Sleep Related Movement Disorders: What's New and Changing Clinical Practice.

Restless legs syndrome (RLS) is a common sensorimotor disorder, and the most common sleep-related movement disorder with a prevalence of up to 15% in the European and US population. This review addresses key aspects of RLS, focusing on novel data that have or will likely have an impact on clinical practice. These include novel insights into pathophysiology and motor activity during sleep, with a key focus on implications for RLS treatment. Along this line, we discuss the problem of augmentation before introducing new treatment paradigms and insights into new drug targets from genetics. Besides

[Psychiatric aspects of sleep].

Sleep disturbances are frequently the consequence of physiological and psychological problems and may be classified in three groups: diminished total quantity of sleep, loss of the quality and inadequate timing of the sleep process. It is considered that one third of the population has a sleep disturbance some time in the life. Insomnia is the most frequent sleep disturbance but timely and careful diagnostic procedure is fundamental for the specific treatment. Factors connected with the increased prevalence of sleep disturbance are female gender, presence of mental or somatic diseases and adva

Sleep disorders in psychiatry.

Sleep is an active state that is critical for our physical, mental, and emotional well-being. Sleep is also important for optimal cognitive functioning, and sleep disruption results in functional impairment. Insomnia is the most common sleep disorder in psychiatry. At any given time, 50% of adults are affected with 1 or more sleep problems such as difficulty in falling or staying asleep, in staying awake, or in adhering to a consistent sleep/wake schedule. Narcolepsy affects as many individuals as does multiple sclerosis or Parkinson disease. Sleep problems are especially prevalent in schizoph

Effects of Chronic Opioid Use on Sleep and Wake.

Chronic use of opioids negatively affects sleep on 2 levels: sleep architecture and breathing. Patients suffer from a variety of daytime sequelae. There may be a bidirectional relationship between poor sleep quality, sleep-disordered breathing, and daytime function. Opioids are a potential cause of incident depression. The best therapeutic option is withdrawal of opioids, which proves difficult. Positive airway pressure devices are considered first-line treatment for sleep-related breathing disorders. New generation positive pressure servo ventilators are increasingly popular as a treatment op

Actigraphy-based sleep outcomes in substance use disorders: A systematic review and meta-analysis.

Sleep disturbances are prevalent in substance use disorders (SUDs). Objective (e.g., polysomnography) and subjective (e.g., Pittsburgh sleep quality index [PSQI]) assessments are commonly used, with polysomnography enabling sleep architecture analysis and diagnosis of sleep disorders, but its use limited by logistical constraints. Actigraphy offers a feasible alternative for longitudinal and naturalistic assessment. We aimed to synthesize actigraphy-based sleep outcomes in individuals with SUDs and compare them with subjective sleep measures. We conducted a meta-analysis (PROSPERO: CRD42025107

A novel approach to the prevention of postoperative delirium in the elderly after gastrointestinal surgery.

PURPOSE: Postoperative delirium (POD) is known to be one of the most critical complications of major operative procedures in elderly patients. Since disorders of the sleep-wake cycle have been reported to be one of the key factors in POD, we attempted to clarify the effectiveness of improving sleep-wake cycle disorders with medication after surgery to prevent POD, by conducting a prospective randomized study of 42 elderly patients who underwent resection of either gastric or colon cancer through an open laparotomy. METHODS: The delirium-free protocol (DFP) group was given an intramuscular inj

Cannabinoid therapies in the management of sleep disorders: A systematic review of preclinical and clinical studies.

Cannabinoids, including the two main phytocannabinoids Δ9-tetrahydrocannabinol (THC) and cannabidiol (CBD), are being increasingly utilised as pharmacological interventions for sleep disorders. THC and CBD are known to interact with the endocannabinoid and other neurochemical systems to influence anxiety, mood, autonomic function, and circadian sleep/wake cycle. However, their therapeutic efficacy and safety as treatments for sleep disorders are unclear. The current systematic review assessed the available evidence base using PubMed, Scopus, Web of Science, Embase, CINAHL and PsycInfo database

Melanopsin gene variations interact with season to predict sleep onset and chronotype.

The human melanopsin gene has been reported to mediate risk for seasonal affective disorder (SAD), which is hypothesized to be caused by decreased photic input during winter when light levels fall below threshold, resulting in differences in circadian phase and/or sleep. However, it is unclear if melanopsin increases risk of SAD by causing differences in sleep or circadian phase, or if those differences are symptoms of the mood disorder. To determine if melanopsin sequence variations are associated with differences in sleep-wake behavior among those not suffering from a mood disorder, the auth

Persistent Sleepiness After Effective Positive Airway Pressure Therapy Deserves Independent Clinical Evaluation.

Positive airway pressure (PAP) therapy often improves excessive daytime sleepiness in obstructive sleep apnea (OSA), yet effective control of sleep-disordered breathing does not invariably restore normal daytime alertness. Persistent sleepiness after PAP therapy should therefore be regarded as a clinical observation requiring explanation rather than automatically attributed to treated OSA. The term "residual excessive daytime sleepiness" may become misleading when it is interpreted as an etiologic conclusion rather than a descriptive label. Evaluation should distinguish PAP adherence from trea

Pharmacological and non-pharmacological management of sleep disturbances in Parkinson's disease: if when and how.

INTRODUCTION: Sleep dysfunction occurs in various forms and is a bothersome and intrusive non-motor symptom of Parkinson's disease (PD). Frequently undiagnosed, their poor management can have a great impact on the quality of life of PD patients and their caregivers. AREAS COVERED: This article covers the safety and efficacy of pharmacological strategies for the management of the most frequent sleep disturbances in Parkinson's disease. Non-pharmacological aspects are also discussed, but these are not the main focus. Literature searches using electronic databases (Medline, Cochrane Library) and

Daily stress interacts with trait dissociation to predict sleep-related experiences in young adults.

Building on the previously documented effects of stress and dissociation on sleep and dreaming, we examined their interactive role in general sleep-related experiences (GSEs; e.g., nightmares, falling dreams, hypnagogic hallucinations; see Watson, 2001). Stress, sleep quality, and GSEs were assessed daily for 14 days among young adults. Baseline assessment included life stress, sleep quality, psychopathology, dissociation, and related dimensions. Multilevel analyses indicated that daily stress brings about GSEs among highly dissociative young adults. Additionally, baseline trait dissociation p

Sleep-dependent changes in the coupling between heart period and blood pressure in human subjects.

We investigated whether in human subjects, the pattern of coupling between the spontaneous fluctuations of heart period (HP) and those of systolic blood pressure (SBP) differs among wake-sleep states. Polysomnographic recordings and finger blood pressure measurements were performed for 48 h in 15 nonobese adults without sleep-disordered breathing. The cross-correlation function (CCF) between the fluctuations of HP and SBP at frequencies <0.15 Hz was computed during quiet wakefulness (QW), light (stages 1 and 2) and deep (stages 3 and 4) nonrapid-eye-movement sleep (NREMS), and rapid-eye-moveme

Sleep-wake disturbances in Parkinson's disease: current evidence regarding diagnostic and therapeutic decisions.

Sleep disorders have been frequently reported in patients with Parkinson's disease (PD). However, there is insufficient evidence to guide precise recommendations on some diagnostic and treatment strategies. Here, we review clinical studies dealing with sleep abnormalities in PD and present clinical recommendations. Previous studies describing insomnia, excessive daytime sleepiness, narcolepsy-like episodes, circadian changes, sleep-disordered breathing, rapid eye movement sleep behavior disorder, vivid dreams and restless legs syndrome are evaluated. Longitudinal studies associating sleep diso

Sleep Disorders in Adults with Prader-Willi Syndrome: Review of the Literature and Clinical Recommendations Based on the Experience of the French Reference Centre.

Prader-Willi syndrome (PWS) is a rare, genetic, multisymptomatic, neurodevelopmental disease commonly associated with sleep alterations, including sleep-disordered breathing and central disorders of hypersomnolence. Excessive daytime sleepiness represents the main manifestation that should be addressed by eliciting the detrimental effects on quality of life and neurocognitive function from the patients' caregivers. Patients with PWS have impaired ventilatory control and altered pulmonary mechanics caused by hypotonia, respiratory muscle weakness, scoliosis and obesity. Consequently, respirator

The interfaces between vitamin D, sleep and pain.

The role of vitamin D in osteomineral metabolism is well known. Several studies have suggested its action on different biological mechanisms, such as nociceptive sensitivity and sleep-wake cycle modulation. Sleep is an important biological process regulated by different regions of the central nervous system, mainly the hypothalamus, in combination with several neurotransmitters. Pain, which can be classified as nociceptive, neuropathic and psychological, is regulated by both the central and peripheral nervous systems. In the peripheral nervous system, the immune system participates in the infl

Factor Structure of the Sleep Disturbance Scale for Children in a Cohort of Youth With Autism.

Despite the substantial evidence base for poor sleep in autism, sleep problems are frequently under-identified and inefficiently managed. This is, in part, due to the fact that available tools do not comprehensively capture the specific sleep phenotypes in autism. The Sleep Disturbance Scale for Children (SDSC) is one of the highest ranked pediatric sleep assessment tools, yet it is scarcely used in autism research. Consequently, the utility of the SDSC for characterizing sleep disturbances in autism remains unknown. This study explored the factor structure of the SDSC in a sample of 513 autis

Psychocutaneous disease: Clinical perspectives.

Psychocutaneous disease, defined in this review as primary psychiatric disease with skin manifestations, is commonly encountered in dermatology. Dermatologists can play an important role in the management of psychocutaneous disease because patients visit dermatology for treatment of their skin problems but often refuse psychiatric intervention. This review describes common psychocutaneous syndromes, including delusional, factitious, obsessive-compulsive and related, and eating disorders, as well as psychogenic pruritus, cutaneous sensory (pain) syndromes, posttraumatic stress disorder, and sle
