Syncope: experience at a tertiary care hospital in Karachi, Pakistan.

INTRODUCTION: Our aim was to determine the characteristics of patients presenting with syncope at a tertiary care hospital in Karachi, Pakistan. METHODS: A review of medical records was conducted retrospectively at the Department of Medicine, Aga Khan University Hospital, Karachi. Patients aged 16 and above, admitted from January 2000 to December 2005 with the diagnosis of syncope made by the attending physician were included. RESULTS: A total of 269 patients were included (75% males, mean age: 57.4 years). Neurogenic (vasovagal) syncope was the most common cause (47%), followed by cardiogen…

Glossopharyngeal neuralgia with syncope as a sign of neck cancer recurrence.

Glossopharyngeal neuralgia with syncope as a sign of neck cancer is a very rare condition. A review of the literature revealed only 29 cases formerly reported. We present the first Brazilian case of such association. A 68-year-old man presented with paroxysmal excruciating pain over the right side of the neck, sometimes followed by syncope. Given the suspicion of recurrent tumor from a previously treated neck malignancy, a computed tomography scan was performed and a right parapharyngeal tumor was shown. Pain and syncope were successfully controlled with carbamazepine and the patient underwent…

Role of the peripheral serotoninergic system in the pathogenesis of vasovagal syncope.

OBJECTIVE: To evaluate the role of peripheral serotoninergic system in the pathogenesis of vasovagal syncope. BACKGROUND: Increased central serotoninergic activity was suggested to play a role in sudden inhibition of sympathetic activity responsible for the genesis of vasovagal syncope. There is good correlation between the central serotoninergic activity and the plasma levels of serotonine. METHODS: In twenty-two patients (mean age 48 +/- 19 years, 10 men, 12 women) with suspected vasovagal syncope head-up tilt test (HUT) was performed. Passive HUT (60 degrees, 20 minutes) was followed, if …

Post-pneumonectomy syndrome in an adult presenting with positional syncope.

Post-pneumonectomy syndrome is a rare late complication of pneumonectomy, which is due to extreme mediastinal shift and rotation into the pneumonectomy space. Recurrent syncope following a right pneumonectomy in a 21-year-old woman was treated by insertion of an expandable silicone prosthesis into the pneumonectomy space.

Diagnostic criteria for vasovagal syncope based on a quantitative history.

AIMS: Our goal was to develop historical criteria for the diagnosis of vasovagal syncope. METHODS AND RESULTS: We administered a 118-item historical questionnaire to 418 patients with syncope and no apparent structural heart disease. The prevalence of each item was compared between patients with positive tilt tests and those with syncope of other, known causes. The contributions of symptoms to diagnoses were estimated with logistic regression, point scores were developed, and the scores were tested using receiver operator characteristic analysis. The accuracy of the decision rule was assessed…

[A 66-year old female patient with tachycardia and syncope].

A 66-year old female patient suffered from paroxysmal tachycardias, palpitations, dizziness and once a short period of unconsciousness. The surface ECG showed preexcitation, and the clinical diagnosis of WPW syndrome was established. The electrophysiological study revealed the rare occurrence of an epicardial posteroseptal accessory pathway. Retrograde venous angiography of the coronary sinus showed a coronary sinus diverticulum. Ablation of the accessory pathway in the neck of the coronary sinus diverticulum was successful. Epicardial accessory pathways in a coronary sinus diverticulum are ra

[Syncope with cardiac arrest in a 30-year-old woman].

HISTORY AND ADMISSION FINDINGS: A 30-year-old woman presented with a cardiac arrest, which spontaneously converted into a regular sinus rhythm. The patient had no prior history of cardiovascular diseases. On admission, the cardiovascular clinical examination of this young female was regular. INVESTIGATIONS: On admission, discrete inferior repolarisation changes were documented which were slightly progressive during the next day. Electrophysiologic examination of the heart was regular, angiography showed a coronary fistula and an atypical descense of the circumflex artery from the right corona

[Assessment of cases with syncope due to primary pulmonary hypertension].

Primary pulmonary hypertension is an uncommon disease. Its diagnosis is suspected after clinical examination however it can be made only after detailed evaluation of heart and lungs and exclusion of all etiologies for secondary pulmonary hypertension. Prognosis in general is poor but it ranges individually. In this article we aimed to examine this subject by interpreting a patient referred to our clinic for syncope and diagnosed as primary pulmonary hypertension.

Predisposition to vasovagal syncope in subjects with blood/injury phobia.

BACKGROUND: Most subjects with blood/injury phobia experience syncope or presyncope as part of the phobic response. We tested the hypothesis that these subjects have a constitutional autonomic dysregulation that predisposes them to vasovagal syncope during head-up tilt. METHODS AND RESULTS: We studied 11 subjects (9 females, 2 males) who had a history of syncope or presyncope only in response to a blood or injury stimulus and 11 healthy matched controls (10 females, 1 male) without a history of syncope. Blood pressure (BP) and heart rate (HR) were measured during a 15-minute baseline period w…

Prospective evaluation of patients with syncope: a population-based study.

PURPOSE: To determine the diagnostic yield of a standardized sequential evaluation of patients with syncope in a primary care teaching hospital. PATIENTS AND METHODS: All consecutive patients who presented to the emergency department with syncope as a chief complaint were enrolled. Their evaluation included initial and routine clinical examination, including carotid sinus massage, as well as electrocardiography and basic laboratory testing. Targeted tests, such as echocardiography, were used when a specific entity was suspected clinically. Other cardiovascular tests (24-hour Holter monitoring…

Subclavian steal syndrome: a rare but important cause of syncope.

An elderly woman came to our emergency room for evaluation of a syncopal episode. While climbing a flight of stairs, she had turned her head to the left and abruptly passed out. Positive physical findings included blood pressure of 141/65 mm Hg (right arm) and 80/43 mm Hg (left arm), as well as nonpalpable left radial and brachial pulses that were detectable only by Doppler ultrasonography. Carotid duplex ultrasonography showed reverse flow in the left vertebral artery and an abnormal, stenotic distal left subclavian artery. Magnetic resonance angiography confirmed complete occlusion of the le

Efficacy and safety of clonazepam in refractory neurally mediated syncope.

Neurally mediated syncope is a complex syndrome that is often difficult to manage using currently available treatment strategies. The efficacy and safety of clonazepam was evaluated in 35 patients with refractory neurally mediated syncope. All patients had syncope (n = 33) or disabling presyncope (n = 2) and a positive head-up tilt table test (HUTT) despite treatment with one or more of the following therapies: beta-blocker, high-salt diet, fludrocortisone, elastic compression stockings, and disopyramide. Clonazepam was initiated at 0.5 mg/day and titrated in 0.25-0.5 mg/day increments for sym…

[Heart rate variability and vagal syncope in the young adult].

Heart rate variability is a sign of sympathetic activity. The authors compared two study populations of young males aged 19 to 30 years: population T comprised 15 healthy volunteers who had two negative tilt tests, one under basal conditions and the other after a bolus of isoproterenol; population S comprised 12 patients without cardiac or other disease, who were followed up for malaise and in whom the basal tilt test was positive, confirming the vagal origin of syncope. Temporal and spectral (total power, low frequency 0.04-0.15 Hz, hight frequency 0.16-0.40 Hz) data was obtained concerning h…

[Present trends in the pathogenesis, diagnosis and therapy of vasovagal syncope].

Vasovagal syncope is the most frequent cause of syncopal conditions. In its pathogenesis autonomic nervous, neurohumoral, cerebrovascular and other mechanisms participate. The gold standard in the diagnosis of vasovagal syncope is in recent years the head up tilt test. In treatment pharmacotherapy, cardiac pacing or their combination can be used.

[Orthostatic hypotension with syncope--a problem of hypertensive therapy in the aged].

In 12 of 32 elderly (> 65 years) hypertensives admitted with syncope, a diagnosis of orthostatic hypotension was made after a positive orthostatic tolerance test and extensive diagnostic evaluation with exclusion of other causes of syncope. Blood pressure of these 12 patients (pts.) after admission was 145 +/- 16/83 +/- 6 mmHg; at that time the pts. were treated with 2.6 +/- 1.6 different antihypertensive agents, and 67% of the pts. received a combination therapy. After reevaluation of treatment the pts. could be discharged with 1.3 +/- 0.7 different antihypertensive agents (significant differ…

The role of electrophysiologic studies in the management of patients with unexplained syncope.

We evaluated the frequency and type of electrophysiologic abnormalities in an unselected population of consecutive patients with unexplained syncope. Fifty patients were entered in the study; all had 24-hour dynamic electrocardiographic (Holter) recordings and underwent complete electrophysiological studies. An abnormal electrophysiologic study was found in 74% of the patients. Sinus node abnormality was observed in 30%, abnormal AV node function in 14%, long HV in 10%, block distal to H during rapid atrial pacing in 6%, paroxysmal supraventricular tachycardia in 12%, ventricular tachycardia/f…
