Single coronary artery arising anomalously from the pulmonary trunk.

A case of sudden death in a 10-day-old infant was studied. Pathologic findings included myocardial infarction and a single coronary artery that arose anomalously from the pulmonary artery. This was the only source of the coronary arterial blood supply.

Resuscitated Sudden Cardiac Arrest of a Neonate with Congenital LQT Syndrome-Associated Torsades de Pointes: A Case Report and Literature Review.

Sudden infant death syndrome (SIDS), the most common cause of infant death in developed countries, is attributed to diverse trigger factors. Malignant cardiac dysrhythmias are potentially treatable etiologies, and congenital long QT syndrome (LQTS) is the most common cardiac ionic channelopathy confronted. β-Blockers or class Ib agents are the drugs of choice for the control of arrhythmias, and an implantable cardioverter defibrillator (ICD) should be considered for secondary prevention in survivors of lethal cardiac death. We report the case of a 4-day old neonate, later genetically confirmed…

Molecular biology and cellular mechanisms of Brugada and long QT syndromes in infants and young children.

Sudden cardiac death accounts for 19% of sudden deaths in children between 1 and 13 years of age and 30% of sudden deaths that occur between 14 and 21 years of age. The incidence of sudden cardiac death displays 2 peaks: one between 45 and 75 years of age, as a result of coronary artery disease, and the other between birth and 6 months of age, caused by sudden infant death syndrome. The role of cardiac arrhythmias in sudden infant death syndrome has long been a matter of debate and the role of cardiac arrhythmias in children in general is not well defined. Recent findings point to a contributi…

Unexpected death of an infant suffocated in the course of breastfeeding when the mother fell asleep.

Death following accidental suffocation due to overlaying is often overlooked, and still attributed to Sudden Infant Death Syndrome (SIDS). We report a case of sudden infant death following accidental suffocation due to breast overlaying during breastfeeding. We report the death of a two-month-old male baby, without medical history, who was found lifeless wedged under his mother's breast. The mother subsequently admitted that she had fallen asleep while breastfeeding and awoke a few minutes later to find him wedged under her breast and not breathing. External examination of the infant's body re…

Disparities in mortality rates among US infants born late preterm or early term, 2003-2005.

The purpose of this study was to identify disparities in neonatal, post-neonatal, and overall infant mortality rates among infants born late … ants born early term, the rate was higher among twins (3.67 vs. 3.15). Congenital malformations and sudden infant death syndrome were the leading causes of death among both late preterm and early term infants. Infant mortality rates among infants born late preterm or early term varied substantially by maternal race/ethnicity, maternal age, and plurality. Information …

Abnormal spontaneous movements in infants with repeated sleep apnoeas.

Infants with repeated apnoea during sleep have received great attention for the assumed reason of being at-risk for sudden infant death. The present paper reports findings which indicate a different risk, namely for neurological impairment during infancy due to repeated hypoxia. A very strong correlation exists between the respiratory measurements based on a polygraphic all-night recording (PtcO2 drops and apnoea incidence and duration) and the impairment of the spontaneous movement repertoire in 114 infants, aged between 3 and 26 weeks. All infants have been video recorded at the same day as …

[Molecular autopsy of sudden cardiac death: from postmortem to clinical approach].

Sudden cardiac death is one of the most prevalent cause of death in developed countries. Its aetiology varies according to the age. Some cardiac diseases may explain sudden death with minimal or no anatomic findings. However, many cardiac diseases, as for example channelopathies and hypertrophic cardiomyopathy have a genetic basis. Therefore genetic analyses (molecular autopsy) are becoming a useful tool in forensic medicine to identify the cause of sudden cardiac death and to improve the early diagnosis of asymptomatic carriers among relatives.

Hypoxic arousal responses in infants with bronchopulmonary dysplasia.

Infants with bronchopulmonary dysplasia have a high incidence of sudden, unexplained death in the postneonatal period, yet the cause of these deaths is unknown. Frequent episodes of clinically unsuspected arterial oxygen desaturation have recently been described in infants with bronchopulmonary dysplasia. We hypothesized that infants with bronchopulmonary dysplasia who experience frequent episodes of hypoxia may have abnormal arousal responses to these hypoxic episodes. We studied 12 infants with bronchopulmonary dysplasia at 41.4 +/- 1.3 weeks postconceptional age. Hypoxic arousal responses w

International comparison of sudden unexpected death in infancy rates using a newly proposed set of cause-of-death codes.

BACKGROUND: Comparing rates of sudden unexpected death in infancy (SUDI) in different countries and over time is difficult, as these deaths … as a great variability in how each country codes SUDI. For example, the proportion of SUDI coded as sudden infant death syndrome (R95) ranged from 32.6% in Japan to 72.5% in Germany. The proportion of deaths coded as accidental suffocation and strangulation in bed (W75) ranged from 1.1% in Germany to 31.7% in New Zealand. Japan was the only country to consistently us …

Pathological causes of sudden death in autopsied children with reference to peculiar findings: An Egyptian perspective.

Childhood is a long period extending up to the age of 18 years. Childhood encompasses different developmental stages; each stage has specific characteristics. This 5-year study included 244 autopsied children who died unexpectedly due to natural causes. This study was conducted in the forensic pathology unit of the Egyptian Forensic Medicine Authority (EFMA). Pathological causes of death were diagnosed in 181 cases, representing nearly three-quarters (74.2 %) of cases. Males represented 60.8 % of these cases. More than half (51.4 %) of deaths due to natural disease occurred within the first mo

[Sudden death in new-born babies (author's transl)].

This sudden death of a new-born baby, which had been thriving up to then, is a dramatic but not exceptional event. A systematic search for the etiology must be made, and this should include an autopsy in order that the diagnosis of "sudden and unexplained death" is confirmed only after eliminating all other possible causes. Knowledge of this syndrome has benefited from epidemiological surveys which have defined the previous clinical condition, but none of the many etiollotical hypotheses envisaged can take into account the total number of cases reported. One of the most recent cases developed

Sudden death due to ventricular septal defect.

Ventricular septal defects (VSD) are usually considered non-life-threatening, usually closing spontaneously or causing symptoms of congestive heart failure, which can be surgically treated in time to save the patient's life. Despite the usually benign clinical course of VSD, serious arrhythmias occur in 16-31% of patients. Sudden death accounted for one-third of all deaths in a series of medically managed patients and occurred in 4.2% of patients in a study of VSD and arrhythmias. Cardiac hypertrophy is the common denominator in all cases reported in detail of VSD-associated sudden death. We h

Short note: The fetal origins hypothesis: linking pineal gland hypoplasia with coronary heart disease and stroke.

Pineal gland hypoplasia secondary to intrauterine malnutrition is suggested as a fetal contribution to sudden infant death syndrome, coronary artery disease and ischemic stroke. The loss of melatonin's antioxidant activity is proposed as an additional factor to consider in atherosclerotic vascular disease.

Infants who die in shared sleeping situations differ from those who die while sleeping alone.

AIM: To determine whether there are differences between infants who are sharing a sleeping surface with others, compared to those who die alone. METHODS: A literature review was undertaken of PubMed and Google Scholar databases using search terms: sudden infant death syndrome, SIDS, co-sleeping and overlaying. RESULTS: Statistically significant differences were found between the two groups in the sex ratios, and in staining of brain sections for β-amyloid precursor protein (β-APP), glial fibrillary acidic protein (GFAP) and terminal deoxynucleotidyl transferase-mediated dUTP nick-end labelli…

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) diagnosed in children and adolescents.

BACKGROUND: Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare but potentially fatal congenital coronary anomaly associated with early infant mortality and sudden adult death. By the development or lack of coronary collateral, it can be classified as infantile or adult type. However, even with the compensatory mechanism in adult patients, there is an estimated 80 to 90% incidence of sudden death at the mean age of 35 years. METHODS: We enrolled 9 patients with ALCAPA within the age group 5 to 16 years. RESULTS: Only one patient developed symptoms (apsyc

Maternal marital status and the risk of stillbirth and infant death: a population-based cohort study on 40 million births in the United States.

OBJECTIVE: The objective of our study was to evaluate the association between maternal marital status and the risk of fetal and infant death, including sudden infant death syndrome (SIDS). METHODS: We conducted a population-based cohort study using the Centers for Disease Control and Prevention's Linked Birth-Infant Death and Fetal Death data on all births in the United States between 1995 and 2004. Marital status was obtained from the birth certificate. The adjusted effect of marital status on the risk of fetal and infant mortalities was estimated using unconditional logistic regression anal…
