How contemporary academic structures constrain scientific creativity and hold back early-career researchers.

We argue that contemporary scientific systems progressively constrain high-risk and conceptually innovative research while being increasingly structured around short funding cycles, productivity-based evaluation criteria and risk-averse frameworks that favour predictable and non-transformative research outputs. Drawing on recent empirical literature, we show how such systems can place disproportionate pressure on early-career researchers by incentivizing safe, tractable and easily evaluated outputs. Structural academic mechanisms such as peer review and funding, escalating publication costs an

Mechanisms and precision intervention strategies of time-dependent dynamic regulation of matrix metalloproteinase-mediated cerebrovascular barrier disruption and neurovascular unit remodeling in stroke.

Stroke remains a leading cause of global mortality and long-term disability. Despite the clinical success of reperfusion therapies, their efficacy is hampered by narrow therapeutic windows and the risk of secondary brain injury. Matrix metalloproteinases (MMPs), a family of zinc-dependent endopeptidases, have emerged as pivotal biphasic regulators in the onset, progression, and resolution of ischemic stroke. This review synthesizes current evidence regarding the multifaceted roles of MMPs in stroke pathophysiology. Prior to ictus, MMPs increase vascular vulnerability by driving pathological re

The rise and fall of HIV in Kinshasa over a century.

Kinshasa was the crucible of the HIV pandemic, the only place on Earth where its path can be tracked over a century through history, demography, evolutionary biology, epidemiological studies, and population-based surveys. Altogether, these tools demonstrate that HIV is fading away from Kinshasa. Between HIV's arrival (≈1916) and the last survey (2023-2024), Kinshasa's population increased from 14 000 to 17 million inhabitants. After an exponential growth of persons with HIV (PWH) from the mid-1950s until 1988, this stalled between 1988 and 2013-2014. From 85 900 PWH in 2013-2014, this number d

Breaking Barriers: Essential Reforms for a Competitive and Affordable Biosimilars Market.

PURPOSE OF REVIEW: This update examines the current policy landscape for biosimilars, particularly those approved for treating rheumatoid arthritis. We review recent literature and recommend policies to align the biosimilars market with the goals of the Biologic Price Competition and Innovation Act (BPCIA). We examine updated evidence on biosimilar safety and effectiveness, recent FDA guidance on interchangeability, and challenges posed by anti-competitive behaviors and government regulations. Additionally, we analyze financial incentives and market dynamics affecting biosimilar adoption and c

Mycoplasma corogypsi-associated polyarthritis and tenosynovitis in black vultures (Coragyps atratus).

Three wild American black vultures (Coragyps atratus) were presented to rehabilitation centers with swelling of multiple joints, including elbows, stifles, hocks, and carpal joints, and of the gastrocnemius tendons. Cytological examination of the joint fluid exudate indicated heterophilic arthritis. Radiographic examination in 2 vultures demonstrated periarticular soft tissue swelling in both birds and irregular articular surfaces with subchondral bone erosion in both elbows in 1 bird. Prolonged antibiotic therapy administered in 2 birds did not improve the clinical signs. Necropsy and histolo

Recovery from depression.

Full recovery from depression, as contrasted with symptom improvement, is a relatively new concept and therapeutic goal. It is an important goal, because the failure to achieve this goal leaves many patients with less productive and fulfilling lives, it leaves some children with lasting deficits, and it deprives families and societies of loved ones’ and employees’ care and investment. As a new therapeutic concept, recovery from depression is not as easy to define as it might seem; many or most patients were not euthymic before an episode of depression or have had some level of depression throu

Leadership run amok. The destructive potential of overachievers.

The desire to achieve is a major source of strength in business, and it is on the rise. The authors' consulting firm has seen a steady increase in the extent to which achievement motivates managers. There's a dark side to the trend, however. By relentlessly focusing on tasks and goals, an executive or company can damage performance. Overachievers tend to command and coerce, stifling subordinates. Psychologist David McClelland identified three drivers of behavior: achievement, meeting a standard of excellence; affiliation, maintaining close relationships; and power, having an impact on others.

A pattern recognition approach for the quantification of horse and rider interactions.

REASONS FOR PERFORMING STUDY: Interactions of various systems were investigated in several studies of dynamic systems, but the interactions between horse and rider have not yet been documented. These interactions include the rider's ability to control the horse, adapt to the horse and maintain both participants' body position. An optimum interaction is also adapted to the individual nature of the horse. OBJECTIVE: To identify rider-horse interactions by means of artificial neural nets analysing the time-continuous pattern. METHODS: Fourteen horses were measured trotting on hand, and ridden a

Is overcorrection preferable for repair of degenerated articular cartilage after open-wedge high tibial osteotomy?

PURPOSE: This study was performed to determine whether the overcorrected knee could obtain a higher ratio of articular cartilage repair in the medial compartment of the femorotibial joint after open-wedge high tibial osteotomy (HTO). The hypothesis of the study was that overcorrected knees had a higher ratio of articular cartilage repair than moderately corrected knees. METHODS: A total of 71 knees that underwent arthroscopy to evaluate the articular cartilage during open-wedge HTO and second-look arthroscopy were reviewed. The articular cartilage was classified as no repair or repair accordi

Rigid stepped plate for internal fixation for high tibial osteotomy.

High tibial osteotomy is regarded as an effective treatment for unicompartmental arthrosis of the knee, but the optimal fixation method has not yet been devised. The purpose of this study was to confirm the superior strength of a rigid stepped plate as compared to a 4-hole L-plate device. Lateral closing-wedge high tibial osteotomy was performed on 30 ten-month-old porcine tibiae. Fifteen tibiae were fixed using a rigid stepped plate and 15 were fixed with a 4-hole L-plate. Three types of deforming force-compression, valgus bending, and varus bending-were each applied to 5 pair of tibiae fitte

Rehabilitation after extra-articular stabilisation of cranial cruciate ligament rupture in dogs.

Patients at the Blue Star Foundation animal hospital in Gothenburg, Sweden, underwent a procedure with an extracapsular suture technique for cranial cruciate ligament rupture. Two groups were compared; one of the groups received rehabilitation by the owner, and the other received professional rehabilitation by a trained physiotherapists. The dogs where reexamined at four, 12 and 24 weeks postoperatively. The results were 'good' in both groups but a difference was not seen between the groups. This study shows that in the dogs with a body weight of between 25 and 50 kg with cranial cruciate liga

Assessment of the efficacy of intra-articular platelet rich plasma treatment in an ACLT experimental model by dynamic contrast enhancement MRI of knee subchondral bone marrow and MRI T2∗ measurement of articular cartilage.

OBJECTIVE: The vascularization of subchondral bone plays a significant role in the progression of knee osteoarthritis (OA). Treatment with platelet-rich plasma (PRP) has positive effects on cartilage lesions. However, PRP's efficacy for subchondral bone marrow lesions and the relationship of these lesions to cartilage are still undiscovered. Therefore, our aims were first to longitudinally investigate the change in subchondral flow by dynamic contrast enhanced MRI and degeneration of cartilage by MRI T2∗ in an anterior cruciate transection rodent (ACLT) model, and second to examine changes in
