Glycaemic control influences the relationship between plasma PCSK9 and LDL cholesterol in type 1 diabetes.

Pro-protein convertase subtilisin/kexin type 9 (PCSK9) is a critical regulator of LDL cholesterol metabolism. Little is known, however, about the regulation of PCSK9 in patients with type 1 diabetes (T1D). In the present study, we aimed to determine the relationship between circulating PCSK9 and metabolic variables in T1D. Plasma PCSK9 levels were measured in 195 people with T1D (mean age 38.8 years, mean diabetes duration 17.2 years, mean glycated haemoglobin [HbA1c] 8.3%), who were free of any lipid-lowering agent. Plasma PCSK9 was positively correlated with LDL cholesterol (P = .0007), trig

Clinical implications of the IMPROVE-IT trial in the light of current and future lipid-lowering treatment options.

INTRODUCTION: A residual risk of morbidity and mortality from cardiovascular (CV) disease remains despite statin therapy. This situation has generated an interest in finding novel approaches of combining statins with other lipid-lowering agents, or finding new lipid and non-lipid targets, such as triglycerides, high-density lipoprotein cholesterol (HDL-C), non-HDL-C, proprotein convertase subtilisin/kexin type 9 (PCSK9) gene, cholesterol ester transfer protein (CETP), lipoprotein (a), fibrinogen or C-reactive protein. Hypolipidaemic drug treatment: yesterday is not gone yet, today is challenging and tomorrow is coming soon; let us combine them all.

Statins remain the cornerstone of hypolipidaemic drug treatment. The recent American College of Cardiology (ACC)/American Heart Association (AHA) lipid guidelines suggest using percent reductions of low density lipoprotein cholesterol (LDL-C), according to cardiovascular disease (CVD) risk, rather than specific LDL-C targets. These guidelines raised concerns and other Societies (US, International, European) have not endorsed them. The implementation of previous guidelines in clinical practice is suboptimal due to attitudes of physicians and restrictions in health care systems. Monoclonal antib

Computational reverse-engineering of a spider-venom derived peptide active against Plasmodium falciparum SUB1.

BACKGROUND: Psalmopeotoxin I (PcFK1), a protein of 33 aminoacids derived from the venom of the spider Psalmopoeus Cambridgei, is able to inhibit the growth of Plasmodium falciparum malaria parasites with an IC50 in the low micromolar range. PcFK1 was proposed to act as an ion channel inhibitor, although experimental validation of this mechanism is lacking. The surface loops of PcFK1 have some sequence similarity with the parasite protein sequences cleaved by PfSUB1, a subtilisin-like protease essential for egress of Plasmodium falciparum merozoites and invasion into erythrocytes. As PfSUB1 has

High-dose atorvastatin causes a rapid sustained increase in human serum PCSK9 and disrupts its correlation with LDL cholesterol.

Proprotein convertase subtilisin kexin type 9 (PCSK9) is a key regulator of serum LDL-cholesterol (LDL-C) levels. PCSK9 is secreted by the liver into the plasma and binds the hepatic LDL receptor (LDLR), causing its subsequent degradation. We first demonstrated that a moderate dose of atorvastatin (40 mg) increases PCSK9 serum levels, suggesting why increasing statin doses may have diminished efficacy with regard to further LDL-C lowering. Since that initial observation, at least two other groups have reported statin-induced PCSK9 increases. To date, no analysis of the effect of high-dose ator

Toward an alternative intrinsic probe for spectroscopic characterization of a protein.

The intrinsic fluorescent amino acid tryptophan is the unanimous choice for the spectroscopic investigation of proteins. However, several complicacies in the interpretation of tryptophan fluorescence in a protein are inevitable and an alternative intrinsic protein probe is a longstanding demand. In this contribution, we report an electron-transfer reaction in a human transporter protein (HSA) cavity which causes the tryptophan residue (Trp214) to undergo chemical modification to form one of its metabolites kynurenine (Kyn214). Structural integrity upon modification of the native protein is con

Effects of age and retinal degeneration on the expression of proprotein convertases in the visual cortex.

Proprotein convertases (PCs) comprise a large family of subtilisin-like, eukaryotic, serine endoproteases that process substrates important in the development, homeostasis, and pathology of the nervous system. Despite important interactions with these substrates, including neurotrophins, PC expression throughout normal postnatal development and disease progression in the brain remains unknown. The primary objective of this study was to determine whether the expression profiles of widely expressed and tissue-specific PCs varied during normal brain development or neurological disorders. We exami

Familial hypercholesterolaemia.

Familial hypercholesterolaemia (FH), defined as the heritable occurrence of severe hypercholesterolaemia with cholesterol deposits in tendons and premature heart disease, is caused by at least four genes in sterol and lipoprotein pathways and displays varying gene-dose effects. The genes are the low-density lipoprotein (LDL) receptor, apolipoprotein (apo) B, proprotein convertase subtilisin/kexin 9, and the autosomal recessive hypercholesterolaemia (ARH) adaptor protein. All of these disorders have in common defective clearance of LDL within a complex system of lipid and lipoprotein metabolism

Determinants of circulating PCSK9 levels and the efficacy of PCSK9 inhibitor therapies in chronic kidney disease: a systematic review.

PURPOSE OF REVIEW: Chronic kidney disease (CKD) significantly increases the risk of atherosclerotic cardiovascular disease (ASCVD), with dyslipidemia-particularly elevated low-density lipoprotein cholesterol (LDL-C)-being a shared risk factor. While statin-based therapies are effective in early-stage CKD, their benefits in dialysis and kidney transplant patients remain inconclusive. Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a key regulator of lipid metabolism. This review aims to summarize the factors influencing PCSK9 levels in CKD patients and evaluate the safety and efficacy

Management of autosomal recessive hypercholesterolemia in a patient with an LDLRAP1 mutation.

Familial hypercholesterolemia (FH) is characterized by a lifelong elevation of low-density lipoprotein cholesterol (LDL-C), conferring an increased risk of premature atherosclerotic disease and its associated burden of morbidity and mortality. Autosomal recessive hypercholesterolemia (ARH) is a rare form of homozygous FH (HoFH) and is a distinct subset caused by mutations in the low-density lipoprotein receptor adaptor protein 1 (LDLRAP1) gene. We present a 29-year-old South Asian male who visited the lipid clinic with a markedly elevated, untreated LDL-C level of 557 mg/dL. Clinical and genet

Update on familial hypercholesterolemia: An expert clinical consensus from the National Lipid Association.

Familial hypercholesterolemia (FH) is a common genetic disorder characterized by lifelong elevated low-density lipoprotein cholesterol (LDL-C), leading to a high risk of early onset atherosclerotic cardiovascular disease (ASCVD). This document provides an update to the National Lipid Association's 2011 clinical guidance, summarizing the remarkable progress in the field. With a global prevalence of approximately 1 in 311, FH remains severely underdiagnosed. This guidance reviews current diagnostic criteria, including the expanding role of genetic testing to complement diagnosis and to facilitat

JCL Roundtable: Rewriting the rules for primary prevention: New outcomes from intensive LDL-C reduction in high-risk patients.

This Journal of Clinical Lipidology Roundtable presents a conversation between Drs Robert P. Giugliano, Nicholas A. Marston, and Kevin C. Maki, who discussed clinical trials investigating intensive low-density lipoprotein cholesterol (LDL-C) reduction in high-risk patients. Their focus was on cardiovascular outcomes data from the recent Effect of Evolocumab in Patients at High Cardiovascular Risk Without Prior Myocardial Infarction or Stroke (VESALIUS-CV) trial, as well as earlier secondary prevention outcomes trials for proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor monoclona

A new strategy to reduce casein antigenicity: Investigating immune response, gut microbiota dysbiosis, and metabolic changes in allergic mice.

As the primary allergen in cow's milk, casein (CN) has restricted the development of hypoallergenic infant formulas. This study proposes a targeted enzymatic hydrolysis strategy to eliminate the antigenic epitopes of CN by taking advantage of the hydrolytic characteristics of different proteases, and to evaluate its antigen-lowering effect from multiple perspectives with the help of an allergic mouse model. The results of LC-MS/MS and ELISA indicate that the SC group (subtilisin protease + chymotrypsin hydrolysate) could effectively remove antigen epitopes and significantly reduce the antigeni

A novel Kazal-type protease inhibitor from Aedes aegypti effective for NS2B-NS3 protease from the Zika virus and subtilases.

In recent years, Zika virus infections have spread geographically across various territories, including French Polynesia, the South Pacific, and South America, with a significant impact on Brazil. Severe Zika virus infections can lead to neurological complications, such as Guillain-Barré syndrome in adults. Furthermore, infection during pregnancy can result in microcephaly and other congenital brain malformations in the fetus. Aedes aegypti cohabits with humans and is the principal vector responsible for transmitting Zika virus to humans. Recent advances have elucidated the molecules involved

Inclisiran in Cardiovascular Health: A Review of Mechanisms, Efficacy, and Future Prospects.

Dyslipidemia is a significant risk factor for cardiovascular disease, and managing low-density lipoprotein cholesterol levels is a crucial strategy for both the prevention and treatment of cardiovascular disease. The active ingredient in the novel lipid-lowering drug, inclisiran, is a new type of small interfering RNA that primarily regulates blood lipid levels by inhibiting the synthesis of proprotein convertase subtilisin type 9 (PCSK9). Enhanced lipid-lowering effects can be achieved following 3 administrations. This article summarizes inclisiran's mechanism of action, efficacy, safety, cur

Shared Risk Factors and Molecular Mechanisms Between Aortic Stenosis and Atherosclerosis: A Rationale for Therapeutic Repositioning.

Aortic stenosis (AS) and atherosclerosis are progressive cardiovascular conditions that frequently coexist and share multiple clinical and molecular features. Medical therapies have shown effectiveness in preventing and treating atherosclerosis and its consequences. For AS, effective pharmacological therapies remain limited. Understanding the shared risk factors and mechanisms between the two conditions may provide opportunities for therapeutic repositioning in AS. We performed a narrative review focusing on studies published from 2005 to 2025. Inclusion criteria encompassed clinical trials, e

Design of the "EAST" strategy in patients with symptomatic intracranial atherosclerotic stenosis.

INTRODUCTION: There is a high risk of stroke occurrence and recurrence in patients with intracranial atherosclerotic stenosis (ICAS) despite aggressive medical therapy. Evolocumab is a monoclonal antibody which can inhibit proprotein convertase subtilisin-kexin type 9 (PCSK9) and effectively reduce the level of low-density lipoprotein cholesterol. We hypothesize that evolocumab added to statin therapy (EAST) can stabilize intracranial plaques in patients with symptomatic ICAS. METHODS AND ANALYSIS: This is a prospective, randomized, open-label, blinded end-point study, which will assess the e

Chimeras co-targeting antigens and FcγRIIb trigger degradation of extracellular soluble proteins and pathological aggregates.

While the clinical utility of conventional antibody therapies is undeniable, their therapeutic potential is often constrained high antigen loads and the recycling of antibody-antigen complexes via neonatal Fc receptor (FcRn). Here, we present a platform, based on a design similar to bispecific antibodies, FcγRIIb-Targeting Chimeras (FcRTAC). These constructs recognise antigens with one arm and bind FcγRIIb with the other arm to harness the unique endocytic properties of FcγRIIb to direct the recognized pathogenic antigens to lysosomes for irreversible degradation. The FcRTAC platform demonstra

Emerging roles of PCSK9 in kidney disease: lipid metabolism, megalin regulation and proteinuria.

Chronic kidney disease (CKD) is a significant risk factor for cardiovascular disease (CVD). Key features of CKD include proteinuria and reduced glomerular filtration rate, both of which are linked to disease progression and adverse outcomes. Dyslipidemia, a major CVD risk factor, often correlates with CKD severity and is inadequately addressed by conventional therapies. Proprotein convertase subtilisin/kexin type 9 (PCSK9) plays a critical role in lipid metabolism by modulating low-density lipoprotein receptor (LDLR) levels and has emerged as a therapeutic target for managing dyslipidemia. Sepsis-associated liver dysfunction confounds the association between circulating PCSK9 concentration and mortality in septic patients: A single-center retrospective cohort study.

BACKGROUND: Pro-protein convertase subtilisin/kexin 9 (PCSK9) is a potential therapeutic target in sepsis. However, its role as a prognostic biomarker remains unclear. This study investigated the relationship between plasma PCSK9 concentration and mortality in septic patients, focusing on the impact of sepsis-associated liver dysfunction (SALD). METHODS: Patients meeting the Sepsis-3 criteria were enrolled. Plasma samples were collected within 24 h of admission. Patients were grouped into tertiles based on PCSK9 concentration and followed up for survival analysis. Further analyses were perfor
