An unusual presentation of sporotrichosis.

Sporotrichosis can mimic a few diseases. Sporotrichosis presenting as a hypoaesthetic plaque or plaque suggestive of leprosy has not been mentioned in literature. Herein we present a case of a 26 years old man who presented with hypoesthetic plaque on left ankle and was clinically diagnosed to have borderline tuberculoid leprosy who later developed few nodules in linear fashion on left leg, which ruptured to form non healing ulcers. Culture from ulcer grew Sporothrix schenckii. Patient was successfully treated with oral potassium iodide.

Sporotrichosis of the hand: an urban experience.

Sporotrichosis is uncommon in Canadian urban centers. Lymphocutaneous and fixed cutaneous are the most common cutaneous forms of the disease, typically seen in the upper extremity in adult patients. History usually reveals a puncture injury contaminated with soil. Lesions are refractory to commonly used antibiotics. Cultures of biopsy specimens reliably grow the fungus Sporothrix schenkii. First line treatment is oral potassium iodide. We have reviewed a series of 7 cases from various Toronto teaching hospitals and compared them to the world literature.

Therapeutic failure with itraconazole in sporotrichosis due to bariatric surgery.

Sporotrichosis is a deep mycosis of subacute or chronic evolution, caused by the dimorphic fungus of the genus Sporothrix. The treatment is carried out with antifungal orally or intravenously. Therapeutic success can be affected by several factors, such as altered gastrointestinal physiology by surgery. More and more patients are submitted to bariatric surgeries and the literature for the alterations of the absorption of medications in this context is very scarce. We intend to contribute to a better understanding with this case report of cutaneous-lymphatic sporotrichosis in a patient after ba…

Disseminated cutaneous sporotrichosis: unusual presentation in an alcoholic patient.

Sporotrichosis is an implantation mycosis highly prevalent in Brazil, associated with soil activities and contact with infected animals. It has numerous clinical manifestations and its disseminated cutaneous form is uncommon, found in approximately 4% of cases and usually related to immunosuppressive conditions. We report an alcoholic 56-year-old male patient with no other comorbidities, presenting with multiple cutaneous nodules and ulcers. The patient was diagnosed with disseminated cutaneous sporotrichosis based on the isolation and identification of Sporothrix spp. The patient was initiall…

[Disseminated cutaneous sporotrichosis with joint involvement in a woman with type 2 diabetes].

Disseminated sporotrichosis is a rare presentation of this mycosis. Although it has been described in immunocompetent individuals, there is often T cell-mediated immune compromise. We report the case of a woman with uncontrolled Type 2 diabetes who developed disseminated cutaneous sporotrichosis. The course of the disease presented several complications including hepatitis secondary to itraconazole and knee arthritis with culture positive for Sporothrix schenckii during treatment with saturated solution of potassium iodide. The case discussion includes aspects of the pathogenesis of disseminat…

Molluscum-like lesions in a patient with sporotrichosis.

Sporotrichosis is a subcutaneous fungal infection caused by Sporothrix schenckii and acquired by direct inoculation. Although the majority of cases consist of the classic lymphocutaneous presentation, the frequency of atypical and severe clinical forms of the disease has increased progressively. Systemic and disseminated cutaneous sporotrichosis constitute rare variants and such cases are generally associated with cellular immunodeficiency or debilitated states. The present paper describes the first published case of molluscum-like lesions in disseminated mucocutaneous sporotrichosis. Direct m…

Sporotrichosis in north-east India.

Reports on the occurrence of sporotrichosis in some parts of north-east India are scarce. We report here five cases of lymphocutaneous sporotrichosis from north-east India, including one from Sikkim State and four from distant places in the north-eastern part of West Bengal. All patients were full or part-time farmers or gardeners. Diagnosis of sporotrichosis was confirmed by isolation of Sporothrix schenckii in culture and in vitro conversion of the isolates to yeast form. The cases were treated with 50% solution of potassium iodide.

Disseminated Sporothix schenckii in a patient with AIDS.

Sporothrix schenckii is a widespread dimorphic fungus which can cause cutaneous infection following local implantation. Disseminated sporotrichosis may occur in immunodeficient individuals but meningitis remains a rare complication. Diagnosis is usually difficult, requiring isolation of the organism from the CSF or skin so appropriate treatment can be promptly initiated. We present the first case of S. schenckii meningitis reported in the UK in a patient with AIDS. He presented with insidious features of meningoencephalitis, hydrocephalus and multiple cutaneous lesions and failed to respond to…

LC-QTOF-MS Chemical Characterization, Cytotoxicity, and Antimicrobial Activity of Blue Geopropolis: An Emerging Type of Brazilian Propolis.

Background/Objectives: This study characterized blue geopropolis from Melipona quadrifasciata quadrifasciata (southern Brazil) by comparing … st Candida albicans or Candida parapsilosis (MFC > 1000 µg/mL) but showed variable activity against Sporothrix spp. The colorimetric assay indicated that phenolic-iron complexation may contribute to the blue color. Conclusions: Despite its distinct entomological origin, blue geopropolis shared chemical traits with green and red propolis while demonstrating s …

Insights From a Single Case of Sporotrichosis: T Cell Receptor Repertoire Analysis Highlighting Antigen Specificity.

Sporotrichosis is a subacute subcutaneous fungal infection caused primarily by Sporothrix globosa, typically following traumatic inoculation. The current diagnostic gold standard is fungal isolation and culture from the lesion, which requires significant time. The sporotrichin skin test, an intradermal reaction using crude polysaccharide antigens extracted from S. globosa, has been used in some countries as a relatively rapid diagnostic tool. While it demonstrates high sensitivity, evidence regarding its antigen specificity has not been sufficiently explored. Here, we report a T cell receptor …

Lymphocutaneous Sporotrichosis of Face with Verrucous Lesions: A Case Report.

Sporotrichosis is a cutaneous mycosis caused by a dimorphic fungus, Sporothrix schenckii species complex clinically presenting as lymphocutaneous, fixed, or disseminated forms. A typical lesion is an erythematous papule, noduloulcerative lesion usually occurring at the site of penetrating trauma, mostly on the extremities. Verrucous lesion is an unusual presentation of sporotrichosis which can mimic the verrucous lesions seen in chromoblastomycosis, tuberculosis verruca cutis/lupus vulgaris (TBVC/LV), cutaneous leishmaniasis, and blastomycosis leading to diagnostic dilemma. Herein, we describe…

Sporotrichosis in an unusual location--Case report.

Sporotrichosis is the most common subcutaneous mycosis. It is caused by the dimorphic fungus Sporothrix schenckii, and the infection is usually acquired by traumatic inoculation. We describe a case of sporotrichosis in an uncommon location with an unusual mode of transmission. A 49-year-old female patient who lived in an urban area of Rio de Janeiro presented with involvement of the left ear. No history of contact with soil, plants or animals was elicited. The suspected source of infection was a pair of handmade wooden earrings. The delay in the diagnosis and treatment resulted in higher morbi…

[Disseminated sporotrichosis - clinical case and discussion].

The authors report a case of a patient with a prior history of alcohol abuse who developed nodules and ulcerated skin lesions on his trunk and lower extremities along the line of the lymphatic draining area. The patient's X-ray showed reticular nodular intersticial infiltrates at the lung bases. There was a positive culture of supraclavicular lymph node for Sporothrix schenckii. After specific treatment using mainly potassium iodide, there was regression of cutaneous lesions and lung infiltrates. The authors present a discussion on the deseases' forms of presentation, highlighting the lung inv…

Lymphocutaneous and nasal sporotrichosis in a dog from southern Italy: case report.

Sporotrichosis is a chronic, granulomatous and usually lymphocutaneous infection of humans and animals caused by the dimorphic fungus, Sporothrix schenckii. This study reports a case of lymphocutaneous and nasal sporotrichosis in a hunting dog with a three month history of non-healing skin lesions. Cytological examination of nasal discharge and of the material collected from ulcerated skin surfaces showed a few cigar-shaped organisms within macrophages. Fungal cultures of nasal and ulcerated skin swabs yielded colonies of S. schenckii. The dog received oral itraconazole but died of unrelated c…

Cutaneous sporotrichosis in Bangalore, southern India.

BACKGROUND: Sporotrichosis is a granulomatous infection caused by Sporothrix schenkii. Although world-wide in distribution, only four cases have been reported from southern India to date. METHODS: The medical records of all cases seen at the Department of Dermatology at St John's Medical College Hospital, Bangalore, over the last 15 years were reviewed. RESULTS: A total of seven cases of sporotrichosis were seen in this 15-year period. CONCLUSION: Sporotrichosis is considered to occur only sporadically in southern India. Our findings suggest that this may be a result of reporting bias.

[Effect of potassium iodide on the immune response in the sporotrichosis.].

Potassium iodide (KI), the specific treatment for sporotrichosis, apparently does not have a direct action on Sporothrix schenckii. The spontaneous healing and the variability of the clinical presentation in the disease have strengthened the idea that the KI rather interacts with the immune response of the host. The phagocytic process is inefficient in individuals with sporotrichosis in whom the microbicidal mechanism of halogenation fails to control the disease. There is evidence that blocking of free radicals decreases in the presence of KI. Humoral and cellular immunity are present in sporo…

Deep mycotic infections in cats.

A review of deep mycotic infections assessed incidence, signalment, clinical presentation, and outcome in 571 feline cases. Pertinent findings included a predisposition in young, male cats for Sporothrix schenckii and Blastomyces dermatitidis. There was an increased incidence of feline leukemia virus infection in cases with histoplasmosis and of feline panleukopenia virus infection in cases with either aspergillosis or mucoralosis. Few other predisposing conditions were identified. Cryptococcosis, coccidioidomycosis, and sporotrichosis had better prognoses for recovery. Blastomycosis, histopla…
