural and functional MRI in patients with video-polysomnography-confirmed iRBD according to systematic procedures suggested by Reviews and Meta-analyses (PRISMA). The literature search was conducted via the PubMed database for articles related to structural and functional MRI in iRBD fro

Cuneus/precuneus as a central hub for brain functional connectivity of mild cognitive impairment in idiopathic REM sleep behavior patients.

PURPOSE: To investigate brain functional correlates of mild cognitive impairment (MCI) in idiopathic REM sleep behavior disorder (iRBD). METHODS: Thirty-nine consecutive iRBD patients, 17 with (RBD-MCI, 73.6±6.5 years), and 22 without (RBD-NC, 69.6±6.1 years) MCI underwent neuropsychological assessment, 18F-FDG-PET, and 123I-FP-CIT-SPECT as a marker of nigro-striatal dopaminergic function. Forty-two healthy subjects (69.6±8.5 years) were used as control for 18F-FDG-PET analysis. Brain metabolism was compared between the three groups by univariate analysis of variance. Risk stratification for REM sleep behavior disorder in patients with Parkinson's disease: A PRISMA-compliant meta-analysis and systematic review.

This study aimed to compare whether the characteristics of Parkinson's disease (PD) patients between probably rapid eye movement sleep behavior disorder (RBD) and confirmed RBD versus non-RBD are differing using a meta-analytic approach. We systematically searched PubMed, EmBase, and the Cochrane library for eligible studies throughout October 2018 in this meta-analysis. The clinical characteristics of PD patients presented with probably RBD, confirmed RBD, or non-RBD were analyzed. The pooled odds ratios and weighted mean differences with corresponding 95 % confidence intervals were calculate

A preliminary investigation of the relationship between rapid eye movement (REM) sleep and fear inhibition processes in the context of circadian misalignment.

OBJECTIVE: Rapid eye movement (REM) sleep is implicated in fear inhibition processes critical for trauma recovery. However, prior experimental models of REM sleep disruption have lacked ecological validity by manipulating sleep without accounting for circadian timing. Circadian misalignment frequently occurs in occupations at high risk of trauma exposure. The present study examined the relationship between REM sleep and fear inhibition in the context of experimentally induced circadian misalignment. METHOD: A total of 42 healthy adults (Mage = 24.89, SD = 5.11) were randomized to one of three

Adjusting the apnea-hypopnea index in children with a low percentage of REM sleep and its potential impact on OSA diagnosis and severity.

STUDY OBJECTIVES: A decrease in rapid eye movement (REM) time during polysomnography in patients with obstructive sleep apnea (OSA) can result in underestimation of apnea-hypopnea index (AHI). We propose adjusting AHI to normalized REM% in individuals with REM% ≤ 15% to avoid underdiagnosis of OSA. METHODS: All children who completed diagnostic polysomnography from 2016-2023 with REM% of ≤ 15% of total sleep time were selected for adjustment. AHI adjustment was done by multiplying AHI by a normalization factor (20%)/REM%. Changes in OSA diagnosis and severity were evaluated before vs after ad

Excessive Daytime Sleepiness and REM Sleep Behavior Disorders in Parkinson's Disease: A Narrative Review on Early Intervention With Implications to Neuroprotection.

Sleep contributes to the consolidation of our memory and facilitates learning. Short term sleep deprivation temporarily reduces mnestic capacity, whereas long lasting sleep deprivation is associated with structural changes in the hippocampus and cortical areas. However, it is unknown whether early intervention and treatment of sleep disorders could have a neuroprotective effect. In neurodegenerative diseases sleep disorders can occur at preclinical stages and are frequently observed in patients with established Parkinson's disease (PD) and other α-synucleinopathies. REM sleep behavior disorder

Increased REM sleep density at admission predicts relapse by three months in primary alcoholics with a lifetime diagnosis of secondary depression.

BACKGROUND: Having previously reported that 3-month relapse was associated with increased admission REM pressure in nondepressed primary alcoholics, we hypothesized that baseline polysomnography would predict outcome in primary alcoholics with a lifetime diagnosis of secondary depression. METHODS: Twenty-one primary alcoholics with secondary depression received polysomnography and the Hamilton Depression Rating Scale during the first and fourth weeks of a 1-month inpatient alcohol treatment program. Exclusion criteria included serious illness, current major alcohol withdrawal symptoms, other

Cognition and Other Non-Motor Symptoms in an At-Risk Cohort for Parkinson's Disease Defined by REM-Sleep Behavior Disorder and Hyposmia.

BACKGROUND: REM-sleep behavior disorder (RBD) and other non-motor symptoms such as hyposmia were proposed by the Movement Disorder Society as research criteria for prodromal Parkinson's disease (P-PD). Global cognitive deficit was later added. OBJECTIVE: To compare non-motor symptoms, focusing on cognition, between a P-PD group and a matched control group. METHODS: In this cross-sectional, case-control study, in a first set of analyses, we performed extensive cognitive testing on people with (n = 76) and a control group without (n = 195) probable RBD and hyposmia. Comparative study of the substantia nigra echogenicity and 123I-Ioflupane SPECT in patients with synucleinopathies with and without REM sleep behavior disorder.

OBJECTIVES: Hyperechogenicity of the substantia nigra (SN) and abnormal dopamine transporter-single-photon emission computed tomography (DAT-SPECT) are biomarkers commonly used in the assessment of prodromal synucleinopathy. Our goals were as follows: (1) to compare echogenicity of SN in idiopathic rapid eye movement (REM) behavior disorder (iRBD), Parkinson's disease (PD) without RBD (PD-noRBD), PD with RBD (PD + RBD), and control subjects; and (2) to examine association between SN degeneration assessed by DAT-SPECT and SN echogenicity. Involvement of peripheral alpha2A adrenoceptor in the acceleration of gastrointestinal transit and abdominal visceral pain induced by intermittent deprivation of REM sleep.

Many studies have associated sleep alterations with the severity of irritable bowel syndrome (IBS) symptoms, but the direct pathophysiological relationship has not been clarified. In addition, alterations in noradrenergic signaling have been implicated in the pathophysiology of IBS, and alpha2-adrenoceptors are potential treatment targets. We have previously shown that acceleration of gastrointestinal transit (GIT) elicited by intermittent rapid eye movement (REM) sleep deprivation stress may fulfill the profile of a model of IBS. Moreover, we showed hypernoradrenergic function in the brain of

Cholinergic mechanisms related to REM sleep. III. Tonic and phasic inhibition of monosynaptic reflexes induced by an anticholinesterase in the decerebrate cat.

The depression of the postural activity induced by intravenous injection of eserine sulphate (0.1 mg/kg), an anticholinesterase, has been studied in precollicular decerebrate cats. The extensor and flexor monosynaptic reflexes elicited by single shock stimulation of the GS, P1-FDHL and DP nerves are tonically depressed during the episodes of postural atonia induced by the anticholinesterase. A further phasic depression of the monosynaptic reflexes occurs during the bursts of rapid eye movements (REM) typical of these episodes. These changes in spinal reflex activity closely resemble the tonic

Glossopharyngeal airway collapse is the main factor leading REM-related oxygen desaturation in patients with obstructive sleep apnea: a pilot study.

OBJECTIVE: This study aimed to determine the factors underlying rapid eye movement (REM) sleep-related oxygen desaturation (OD) in patients with obstructive sleep apnea (OSA). METHODS: All subjects underwent overnight polysomnography with synchronous genioglossus electromyography using intraoral surface electrodes. For those subjects diagnosed with moderate or severe OSA (AHI > 15 events/h), we respectively calculated the average OD during rapid eye movement (REM-OD) and non-rapid eye movement sleep (NREM-OD). The ratio of REM-OD/NREM-OD ≥ 1.5 was defined as patients with REM-related OD. The

Case Report: non-monotonic longitudinal RSWA with tonic-phasic dissociation across serial attended video-polysomnography in isolated REM sleep behaviour disorder.

BACKGROUND: Quantitative REM sleep without atonia (RSWA) metrics are increasingly used to characterize isolated REM sleep behavior disorder (iRBD) and as candidate biomarkers. Cohort follow-up often reports rising RSWA, but within-individual trajectories, particularly when tonic and phasic components diverge, remain sparsely documented. METHODS: A man in his mid-seventies with iRBD underwent four attended video-polysomnograms over -6.5 years. Submental EMG was quantified using a REM sleep atonia index (RAI) with pre-specified exclusion of REM mini-epochs contaminated by artifact or physiologi

Gut-initiated alpha synuclein fibrils drive parkinsonism phenotypes: temporal mapping of REM sleep behavior disorder-like and other non-motor symptoms.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder marked by both motor and non-motor symptoms. Although non-motor features such as gastrointestinal and sleep disturbances often precede motor impairments and are critical to PD pathogenesis, the mechanisms underlying their onset and progression remain insufficiently characterized. METHODS: To investigate the sequential development of motor and non-motor symptoms in a model of experimental parkinsonism, we injected alpha-synuclein (αSyn) preformed fibrils (PFFs) into the duodenum and antrum of wild-type mice, estab

Impact of chronic REM sleep deprivation on glucose homeostasis, insulin sensitivity, oxidative stress, and histological changes in adult male Wistar rats.

Sleep deprivation (SD) is a potential risk factor for the development of various metabolic disorders, including type 2 diabetes (T2D). In humans, SD has been associated with impaired glucose metabolism. To date, the link between chronic sleep deprivation (CSD) and glucose homeostasis remains unclear. To better understand the underlying mechanisms, we investigated the impact of chronic REM-SD on glucose homeostasis, insulin sensitivity, and physiological functions in Wistar rats, as well as on markers of oxidative stress and histopathological changes observed in organs. Eighteen adult male Wist

Symptomatic treatment of REM sleep behavior disorder (RBD): A consensus from the international RBD study group - Treatment and trials working group.

Rapid-eye-movement sleep behavior disorder (RBD) is a parasomnia causing motor behaviors and vocalizations during sleep, which can lead to injuries in patients and their bed partners. Adult-onset RBD generally precedes a neurodegenerative synucleinopathy, while other cases can be associated with antidepressant use, neurotrauma and narcolepsy. The management of RBD relies on the systematic identification of etiologic and contributing factors, implementation of safety measures, appropriate pharmacotherapy and counseling, which should be patient-centered. In this manuscript, we summarize the evid

Memory deficits link trait-like EEG spectral profiles during REM and slow-wave sleep with shared symptoms of depression and anxiety.

Emerging evidence suggests that trait-level cortical hyperarousal during sleep-indicated by heightened EEG activity in higher frequency bands-disrupts memory processing and contributes to depression and anxiety, particularly during rapid eye movement (REM) and slow wave sleep (SWS). Given that memory deficits are putative risk factors for depression and anxiety, we hypothesized that memory deficits might mediate the association between elevated high-frequency EEG activity and greater symptom severity. A sample of 149 healthy adults (ages 18-59) encoded scenes featuring either negative (e.g., s
