Retrobulbar hemorrhage after tenecteplase thrombolysis for acute stroke following strabismus surgery.

Ocular and orbital hemorrhage are rare but serious complications of thrombolytic therapy. We present the case of an 83-year-old woman who developed a retrobulbar hemorrhage resulting in left orbital compartment syndrome following tenecteplase thrombolysis for acute ischemic stroke shortly after uncomplicated bilateral strabismus surgery. The patient underwent lateral canthotomy with inferior and superior cantholysis for intraocular pressure reduction. Hemostasis was achieved after the use of topical thrombin-gelatin and thrombolytic reversal with cryoprecipitate and platelets. To our knowledge…

Automated Measurement of Strabismus Angle Using a Commercial Virtual Reality Headset.

Current clinical measurements of strabismus angles (SAs) are manual and require a skilled examiner and active cooperation by patients. This results in high interexaminer variability, and clinical use is limited by the availability of trained examiners. An objective and automated procedure, independent of the examiner, would be useful. This single-center, prospective, diagnostic feasibility study compared the vertical and horizontal SA of patients, as measured with a commercially available virtual reality headset (VRH) and custom software with the gold standard measurements performed manually w…

BOTOX injection to treat strabismus after infant botulism type B infection.

PURPOSE: The significance of botulinum toxin to ophthalmologists is twofold. Botulism, a medical emergency, frequently presents with ocular findings including blurred vision, diplopia, ptosis, and photophobia as a result of the neurotoxin produced by Clostridium botulinum. However, botulinum toxins also have therapeutic uses for medical conditions including strabismus. The safety and efficacy of Botulinum toxin A in patients with a history of botulism has not been reported. OBSERVATIONS: We report a 9-week-old infant, diagnosed with type B toxin positive infant botulism treated with human bot…

Atypical restrictive strabismus secondary to an anomalous orbital structure: differential diagnosis.

Anomalous orbital structures are very rare causes of restrictive strabismus. Of the 3 types described in the literature, one involves accessory muscle fibers that are innervated by the III and/or VI cranial nerves originating in the posterior orbit and inserting on the globe, optic nerve, or extraocular muscles. Although these structures do not cause specific symptoms that enable us to make a diagnosis, we must take them into account in patients with atypical restrictive strabismus. Computed tomography and, particularly, orbital magnetic resonance imaging are essential for correct diagnosis an…
