Prevalence of and risk factors for methicillin-resistant Staphylococcus aureus (MRSA) nasal colonization in HIV-infected ambulatory patients.

BACKGROUND: Estimates of the prevalence of colonization with methicillin-resistant Staphylococcus aureus (MRSA) vary in HIV-infected patients. METHODS: HIV clinic patients were prospectively cultured. Bilateral nasal and axillary swabs were plated on BBL CHROMagar MRSA media. Molecular typing was done by pulse-field gel electrophoresis, and staphylococcal cassette chromosomemec typing was determined. A patient questionnaire was conducted to ascertain potential MRSA risk factors; medical records were reviewed. RESULTS: Fifteen of 146 (10.3%) patients had MRSA nasal colonization; 1 also had ax

Impact of methicillin resistance on the outcome of patients with bacteremia caused by Staphylococcus aureus.

BACKGROUND: Uncertainties remain about the contribution of methicillin resistance to morbidity and mortality associated with bacteremia caused by Staphylococcus aureus. OBJECTIVE: To assess the impact of methicillin resistance on patient outcome after staphylococcal bacteremia. METHODS: We investigated a cohort of 145 patients with methicillin-sensitive S aureus bloodstream infection (MSSA BSI) and 39 patients with methicillin-resistant S aureus bloodstream infection (MRSA BSI) and further performed a pairwise-matched (1:1) case-control study. All patients in the University Hospital of Genev

The Synergistic Effect of Nicotine and Staphylococcus aureus on Peri-Implant Infections.

Smoking is considered a key risk factor for implant survival; however, how it interacts with the pathogens in peri-implant infections is not clear. Here, we identified that nicotine, the key component of cigarette smoking, can interact with Staphylococcus aureus and synergistically induce peri-implant infections in a rat osteolysis model. The nicotine-S. aureus combination group increased the gross bone pathology, osteolysis, periosteal reactions, and bone resorption compared to the nicotine or S. aureus single treated group (p < 0.05). Nicotine did not promote the proliferation of S. aureus b

Staphylococcus aureus Toxins: An Update on Their Pathogenic Properties and Potential Treatments.

Staphylococcus aureus is a clinically important pathogen that causes a wide range of human infections, from minor skin infections to severe tissue infection and sepsis. S. aureus has a high level of antibiotic resistance and is a common cause of infections in hospitals and the community. The rising prevalence of community-acquired methicillin-resistant S. aureus (CA-MRSA), combined with the important severity of S. aureus infections in general, has resulted in the frequent use of anti-staphylococcal antibiotics, leading to increasing resistance rates. Antibiotic-resistant S. aureus continues t

Methicillin-resistant Staphylococcus aureus: from the hospital to the community.

Infections due to community-associated methicillin---resistant Staphylococcus aureus (CA-MRSA) are becoming more prevalent. CA-MRSA infections have unique epidemiologic features and virulence factors. Compared with health care-associated MRSA (HA-MRSA), most CA-MRSA is clonal type USA300 or 400 and has the Staphylococcal cassette chromosome mec type IV, which carries the mecA gene that encodes for resistance to methicillin and other beta-lactam antibiotics but generally not for other antibiotics. CA-MRSA often contains various virulence factors that may result in tissue necrosis. CA-MRSA clini

Multi-resistant Staphylococcus haemolyticus in a neonatal unit in New Delhi.

We describe a cluster of infections in a neonatal nursery due to an infrequently reported staphylococcal species, Staphylococcus haemolyticus. S. haemolyticus resistant to penicillin, methicillin, gentamicin, erythromycin, chloramphenicol and tetracycline (PMGECT) was isolated from a series of infections in neonates (conjunctivitis 6, blood 2, pustules 2) over a period of 3 weeks in a neonatal nursery. Surveillance cultures from 22 neonates, their mothers in an adjacent maternity ward and staff revealed that S. haemolyticus with three resistance patterns (PMGECT, PMG and PME) was circulating i

Group A Streptococcus invasive infections: a review.

The incidence of group A Streptococcus (GAS) invasive infections has been increasing worldwide, and there is no obvious explanation for this phenomenon. In 1993, a working group on severe GAS infections was established to define accurately what constitutes an invasive infection. Three types of infection are particularly feared: necrotizing fasciitis, myositis and a newly defined entity, named streptococcal toxic shock syndrome (STSS) because of a certain analogy with its staphylococcal counterpart. GAS produces many toxins responsible for its clinical manifestations. Some of them, labelled str

Skin and skin structure infections in the patient at risk: carrier state of Staphylococcus aureus.

Staphylococcus aureus is a ubiquitous organism that is normally carried on the skin and body surfaces of man. The nares are sites frequently colonized, and patients and hospital personnel represent the major source of infection. The occurrence of staphylococcal infection depends on the availability of staphylococci and the host resistance to infection. Factors that influence the carrier rate of S. aureus include minimal colonizing dose, effects of antimicrobial therapy, disinfectants in the environment, coincidental respiratory infections, possible effect of immune factors, duration of hospita

Intramammary infections in primigravid heifers near parturition.

Mammary secretion from 32 primigravid heifers was obtained aseptically to determine frequency of bacterial isolation and incidence of intram … aphylococci. Twenty infections were observed during the early postpartum period. Coagulase negative staphylococcal infections decreased markedly. However, the number and type of primary pathogen infections during early lactation were similar to those at parturition.

Methicillin-resistant Staphylococcus aureus: a community health threat.

Methicillin-resistant Staphylococcus aureus (MRSA), one of the most common causes of infections, has been traditionally recognized as a nosocomial pathogen. However, in recent years, its epidemiology has radically changed, being now observed even more frequently in the community, and accounting for > 50% of staphylococcal infections in the US outpatient setting. Community-acquired (CA)-MRSA strains typically cause infections among otherwise healthy individuals, with risk factors differing from those of nosocomial MRSA. The clinical manifestations may range from a furuncle to life-threatening i…

Atypical Staphylococcal Septic Arthritis in a Native Hip: A Case Report and Review.

Septic arthritis is a synovial fluid and joint tissue infection with significant morbidity and mortality risk if not diagnosed and treated promptly. The most common pathogen to cause septic arthritis is Staphylococcus aureus, a Gram-positive bacterium. Although diagnostic criteria are in place to guide the diagnosis of staphylococcal septic arthritis, there is a lack of adequate sensitivity and specificity. Some patients present with atypical findings which make it difficult to diagnose and treat in time. In this paper, we present the case of a patient with an atypical presentation of recalcit

Enterococcal endocarditis in heroin addicts.

Enterococci caused eleven of 20 cases (55%) of narcotic-associated endocarditis in Cleveland over a 54-month period. Acute staphylococcal endocarditis seen concurrently in this addict population displayed tricuspid valve involvement and septic pulmonary emboli, but enterococcal endocarditis was subacute, affected only aortic or mitral valves, and was not associated with septic pulmonary emboli. In contrast to enterococcal endocarditis in a nonaddict population, patients were younger and rarely had antecedent valvular disease or genitourinary abnormality. Staphylococcal tricuspid valve endocard

Coordinated adaptation of Staphylococcus aureus to calprotectin-dependent metal sequestration.

UNLABELLED: The host protein calprotectin inhibits the growth of a variety of bacterial pathogens through metal sequestration in a process known as "nutritional immunity." Staphylococcus aureus growth is inhibited by calprotectin in vitro, and calprotectin is localized in vivo to staphylococcal abscesses during infection. However, the staphylococcal adaptations that provide defense against nutritional immunity and the role of metal-responsive regulators are not fully characterized. In this work, we define the transcriptional response of S. aureus and the role of the metal-responsive regulators

Dermatomyositis following chronic staphylococcal joint sepsis.

A young man is reported with recurrent Staphylococcus aureus joint sepsis associated with dermatomyositis. His dermatomyositis failed to resolve on treatment with antimicrobial agents alone, indicating that if staphylococcal infection was the triggering event for the dermatomyositis then the subsequent process was apparently self perpetuating, requiring cytotoxic agents for its control. This case can be interpreted as possible further evidence for the triggering of autoimmune disease by infective agents.

Protease-Mediated Growth of Staphylococcus aureus on Host Proteins Is opp3 Dependent.

Staphylococcus aureus has the ability to cause infections in multiple organ systems, suggesting an ability to rapidly adapt to changing carbon and nitrogen sources. Although there is little information about the nutrients available at specific sites of infection, a mature skin abscess has been characterized as glucose depleted, indicating that peptides and free amino acids are an important source of nutrients for the bacteria. Our studies have found that mutations in enzymes necessary for growth on amino acids, including pyruvate carboxykinase (ΔpckA) and glutamate dehydrogenase (ΔgudB), reduc

Feasibility and Potential Clinical Ramifications of Using Bacteriophage Therapy for S. aureus Necrotizing Fasciitis.

Background:Staphylococcus aureus necrotizing fasciitis is a life-threatening infection requiring aggressive surgical and medical management. Despite these interventions, tremendous morbidity and mortality occur. Thus, novel agents are needed to reduce these negative outcomes. Consequently, the aims of this translational study were to evaluate the feasibility of using bacteriophages and the potential clinical ramifications of using bacteriophages in treatment of S. aureus necrotizing fasciitis. Methods: Necrotizing fasciitis clinical isolates (n = 6) were tested against different Staphylococcal

A case of guttate psoriasis following Kawasaki disease.

We report a case of guttate psoriasis following Kawasaki disease, in a patient with Staphylococcus aureus demonstrated in a throat swab. We suggest that preceding staphylococcal infection can play a key role in the pathogenesis of some cases of guttate psoriasis, possibly by the production of superantigens.

Cefazolin in the treatment of central nervous system infections: A narrative review and recommendation.

Infections of the central nervous system (CNS) are complex to treat and associated with significant morbidity and mortality. Historically, antistaphylococcal penicillins such as nafcillin were recommended for the treatment of methicillin-susceptible staphylococcal CNS infections. However, the use of antistaphylococcal penicillins presents challenges, such as frequent dosing administration and adverse events with protracted use. This narrative reviews available clinical and pharmacokinetic/pharmacodynamic (PK/PD) data for cefazolin in CNS infections and produces a recommendation for use. Based
