Over the past century, cerebrospinal fluid (CSF) dynamics have largely been interpreted through the "third circulation theory," which posits that CSF is produced by the choroid plexus that flows from ventricles to the subarachnoid space and is absorbed mainly through arachnoid granulations. The concepts of the glymphatic system and meningeal lymphatic drainage have since driven a paradigm shift in this framework. In the glymphatic pathway, CSF enters periarterial spaces and, via aquaporin-4 channels on astrocytic endfeet, is exchanged with interstitial fluid to promote parenchymal waste cleara…

Cerebrospinal fluid dynamics were studied in eight dogs during normal, hydrocephalic, and postoperative phases. Radionuclide-labeled substances introduced into the normal ventricular system flow out of the exits from the fourth ventricle to the convexity subarachnoid spaces superiorly to be absorbed in the sagittal sinus, and basorostrally to the exits from the perineural olfactory sheath into the nose to produce physiological cerebrospinal fluid (CSF) rhinorrhea. Serial radionuclide ventriculography of the head following intraventricular isotope injection of labeled proteins and chelate into …

Recurrent haemorrhage into the subarachnoid space causes superficial siderosis, which clinically manifests as cerebellar ataxia, sensorineural hearing loss, and myelopathy. Two patients developed clinical, radiological, and biochemical evidence of superficial siderosis many years after surgery. One had two posterior fossa operations, a left temporal craniectomy, and radiotherapy for a presumed brain tumour before developing clinical evidence of superficial sidersosis 37 years later. The other had small bilateral subdural collections from recurrent shunt revisions following posterior fossa surg…

BACKGROUND: Interhemispheric subdural hematoma (ISH) usually occurs after head trauma; nontraumatic ISH is extremely rare.
CASE DESCRIPTION: The authors describe a 62-year-old male presenting with severe headache and ptosis on the left side. Computed tomography (CT) and magnetic resonance imaging disclosed a hematoma in the interhemispheric subdural space without subarachnoid hemorrhage. Cerebral angiography revealed an aneurysm arising from the left internal carotid-posterior communicating artery (IC-PC) junction. The patient underwent emergency clipping of the aneurysm and was discharged wi

A clinical usefulness of computed tomography was reviewed in 38 consecutive patients with subarachnoid haemorrhage and multiple intracranial aneurysms. CT helped in identification of ruptured aneurysm in more than 70% of cases (it pinpointed source of bleeding in 55% of patients and showed region of haemorrhage in 15% of cases). Its main limitation was inability to detect blood in subarachnoid space (SAS) in patients who were diagnosed later after bleeding (23%). Intracerebral haematoma or localized blood accumulated in SAS showed unequivocally the burst aneurysm. Symmetrical and especially as…

Massive fatal subarachnoid hemorrhage maximal in the posterior fossa following minor trauma to the head and neck was first attributed to rupture of a vertebral artery (VA) in 1971. Typically, a young, healthy, but intoxicated man receives a minor blow and immediately collapses and dies within minutes. Early reports suggested an extracranial site of rupture with upward tracking of blood into the subarachnoid space. In few cases has this site of rupture been histologically verified, and more recent reports have drawn attention to intracranial rupture of the VA. We describe herein six cases in wh…

A cerebrospinal fluid leak from the cranial cavity requires presence of a fistulous communication between the subarachnoid and extracranial space through the skull base. Imaging plays a crucial role in identifying and characterizing the skull base defect and evaluating coexisting pathologies that may alter surgical approach. The authors discuss the epidemiology, imaging, and management of cerebrospinal fluid leak, with particular reference to the imaging modalities and findings.

Extradural arachnoid cysts are uncommon expanding lesions in the spinal canal, which may communicate with the subarachnoid space. Usually located in the lower thoracic spine, they may cause symptoms by compressing the spinal cord or nerve roots. We report a case of an extradural thoracic arachnoid cyst revealed by progressive spinal cord compression. CT myelography and MRI enabled diagnosis. Rapid neurological improvement was observed after surgical resection.

The spinal forms of neurocysticercosis are rather rare. The more common presentation is the leptomeningeal form. We have reported two patients with an exclusively spinal involvement verified through surgery. The current theory of downward migration of the parasites from the cerebral to the spinal subarachnoid space cannot explain primary spinal forms, and it is suggested that retrograde flow through the epidural vertebral veins provides an alternative route.

Small foci of high attenuation are seen within the subarachnoid space on high resolution axial transverse computed tomography (CT) of the lumbar spine. By use of reformatted images in oblique planes, these foci form linear structures of high attenuation corresponding to the nerve roots of the cauda equina. Unenhanced CT is therefore capable of demonstrating the nerves of the cauda equina in selected cases.

BACKGROUND AND PURPOSE: We developed an experimental canine subarachnoid hemorrhage (SAH) model in which one-time blood injection by means of a microcatheter into the ventral cisterna magna (CM) is performed without direct CM puncture. We assessed the severity and duration of the vasospasm produced in this model.
METHODS: Fresh autologous blood (0.25 or 0.5 ml/kg) or saline (0.5 ml/kg) was injected into the ventral CM of dogs through a microcatheter inserted at the lumbar region. Serial angiograms were obtained on days 3, 7, 10, and 14, and chronologic changes in the mean diameter of the basi

Bilateral, idiopathic, essentially asymptomatic but hemodynamically significant stenosis or occlusion of the sigmoid sinuses is extremely rare. We report a child with the angiographic features of occlusion of the right sigmoid sinus and severe stenosis on the contralateral side, presenting with megalencephaly and a cranial bruit. Cranial computed tomographic scans demonstrated increased subarachnoid spaces and borderline ventricular enlargement.

The published literature suggests that the technique of epidural "blood patch" injection is both safe and effective for the treatment and prevention of headache after spinal puncture. However, a case is reported in which the complications following a "blood patch" outweighed the disability and discomfort produced by the headache itself. The etiology is believed to have been inadvertent injection of blood into the subarachnoid space.

PURPOSE: To discuss the possible ramifications of neuraxial analgesia and anesthesia in women with tattoos involving their midline lumbar ar … tattoo. Coring is a complication of neuraxial anesthesia that may lead to epidermoid tumours in the subarachnoid space. Theoretically, a pigment-containing tissue core from a tattoo could be deposited into the epidural, subdural or subarachnoid spaces, leading to later neurological complications.
CONCLUSIONS: There is no information in the literature about possible …

BACKGROUND: Understanding the pathogenesis of the chiari-like malformation in the Cavalier King Charles Spaniel (CKCS) is incomplete, and current hypotheses do not fully explain the development of syringomyelia (SM) in the spinal cords of affected dogs. This study investigates an unconventional pathogenetic theory for the development of cerebrospinal fluid (CSF) pressure waves in the subarachnoid space in CKCS with SM, by analogy with human diseases. In children with achondroplasia the shortening of the skull base can lead to a narrowing of the jugular foramina (JF) between the cranial base sy…

Four premature infants weighing less than 2,500 grams underwent lumboperitoneal shunt for their communicating hydrocephalus. Using a microscope, mini-laminectomy and exact placement of the proximal end of the shunt tube into the subarachnoid space was achieved. After the lumboperitoneal shunting, hydrocephalus in all four patients was relieved without any complications. The author discusses the availability of lumboperitoneal shunt for hydrocephalus in premature infants.
